ABSTRACT

BACKGROUND: Low targeting efficacy and high toxicity continue to be challenges in Oncology. A promising strategy is the glycosylation of chemotherapeutic agents to improve their pharmacodynamics and anti-tumoral activity. Herein, we provide evidence of a novel approach using diglycosidases from fungi of the Hypocreales order to obtain novel rutinose-conjugates therapeutic agents with enhanced anti-tumoral capacity. RESULTS: Screening for diglycosidase activity in twenty-eight strains of the genetically related genera Acremonium and Sarocladium identified 6-O-α-rhamnosyl-ß-glucosidase (αRßG) of Sarocladium strictum DMic 093557 as candidate enzyme for our studies. Biochemically characterization shows that αRßG has the ability to transglycosylate bulky OH-acceptors, including bioactive compounds. Interestingly, rutinoside-derivatives of phloroglucinol (PR) resorcinol (RR) and 4-methylumbelliferone (4MUR) displayed higher growth inhibitory activity on pancreatic cancer cells than the respective aglycones without significant affecting normal pancreatic epithelial cells. PR exhibited the highest efficacy with an IC50 of 0.89 mM, followed by RR with an IC50 of 1.67 mM, and 4MUR with an IC50 of 2.4 mM, whereas the respective aglycones displayed higher IC50 values: 4.69 mM for phloroglucinol, 5.90 mM for resorcinol, and 4.8 mM for 4-methylumbelliferone. Further, glycoconjugates significantly sensitized pancreatic cancer cells to the standard of care chemotherapy agent gemcitabine. CONCLUSIONS: αRßG from S. strictum transglycosylate-based approach to synthesize rutinosides represents a suitable option to enhance the anti-proliferative effect of bioactive compounds. This finding opens up new possibilities for developing more effective therapies for pancreatic cancer and other solid malignancies.

ABSTRACT

Aberrant activation of the Hedgehog (Hh) signaling pathway, through which the GLI family of transcription factors (TF) is stimulated, is commonly observed in cancer cells. One well-established mechanism of this increased activity is through the inactivation of Suppressor of Fused (SUFU), a negative regulator of the Hh pathway. Relief from negative regulation by SUFU facilitates GLI activity and induction of target gene expression. Here, we demonstrate a novel role for SUFU as a promoter of GLI activity in pancreatic ductal adenocarcinoma (PDAC). In non-ciliated PDAC cells unresponsive to Smoothened agonism, SUFU overexpression increases GLI transcriptional activity. Conversely, knockdown (KD) of SUFU reduces the activity of GLI in PDAC cells. Through array PCR analysis of GLI target genes, we identified B-cell lymphoma 2 (BCL2) among the top candidates down-regulated by SUFU KD. We demonstrate that SUFU KD results in reduced PDAC cell viability, and overexpression of BCL2 partially rescues the effect of reduced cell viability by SUFU KD. Further analysis using as a model GLI1, a major TF activator of the GLI family in PDAC cells, shows the interaction of SUFU and GLI1 in the nucleus through previously characterized domains. Chromatin immunoprecipitation (ChIP) assay shows the binding of both SUFU and GLI1 at the promoter of BCL2 in PDAC cells. Finally, we demonstrate that SUFU promotes GLI1 activity without affecting its protein stability. Through our findings, we propose a novel role of SUFU as a positive regulator of GLI1 in PDAC, adding a new mechanism of Hh/GLI signaling pathway regulation in cancer cells.

ABSTRACT

Introducción: La atención prenatal a cada embarazada establece una asistencia médica integral y particularizada, que posibilita determinar y accionar precozmente sobre los factores de riesgo modificables, mediante la vigilancia continua. Objetivo: Describir el comportamiento de la atención prenatal según variables clínico epidemiológicas. Métodos: Estudio observacional descriptivo en el policlínico Pedro del Toro, entre 2014 y 2018. Incluyó 2761 pacientes que representan el total de gestantes en el periodo de estudio que cumplieron los criterios señalados. La información se obtuvo mediante revisión de la base de datos del programa materno infantil, con lo cual se delimitaron las variables que permitieron procesar los datos en una hoja de cálculo EXCEL, utilizando parámetros estadísticos a nivel descriptivo. Resultados: Las gestantes captadas antes de 12 semanas (2442 para 88,44 por ciento) fueron mayoría, primó el rango de 18,9 hasta 25,5 kg/m2 en índice de masa corporal (1657 para 60,01 por ciento). En el parto predominaron gestantes que tenían entre 37 y 41,6 semanas (2531 para 91,66 por ciento) y el rango de peso del recién nacido más relevante fue entre 2500 y 4000 gramos (2443 para 88,48 por ciento). Las edades comprendidas entre 21 a 29 años fueron representativas (1438 para 52,08 por ciento). Conclusiones: De la captación precoz de las embarazadas dependerá, en gran medida, la efectividad de la atención prenatal, a través de la detección oportuna y el seguimiento de todos los elementos que influyen en el bienestar materno fetal(AU)


