ABSTRACT

BACKGROUND: A particular feature of dissociated strabismus is that the frequency of presentation and its magnitude is variable. It is common to find that in certain examinations is very obvious, and in others not so much. To facilitate exploration some maneuvers are described: Bielchowsky's maneuver, Bielchowsky's phenomenon and Posner test. OBJECTIVE: to compare the presence and magnitude of the ocular movement in dissociated strabismus in Posner's Test using opaque versus translucent occluders. METHODS: Prospective, transversal, descriptive study. Patients with diagnosis of were included. Posner's test in two ways: 1) With opaque occluders, and 2) with translucent occluders was perforned. RESULTS: Ninety-five percent of the patients presented horizontal deviation. Posner's test with opaque occluder was positive in all cases (100%). Posner's test with translucent occluders was positive in 94% of the patients (p= 8). The magnitude of the ocular movement was significantly smaller using translucent occluders than opaque occluders. (p= 0.0001). CONCLUSIONS: The Posner's test using opaque occluders was more efective to demonstrate clearly and precisely the size of the ocular movement in dissociated strabismus.


Antecedentes: una característica particular del estrabismo disociado es la variabilidad en la magnitud y en la frecuencia de presentación. En ciertas exploraciones es muy evidente y en otras no tanto. Para facilitar su exploración se han descrito algunas maniobras: de Bielchowsky, del fenómeno de Bielchowsky y la prueba de Posner. Objetivo: determinar la magnitud del movimiento ocular en el estrabismo disociado al aplicar la maniobra de Posner y comparar dos modalidades de ésta: oclusores opacos y oclusores translúcidos. Material y métodos: estudio prospectivo, transversal, descriptivo y observacional efectuado en pacientes con estrabismo disociado a quienes se realizó la maniobra diagnóstica de Posner en dos modalidades: 1) con oclusores opacos, y 2) con oclusores translúcidos. Resultados: la maniobra de Posner con oclusores opacos resultó positiva en 100% de los casos. Al utilizar oclusores translúcidos la prueba diagnóstica resultó positiva en 94% de los casos (p= 0.8). La magnitud del movimiento de recuperación fue considerablemente menor con el oclusor translúcido comparado con el oclusor opaco (p= 0.0001). Conclusión: la maniobra de Posner con oclusores opacos demostró ser más efectiva para mostrar en forma más clara y precisa la magnitud del movimiento en el estrabismo disociado al compararlo con oclusores translúcidos.

ABSTRACT

Since the clinical implementation of fine needle aspiration cytology (FNAC) to diagnose thyroid carcinoma, few patients remain misdiagnosed and little is known about their clinical outcomes. An observational retrospective study was carried out to analyse prognostic factors and follow-up of patients with differentiated thyroid carcinoma (DTC) not disclosed by FNAC before surgery, compared to a control group. From October 2003 to July 2010, 308 patients underwent surgery as treatment for nodular goitre and 53 had DTC. Cases were 12 subjects with DTC and benign (n = 7) or nondiagnostic (n = 5) FNAC. Controls were 39 subjects with DTC and suspicious (n = 19) or malignant (n = 20) FNAC. Prognostic factors, recurrence and survival rates were compared. Cases had longer time from FNAC to surgery than the control group (86.8 ± 74.1 vs. 16.4 ± 23.8 weeks; P < 0.001), higher prevalence of follicular carcinoma (33.3 vs. 2.6%; P = 0.009), and of two-time total thyroidectomy (75 vs. 30.8%; P = 0.016). Average follow-up was 42.7 ± 25.3 months (2-86 months). There were no deaths. Disease-free survival for cases was 66.9 ± 5.8 months, and for controls 78.7 ± 3.9 months (P: ns). In patients with DTC, the result of the FNAC performed before surgery was not an independent predictor of recurrences or mortality in the first 7 years of follow-up. Thus, false negative or nondiagnostic FNAC in a patient with DTC does not seem to be a primary prognostic factor, but it may reveal other adverse prognostic factors such as longer time to therapy and higher prevalence of follicular carcinoma that may influence long-term outcomes.

