ABSTRACT

The authors report a case of encephalocraniocutaneous lipomatosis (ECCL) in a female infant. Clinical manifestations in this case showed significative overlap with another neurocutaneous syndrome, namely oculocerebrocutaneous (OCC) Delleman's syndrome. This may support the theory of somatic mosaicism. The patient underwent a cerebrospinal fluid (CSF) shunt procedure due to marked tetraventricular hydrocephalus that was not present at birth. The need for close and multidisciplinary follow-up of these patients is stressed.

ABSTRACT

A case of thrombosed aneurysm of the vein of Galen associated with superior sagittal sinus thrombosis is reported. Clinical course was characterized by multiple intraparenchimatous hemorrhages and hydrocephalus. NMR showed dural sinuses thrombosis better than CT even though cerebral angiography was more effective for the diagnosis. Intracranial hypertension due to hydrocephalus was relieved with a ventriculoperitoneal shunt. The authors suggest that the abnormal hemodynamic patterns due to the drainage of the malformation into the vein of Galen and dural sinuses led to venous occlusion and multiple intracranial hemorrhages.

ABSTRACT

The Authors report an analysis on 40 cases of craniocerebral gunshot wounds treated in a civil hospital over a 8-year period. The important role of CT for a correct diagnosis and treatment planning is stressed even though patients with a G.C.S. lower than 4 die regardless of their CT findings; subdural and intracerebral hematomas are not a serious complication unless patient's neurological status is poor; timing of surgical treatment plays a major role in order to avoid infection of the wound.

ABSTRACT

The Authors report 8 cases of Failed Lumbar Disk Syndrome (FLDS), on 210 patients operated for slipped disk. Risk factors and the concept that an abnormality involving either neurons in the substantia gelatinosa or internucial fibers in Lissauer's tract is responsible for pain in patients with FLDS are discussed. Since iatrogenic deafferentation pain response to almost all current forms of therapy is generally poor, the most rational approach remains prevention.

ABSTRACT

The Authors report their results on the treatment of 10 cases of primary spinal epidural lymphoma. All patients underwent surgery followed by radiotherapy and 60% of them improved after treatment. The Authors stress the importance the preoperative neurological status and early diagnosis play on the prognosis. They conclude that only further studies on a large series can determine the influence of the histological type of the neoplasm on prognosis.

ABSTRACT

In the recent years an increasing attention has been focused on the systolic time intervals (STI) as reliable indicators of left ventircular contractile performance. As regards the diagnostic usefulness of STI in old persons only few papers can be found in the literature, all of them being concerned with subjects aged less than 90 years. With the aim to assess the usefulness and the reliability of the method and the caracteristics of left ventricular function in the tenth decade, STI as well as heart volume (HV) were determined on a group of 35 subjects aged 90 years and over (mean age 93.2 years) normotensive and free from clinical evidence of heart disease. STI (PEPI, LVETI, QS2I, PEP/LVET) values showed no differences with those considered as normal. A marked increase was recorded for the HV values. These results suggest that in the very advanced age a good level of left ventricular contractile performance can be mantained by the compensatory mechanism represented by heart dilation according to the Frank Starling principle.