Introduction: Prenatal care to each pregnant woman establishes a comprehensive and particularized medical assistance, which makes it possible to determine and act early on modifiable risk factors through continuous surveillance. Objective: To describe the characteristics of prenatal care according to clinical-epidemiological variables. Methods: Descriptive and observational study carried out at Pedro del Toro outpatient polyclinic between 2014 and 2018, with 2,761 patients, which represented the total number of pregnant women in the study period who met the indicated criteria. The information was obtained by reviewing the database of the mother and infant program, which permitted to define the variables for processing the data in an Excel worksheet, using statistical parameters at a descriptive level. Results: The pregnant women diagnosed before the twelfth weeks (2442; 88.44 percent) were the majority, with a body mass index range of 18.9 to 25.5 kg/m2 (1657; 60.01 percent). Regarding delivery, pregnant women between 37 and 41.6 weeks predominated (2531; 91.66 percent) and the most relevant newborn weight range was between 2500 and 4000 grams (2443; 88.48 percent). Ages between 21 to 29 years were representative (1438; 52.08 percent). Conclusions: The effectiveness of prenatal care through timely detection and follow-up of all the elements that influence maternal and fetal well-being will depend, to a large extent, on early detection of pregnant women(AU)

RESUMEN: La enfermedad originada por el nuevo tipo de coronavirus, SARS-CoV-2, se ha convertido en un problema de Salud Pública a nivel mundial. Esto ha llevado a posponer las atenciones clínicas electivas de pacientes, exceptuando las atenciones de urgencia o emergencia. Las urgencias odontológicas han continuado con alta demanda en el Sistema Público de Salud durante la pandemia COVID-19, e incluso han aumentado en severidad de los cuadros. Las restricciones de horario y las medidas implementadas a nivel país, llevan a que los pacientes consulten en el Centro de Salud más cercano a su domicilio, es decir, Centro de Salud Familiar (CESFAM) o en los Servicios de Atención Primaria de Urgencia (SAPU). Como Cirujanos Dentistas somos parte de un equipo multidisciplinario de salud, por lo que debemos conocer la enfermedad de COVID-19, sus principales signo s, síntomas y riesgos asociados. El Cirujano Dentista es indispensable en el diagnóstico, tratamiento y derivación oportuna de enfermedades buco-maxilofaciales, las cuales pueden afectar de forma física y psicológica a los pacientes, llegando a producir complicaciones sistémicas graves si no son tratadas oportunamente. La atención odontológica actual debe incluir el manejo previo, durante y posterior al tratamiento del paciente de APS. Debemos considerar que el personal odontológico tiene un elevado riesgo de transmisión viral, debido a la cercanía con fluidos desde la cavidad oral del paciente. En este artículo se entregaran recomendaciones, basadas en la mejor evidencia disponible y la experiencia clínica actual para la atención odontológica de urgencias enfocadas en la Atención Primaria de Salud.


ABSTRACT: The disease caused by the new coronavirus, SARS-CoV-2 has become a world wide public health problem. This has lead to pospone elective clinical care, maintaining urgent and emergency care. Dental emergency maintains high demand, even more severe clinical situations, at the public health system during the pandemic COVID-19. The local restrictions implemented makes patients to consult at the closest health center, like the Family health centers (CESFAM) or at primary health urgency centers (SAPU). As dental surgeons, part of the multidisciplinary health team, it is a duty to know the COVID-19, signs, simptoms and associated risks. The dental surgeon it is essential in the diagnosis, traetment and timely referral of maxilofacial diseases, wich can affect patients physically and psychologically, even leading to serious systemic complications if not treated promptly. Dental care should have previous, during and posterior considerations in pandemic situations. Dental staff has high viral transmisión risk, due to closeness with oral cavity fluids. This article will provide evidence based recommendations and current clinical experience for dental emergency care at primary health centers.

Dr Héctor Javier Sánchez specializes in public health and research methodology and holds a master's degree in epidemiology. He is a senior researcher in the Society, Culture and Health Department at El Colegio de la Frontera Sur, Mexico. The Colegio is a public research institu-tion concentrating on environmental, economic and social issues related to a sustainable future for Mexico's southern border area, and belongs to the National Council of Science and Technol-ogy (CONACYT). In Chiapas State, the country's poorest region and home to many indigenous peoples, Dr Sánchez has carried out studies on TB, poverty and health, domestic violence, hu-man rights, maternal-child health and the effect of agrochemicals on human health.

