ABSTRACT

The authors share their experiences with diagnosing and surgical treatment of chronic postoperative osteomyelitis of sternum developed in 36 patients after operations on the heart, lungs and organs of mediastinum. The purulent process can be liquidated in all the patients due to performing saving resection of the sternum and plasty of the bone defect with local tissues, application of a gauze-cotton roll pressing the subcutaneous fat into the bone defects formed after treatment of the affected portions of the sternum, administration of antibiotics according to sensitivity of the microflora.

ABSTRACT

The results of 8936 abdominal ultrasonographies were analysed. Primary hepatic cancer was detected in 27 (0.3%) patients of whom 8 (28.6%) showed concomitant chronic hepatitis or cirrhosis. Ultrasonographic signs of primary carcinoma as well as their clinical features were evaluated. The reliability of ultrasonography proved 85.7% which assures better treatment and ability assessment.

ABSTRACT

The number of operated on patients who are referred to a commission for expert medical assessment of working capacity is growing at present. Correct expert assessment of the working capacity allows the results of surgery to be reliably consolidated and promotes restoration of working ability. The duration and severity of working ability. The duration and severity of the pulmonary abscess, the method of surgical treatment, the lapse of time after the operation, the course of the restorative processes, complications and concomitant diseases, the degree or respiratory and circulatory insufficiency, the patients' age, profession, and the conditions and character of work are taken into account during examination. Timely and complete treatment and rational choice of occupation are important factors in the prevention of invalidism and enable most patients (86.8%) to resume work in full volume or with limitations as specified by the commission.

ABSTRACT

Treatment and analysis of immediate results were described in 269 patients with chronic empyema of the pleura as well as their long-term results in 207 of them. Under consideration were main criteria of the examination of labor capacity in such patients. It was shown that the establishment of the phase of "compensation at the limit" (IIa degree of pulmonary insufficiency) and subcompensation (IIb degree of pulmonary insufficiency) allowed more exact evaluation of the degree of compensatory processes of respiratory organs in patients after operation and the degree of loss of their working capacity.

ABSTRACT

Out of 154 patients subjected to a wide opening of the residual cavity 138 patients recovered (89,6%), 11 patients died (7,2%), empyema was not liquidated in 5 patients (3,2%). All the 115 patients who were treated by punctures were cured.

ABSTRACT

Postoperative chondritis and osteomyelitis of the ribs and sternum were observed in 90 of 4620 operated patients. The main causes contributing to the development of this complication are errors in treatment of cartilages during thoracotomy and suppurative complications in the pleural cavity or in the operative wound. A radical removal of the affected costal and sternal portions as early as possible is considered to be the method of choice in treatment of chondirtis and osteomyelitis of the ribs and sternum.
