Manía inducida por el tratamiento con antidepresivos. A propósito de un caso / Mania induced by antidepressant treatment. A case study

La incidencia de manía e hipomanía en personas que reciben terapia farmacológica frente a la depresión constituye un efecto adverso del tratamiento con antidepresivos. Sin embargo, no queda claro si los antidepresivos causan manía aguda o hipomanía en pacientes con depresión unipolar o desencadenan la expresión de un trastorno bipolar subyacente. Los estados de manía aguda han sido especialmente asociados al tratamiento con antidepresivos tricíclicos (ATC), inhibidores de la monoaminooxidasa A (IMAO) y antidepresivos de acción dual como la venlafaxina. A continuación, describimos el caso clínico de una mujer de 78 años, sin antecedentes psiquiátricos previos, que había iniciado un tratamiento antidepresivo mediante dosis ascendentes de sertralina tras un episodio de ánimo bajo reactivo a un acontecimiento vital estresante. Seguidamente, presentó manifestaciones clínicas características de un episodio maniaco probablemente inducido por el tratamiento antidepresivo mencionado, dada la coincidencia temporal. Durante su estancia en la Unidad, se suspendió el tratamiento con sertralina y se pautó quetiapina de forma progresiva, mejorando el contacto afectivo, disminuyendo la inquietud previa y desapareciendo la desorientación inicial (AU)

The occurrence of mania and hypomania in people receiving antidepressant therapy is an adverse effect of treatment with this type of medication. However, it is unclear whether antidepressants cause acute mania or hypomania in patients with unipolar depression or trigger the expression of an underlying bipolar disorder. Acute mania has been particularly associated with tetracyclic antidepressants (TCA), monoamine oxidase inhibitor (MAOI), and dual-action antidepressants, such as venlafaxine. A description is presented on the clinical case of a 78 year-old woman, with no previous psychiatry history, who had started on sertraline after a low mood episode, probably caused by a stressful life event. She later presented symptoms of a manic episode, likely induced by antidepressant treatment according to the timing. During her stay in the Unit, the sertraline treatment was suspended and the patient received increasing doses of quetiapine, which improved the affective contact, diminishing her previous anxiety and making the initial disorientation disappear (AU)

Relaciones estructurales y funcionales del sistema de tareas comunicativas para Vision One en Estomatología 1er año / Structural and functional relations of a system of communicative tasks for Vision One in 1st academic year of Dentistry studies

Con el objetivo de elaborar las relaciones estructurales y funcionales del sistema de tareas comunicativas en lengua inglesa previamente elaborado para el curso Vision One de 1er. año de Estomatología de la Universidad de Ciencias Médicas de Pinar del Río, se realizó el análisis documental del libro de texto de los estudiantes, los informes departamentales y resultados de los exámenes, las actas de las reuniones de colectivos, los informes de los controles a clase, la experiencia del colectivo y la propia experiencia del autor, que permitieron que se constataran la existencia de los problemas y deficiencias en los materiales de enseñanza y aprendizaje utilizados en la dirección del proceso de enseñanza-aprendizaje y en los niveles de preparación alcanzados por los estudiantes, especialmente en el desarrollo de la habilidad de expresión oral; resultados que distan de las expectativas y necesidades sociales, lo cual motivó la realización de esta investigación de tipo descriptivo-explicativa. A través de la unidad 2 del curso Vision One, se refirieron y ejemplificaron las relaciones estructurales y funcionales que debe poseer dicho sistema de tareas comunicativas en Lengua Inglesa.

This research paper was aimed at preparing the structural and functional relations of a system of communicative tasks in English Language (Vision One) to the 1st academic year of Dentistry at Medical University in Pinar del Rio. A documentary analysis of the student's text book, reports of the English department, minutes from the meetings of the teaching staff, results of tests, controls to lessons; experience of the group of professors and of the author's own experience were as well examined, where the existence of problems arose, and observing deficiencies in the teaching and learning materials used in the direction of the teaching-learning process along with the levels of knowledge reached by the students, particularly in the development of oral skills. Results were far from the prospects and social needs, which motivated the implementation of this descriptive-explanatory research. By means of the analysis of Unit-2 of Vision One course, the structural and functional relations that this system of communicative tasks in English Language must contain, were referred and exemplified.