Dr José Ramón Acosta-Sariego is full professor of basic and preclinical scienc-es at the Medical University of Havana's Victoria de Girón Institute, where he also chairs the Scientifi c Research Ethics Committee. He serves as vice-chair of the Board of Directors of UNESCO's Latin American and Caribbean Bioethics Net-work (REDBioética) and in 2020, UNES-CO's Director-General appointed him to its 36-member International Bioethics Committee. Dr Acosta-Sariego has been academic coordinator for the bioethics master's degree program at the Univer-sity of Havana since its inception in 2006, is president of the Neuroethics Chapter of the Cuban Neurosciences Society and is a member of the Cuban National Bioeth-ics Committee.

ABSTRACT

RESUMEN Cuba ha demostrado en todas las épocas, la grandeza de sus científicos, los avances y logros realizados por ellos. Ya desde los inicios de 1830 se comenzaba a hablar de adelantos en la oftalmología cubana. Desde ese momento nuestros científicos tenían presente el pensamiento filosófico en función de la investigación científica para lograr una tecnología de punta en la especialidad. Al triunfo de la revolución, en 1959, el desarrollo de la oftalmología era casi desconocido. En el presente siglo los servicios oftalmológicos del país disponen de todo lo indispensable para hacer frente al desafío que se avecina constantemente con la tecnología. La Misión Milagro, el mejor ejemplo del conocido impacto de los avances y prestigio de oftalmología en Cuba y otros países, permite afirmar que esta tecnociencia abarata los tratamientos y la atención a los pacientes con independencia de sus estados económicos. Cuba ha demostrado que no solo el capital monetario es importante para el desarrollo social, pues el capital humano eleva la sociedad a nuevas dimensiones. En esto consiste el impacto político de la población, beneficiada por los avances revolucionarios en la oftalmología cubana (AU).


SUMMARY Through the times, Cuba has showed the greatness of its scientists and their advances and achievements. So early as from the beginning of 1830 there were commentaries on the advances in Cuban Ophthalmology and from those times Cuban scientists put the philosophic thought in function of the scientific research to accede to latest technology in the specialty. At the triumph of the Revolution in 1959, the development of Ophthalmology was almost inexistent, but in the current century the ophthalmological services of the country have the essential to affront the constant challenge of new technologies. The results of Mision Milagro, (Miracle Mission, the program organized by Cuban Government to heal ophthalmologic diseases in other countries like Venezuela, Ecuador and others) allow us to affirm that techno-sciences reduce the costs of the patients' treatment and care regardless of their economic status. Cuba has showed that not only the monetary capital is important for the social development: human capital raises society to new dimensions. That is the political impact on the population benefited by the revolutionary advances in Cuban Ophthalmology (AU).

Resumen Para el poshumanismo, las tecnologías eliminarán el envejecimiento, potenciarán la cognición, la fuerza y conservarán la psiquis humana en soportes digitales. Estas ideas, presentes en filmes de ciencia ficción, resultarían útiles para educar en Bioética a estudiantes de medicina. Proponemos algunos de estos filmes que tratan sobre cambios tecnológicos de humanos y sus consecuencias: creación de humanoides por ingeniería genética; esclavos; "mercancía" sin derechos ni dignidad; discriminación por manipulación genética eugenésica; inteligencia artificial que domina a humanos; creación de cyborg para matar; supremacía étnica y suplantación de identidad; inteligencia impersonal; traspaso de conciencia e identidad después de la muerte. El análisis de los filmes ayudaría al profesor a analizar con los estudiantes los límites morales de la ciencia y las tecnologías.


Abstract Posthumanism state that technologies will eliminate aging, improve cognitive, physical and psychological skills and keep alive human minds by uploading them in machines as artificial intelligence. These beliefs, as constant features in science fiction films, could be useful to educate medical students in Bioethics. We propose some of these science fiction films related with technological changes of humans and the noxious consequences: Being turned into a humanoid created by genetic engineering; a slave or a merchandize without rights; discrimination because of eugenesic genetic manipulation; and/or the creation of cyborgs of mass destruction. Also, the movies can show the reader issues of ethnic supremacy dilemmas, identity impersonate and ambition, impersonal intelligence and conscience and identity transpass after death. These films will help teachers to analyze with students the moral limits of science and technologies.