ABSTRACT

El siguiente reporte de caso nos brinda el seguimiento de una paciente B/F de 58 años de edad que acude a consulta por presentar trastornos dispépticos de seis meses de evolución acompañado de síndrome general y anemia ligera. Se realizó Panendoscopía del Tracto Digestivo Superior, observándose una lesión ulcerada en incisura angular con características de malignidad, se concluyó endoscópicamente como probable Neoplasia Gástrica Ulcerada, con posterior diagnóstico anatomopatológico de Adenocarcinoma Gástrico de células en Anillo de Sello. La paciente fue intervenida quirúrgicamente en nuestro Centro, confirmándose dicho diagnóstico con el estudio de la pieza quirúrgica.


The following case report gives us a follow up of a white, female 58 years-old patient, coming to a consultation for presenting dyspeptic disturbances of six-months evolution together with general syndrome and light anemia. We made panendoscopy of the High Digestive Tract, finding an ulcerated lesion in angular incision with malignant characteristics, endoscopically concluded as probably Ulcerated Gastric Neoplasia, with subsequent anatomopathologic diagnostic of Gastric Adenocarcinoma of signet-ring cells.The patient was operated in our hospital, confirming the diagnosis with the study of the surgical piece.

Durante un brote epidémico de trasmisión hídrica de hepatitis viral A en el área de Versalles, Matanzas, ocurrido entre octubre 2006 y marzo 2007 se incluyó con diagnóstico presuntivo de esta enfermedad al paciente P.A.P M/B, de 45 años de edad, que ingresó en el Servicio de Medicina Interna del Hospital Provincial José Ramón López Tabrane por presentar además de los síntomas típicos, un síndrome febril prolongado; como dato de interés se recogió su dedicación al cuidado de aves de corral. Se realizaron estudios virológicos para virus A,B,C y E los cuales resultaron negativos, el resto de los complementarios confirmaban lesión hepática por lo que se indicó laparoscopia con biopsia hepática y se llegó al diagnóstico de histoplasmosis hepática. Se comenzó tratamiento con Itraconazol con evolución clínica satisfactoria y regresión de las lesiones hepáticas.


During an epidemic sprout of hydric transmission of hepatitis A occurred in Matanzas, Versalles area., between October 2006 and March 2007, the patient P.A.P male, white of 45 years old, which one entered in Medicines Service of the José Ramón López Tabrane Hospital was included with presumptive diagnosis of this disease. The patient presented in addition to typical symptoms, a feverish prolonged syndrome and as fact of interest gathered his dedication to care of poultry bird. The virological studies for virus A, B, C, and E proved to be negative, the rest of the complementary were confirming hepatic lesion, thats why laparoscopia with hepatic biopsy was indicated and it took place the diagnosis of hepatic histoplasmosis. Treatment began with Itraconazol with clinical satisfactory evolution and regression of hepatic injuries.

ABSTRACT

El siguiente reporte de caso se trata de un paciente blanco, masculino, de 68 años de edad, que presentaba un cuadro clínico dado por marcado decaimiento y somnolencia de 6 meses de evolución asociado a la presencia de anemia ferripriva y Sangramiento Digestivo Alto a repetición, con la posterior aparición de una masa abdominal palpable en hipocondrio izquierdo y empeoramiento del estado general del paciente. Todo esto indujo a plantear la presencia de un Leiomiosarcoma Gástrico como diagnóstico presuntivo sin descartar la existencia de un Tumor Estromal, para lo cual fue necesario realizar estudio inmunohistoquímico, confirmándose el diagnóstico.


The following case report is about a white, male patient of 68 years old, that presented a clinical manifestation given marked dejected and sleepiness of 6 months of evolution correlated to the presence of iron deficiency anemia and upper gastrointestinal bleeding to repetition, with the posterior appearing of an abdominal palpable mass in left hypochondrium and aggravation of the general condition of the patient. This clinical manifestation induced to discuss the presence of a Gastric Leiomyosarcoma like presumptive diagnostic without discarding the existence of a gastrointestinal stromal cell tumors (GIST), for which one it was necessary to accomplish immunohistochemistry study for after confirmed the diagnosis.
