ABSTRACT

In Russia, in conditions of reproductive crisis of modern family, miscarriage IS one of the important problems, in particular habitual miscarriage as significant risk factor of development of maternal and perinatal pathology. The timely identification and medical care support of pregnant women suffering from habitual miscarriage is essential in ensuring and maintaining their reproductive health. To obtain reliable and objective information by control boards on effectiveness of organization and provision of obstetric care to women with diagnosis of habitual miscarriage is an actual task. The purpose of the study is to develop information support of management of organization and provision of medical care of women with habitual miscarriage, based on identification and use of specific risk factors of development and prevention of this pathology.

ABSTRACT

The article considers main concepts and clauses of contract of employment with medical personnel of state institutions of health care. The recommended examples of indices and criteria of functioning efficiency assessment based on working functions of various personnel categories on the basis of analysis of practical application of effective contract in the Russian Federation are demonstrated. The possibilities and risks related to implementation of effective contract into functioning of multi-field health care institution as a technique of stimulating the increase of quality of medical care and efficiency of functioning of institution.

ABSTRACT

OBJECTIVE: To develop a rational surgical strategy for acute biliary pancreatitis and its complications. MATERIAL AND METHODS: A 10-year follow-up enrolled 378 patients with acute biliary pancreatitis. Mild pancreatitis was diagnosed in 304 (80%) patients, moderate and severe course - in 74 (20%). Almost all patients with mild acute biliary pancreatitis underwent surgery within 3-7 days after the attack resolution. Patients with severe biliary pancreatitis had general and local complications that required differentiated treatment strategy. CONCLUSION: Strangulation of the calculus in the major duodenal papilla requires emergency endoscopic papillosphincterotomy. Choledocholithiasis, cholangitis and obstructive jaundice in acute biliary pancreatitis are indications for endoscopic papillosphincterotomy. Cholecystectomy should be performed after resolution of inflammatory changes in the gallbladder and pancreas in patients with severe biliary pancreatitis.

ABSTRACT

The article considers the dynamics of planned-regulatory data concerning obstetrics gynecological care and obstetrics business against the background of alteration of common indices of volume of medical care in out-patient and in-patient and in day-stay hospitals. The conclusions are made about necessity of significant modifications in planning of obstetrics gynecological care. The proposed volumes of specialized medical care in hospital conditions according corresponding profiles are presented within the framework of the territorial program of 2018 and азк planned period 2019-2020.

ABSTRACT

The publication presents the stages of activities on standardization of work quota setting in conditions of efficient contract. The article is devoted to particular organizational aspects occurring in the course of analysis of modern labor legislation and other normative legal documents concerning labor in the field of health care. The approaches are considered related to to development of system of work quota setting in medical organization, definition of labor functions and functional duties of medical workers, composition of staffing table of medical organization reflecting scope of work and possibilities of observing indices of qualitative performance.

ABSTRACT

The prevalence of habitual miscarriage varies depending on territories of the Russian Federation and other countries. The rate of premature delivery in the Moscow Zelenograd administrative okrug in 2013-2015 in average made up to 4.5%-4.7%. The percentage of habitual miscarriage made up to 3.6% that is a rather low indicator as compared with other Moscow okrugs and national average indices. The article considers three-level model of evaluation of medical, economic and social efficiency of obstetric gynecologic care support of population at the out-patient stage.