Resumo Para o pós-humanismo, as tecnologias eliminarão o envelhecimento, potencializarão a cognição, a força e conservarão a psique humana em suportes digitais. Essas ideias, presentes em filmes de ficção científica, tornaram-se úteis para educar estudantes de medicina em Bioética. Apresentamos alguns desses filmes que tratam de mudanças tecnológicas de humanos e suas consequências: criação de humanoides por engenharia genética; escravos; "mercadoria" sem direitos nem dignidade; discriminação por manipulação genética eugênica; inteligência artificial que domina os humanos; criação de cyborg para matar; supremacia étnica e substituição de identidade; inteligência impessoal; transferência de consciência e identidade depois da morte. O estudo dos filmes ajudaria o professor a analisar, com os estudantes, os limites morais da ciência e das tecnologias.

RESUMEN Se reportó el caso clínico de una fémina de 50 años de edad. Acudió a los Servicios de Oftalmología, del Hospital Provincial Comandante Faustino Pérez Hernández, de Matanzas. Refirió síntomas irritativos y disminución visual por el ojo izquierdo. El examen oftalmológico mostró en dicho ojo la presencia de lesiones esferoideas de color ámbar, conjuntivo corneales en periferia (horas 3-4 y 9). Se extendían en banda a la córnea central, algunas con aspecto nodular. En el ojo derecho presentaba lesiones similares pero localizadas mayoritariamente en la periferia corneal y conjuntiva horas 3 y 9. Con estos elementos se estableció diagnóstico clínico de degeneración esferoidea, enfermedad degenerativa corneal poco frecuente y generalmente asintomática siempre que no progrese y afecte la visión. Se aplicó como tratamiento una queratectomia superficial, manual, con colocación de una lente de contacto terapéutica en el ojo izquierdo, sin complicaciones. Hubo regresión de los síntomas irritativos y mejoría de la agudeza visual constatada en las consultas de seguimiento. Se recomendó la necesidad de protección solar al constituir las radiaciones ultravioletas un factor de riesgo de importancia en el desarrollo de esta enfermedad.


ABSTRACT The authors reported the case of a woman, aged 50 years, who assisted the Ophthalmology service of the Provincial Hospital "Comandante Faustino Pérez Hernández", of Matanzas. She referred irritant symptoms and left eye visual decrease. The ophthalmologic examination showed the presence of amber-color spheroidal lesions, corneal conjunctives in periphery (3-4 and 9 hours) in that eye. They extended in bands to the central cornea, some of them with nodular aspect. There were similar lesions in the right eye, but they were located mainly corneal and peripheral conjunctive hours 3 and 9. With these elements it was established the clinical diagnosis of spheroidal degeneration, a few frequent corneal degenerative disease that is almost always asymptomatic if it does not progress and affects sight. As a treatment the patient underwent a manual, superficial keratectomy with location of therapeutic contact lens without complications. There it was a regression of the irritant symptoms and an improvement of the visual acuity stated in the follow-up consultations. The patients was recommended sun protection because ultraviolet rays are important risk factors in the development of this disease.

ABSTRACT

RESUMEN Los trastornos de la motilidad ocular constituyen motivo de consulta periódica en Oftalmología. La regeneración aberrante, trastorno muy poco reportado, es considerada la sincinesia oculomotora de mayor invalidez y complejidad. Diversas condiciones neuroftalmológicas están implicadas en la etiopatogenia de la enfermedad, la mayoría de las cuales puede ocasionar la muerte. El manejo de los síntomas y signos provocados por paradójicos movimientos oculares conjugados es difícil. Se reportó un caso con remisión tardía a neuroftalmología por diagnóstico inicial y evolución desfavorable. La historia psicofísica arrojó diagnóstico definitivo de regeneración aberrante del III nervio craneal secundario, a aneurisma cerebral de la carótida interna bilateral, agravado por reanastomosis quirúrgica. Una rigurosa, obligatoria e impostergable historia neuroftalmológica, se impone ante toda parálisis del III nervio craneal para brindar un diagnóstico etiológico preciso y de esta forma proteger la vida (AU).


ABSTRACT The disturbances in ocular motility are the cause of periodical consultation in Ophthalmology. The aberrant regeneration, a scarcely reported disturbance, is considered the oculomotor synkinesis of highest disability and complexness. Several neuro-ophthalmologic conditions are implicated in the disease ethiopathogeny, and most of them could lead to death. The management of the symptoms and signs caused by paradoxical conjugated ocular movements is difficult. A case is reported of late remission to Neuro-ophthalmology due to unfavorable diagnosis and evolution. The psycho-physical history led to a definitive diagnosis of aberrant regeneration of the III secondary cranial nerve, to cerebral aneurism of the bilateral internal carotid, worsened by surgical re-anastomosis. In front of any paralysis of the III cranial nerve, it is necessary a rigorous, obligatory and immediate neuro-ophthalmological history to arrive to a precise etiological diagnosis, protecting life in that way (AU).

