ABSTRACT

This report highlights survival and the patient's perspective after prolonged venovenous extracorporeal membrane oxygenation (ECMO) for COVID-19-related respiratory failure. A 36-year-old man with COVID-19 presented with fever, anosmia, and hypoxia. After respiratory deterioration necessitating intubation and lung-protective ventilation, he was referred for ECMO. After 3 days of conventional venovenous ECMO, he required multiple creative cannulation configurations. Adequate sedation and recurrent bradycardia were persistent challenges. After 149 consecutive days of ECMO, he recovered native lung function and was weaned from mechanical ventilation. This represents the longest-duration ECMO support in a survivor of COVID-19 yet reported. Necessary strategies included unconventional cannulation and flexible anticoagulation.

ABSTRACT

El problema de la dependencia a drogas y sus implicaciones como problema de salud, ha alcanzado en los últimos años una significación especial en la medida que traspasa los límites de la salud y penetra en otras áreas como son lo social, lo político, lo económico y otras más, relacionadas con el devenir del género humano. En el paciente adicto comienzan a aparecer determinados cambios cognitivos debido al uso y abuso de estas sustancias. Cambios que en muchas ocasiones no se perciben como resultantes de la adicción.Por tanto, nos propusimos con el estudio describir el funcionamiento de los procesos cognitivos en un grupo de pacientes adictos a drogas ilícitas. Se utilizó una perspectiva metodológica cualitativa en un grupo de casos múltiples aplicados. Obtuvimos como resultados principales que en nuestro grupo primaban los pacientes adictos al crack y psicofármacos. En las pruebas psicológicas los pacientes que dieron más índice de organicidad fueron los adictos a la cocaína. Los más afectados en el proceso cognitivo de memoria fueron los adictos al crack. La totalidad de la muestra presenta igual afectación a nivel de pensamiento y todos los pacientes estudiados presentan igual déficit de atención(AU)


The problem of drug dependence and its implications as a health problem, has achieved in recent years a special significance far beyond the limits of health and into other areas such as social, political, economic and others, related to the evolution of mankind. In the addicted patient start to show some cognitive changes due to the use and abuse of these substances. Changes that often are not perceived as resulting from addiction.Therefore, we decided to study describing the operation of cognitive processes in a group of patients addicted to illicit drugs. A qualitative methodological perspective in a group of multiple cases applied was used. Main results obtained in our group prevailed patients addicted to crack cocaine and psychotropic drugs. In psychological testing patients given rate of organicity were addicted to cocaine. The most affected cognitive memory process were crack addicts. The entire sample has equal involvement level of thought and all patients studied showed equal attention deficit(AU)