ABSTRACT

RESUMEN Los trastornos de la motilidad ocular constituyen motivo de consulta periódica en Oftalmología. La regeneración aberrante, trastorno muy poco reportado, es considerada la sincinesia oculomotora de mayor invalidez y complejidad. Diversas condiciones neuroftalmológicas están implicadas en la etiopatogenia de la enfermedad, la mayoría de las cuales puede ocasionar la muerte. El manejo de los síntomas y signos provocados por paradójicos movimientos oculares conjugados es difícil. Se reportó un caso con remisión tardía a neuroftalmología por diagnóstico inicial y evolución desfavorable. La historia psicofísica arrojó diagnóstico definitivo de regeneración aberrante del III nervio craneal secundario, a aneurisma cerebral de la carótida interna bilateral, agravado por reanastomosis quirúrgica. Una rigurosa, obligatoria e impostergable historia neuroftalmológica, se impone ante toda parálisis del III nervio craneal para brindar un diagnóstico etiológico preciso y de esta forma proteger la vida.


ABSTRACT The disturbances in ocular motility are the cause of periodical consultation in Ophthalmology. The aberrant regeneration, a scarcely reported disturbance, is considered the oculomotor synkinesis of highest disability and complexness. Several neuro-ophthalmologic conditions are implicated in the disease ethiopathogeny, and most of them could lead to death. The management of the symptoms and signs caused by paradoxical conjugated ocular movements is difficult. A case is reported of late remission to Neuro-ophthalmology due to unfavorable diagnosis and evolution. The psycho-physical history led to a definitive diagnosis of aberrant regeneration of the III secondary cranial nerve, to cerebral aneurism of the bilateral internal carotid, worsened by surgical re-anastomosis. In front of any paralysis of the III cranial nerve, it is necessary a rigorous, obligatory and immediate neuro-ophthalmological history to arrive to a precise etiological diagnosis, protecting life in that way.

ABSTRACT

Actualmente el trabajo educativo y las acciones de extensión no estimulan la apreciación estética y el disfrute del arte como parte de los conocimientos y habilidades específicas en los estudiantes, con lo que se articularían dos procesos centrales de la gestión universitaria. Se presenta un modelo para incorporar la apreciación artística en las asignaturas de la carrera de Medicina como complemento del proceso enseñanza-aprendizaje. A partir de los programas de las principales asignaturas de esta carrera y de acuerdo con los contenidos, se seleccionaron obras de arte que pueden ser utilizados como ejemplos para incorporarlas en el proceso enseñanza-aprendizaje sirviendo de elementos para la motivación y el aprendizaje, conjugando las funciones docentes con las extensionistas. Las obras de arte se ajustan a las asignaturas del ciclo básico, preclínico y clínico como es el caso de la Morfofisiología, Agentes Biológicos, Anatomía Patológica, Psicología, Medicina Interna, Cirugía, Pediatría, Ginecobstetricia, Ortopedia, Otorrinolaringología, y Medicina Legal entre otras. Se estimula a los profesores a seleccionar sus propias obras, a orientar trabajos independientes por asignaturas que sean presentados en jornadas estudiantiles y de extensión universitaria y a trabajar sus nexos con los contenidos. La inclusión de obras de arte en el proceso enseñanza-aprendizaje favorecería el rescate de la formación cultural y el gusto por el arte en los estudiantes y profesores de la carrera de medicina(AU)


Nowadays educative work and university extension do not stimulate aesthetic appreciation and artistic enjoyment as a part of knowledge and specific cleverness in students, to coordinate two central processes of university management. There is a model to incorporate the encouragement of artistic appreciation in medical subjects as a complement of teaching-learning process. They were revised programs of main subjects of medical career, selecting works of art as example according contents of each one, to incorporate them in teaching-learning process, useful for motivation and combining teaching and extension functions. They were show works of art for basic, preclinic and clinic lectures as Morphophysiology, Microbiology, Pathology, Psychology, Internal Medicine, Surgery, Pediatrics, Gynecology and Obstetrics, Orthopedic, Otorhinolaryngology, and Legal Medicine among others, encouraging teachers to choose their preferred works of arts, to give students guidance for extra class works to be present in scientific and university extension conferences. Including works of arts in teaching-learning process will help recovery cultural education and pleasure for the enjoyment of art in students and professors of medicine career(AU)
