ABSTRACT

BACKGROUND: The aim of this study is to evaluate morbidity and mortality in patients taken to conversion to open procedure (CO) and subtotal laparoscopic cholecystectomy (SLC) as bailout procedures when performing difficult laparoscopic cholecystectomy. METHOD: This observational cohort study retrospectively analyzed patients taken to SLC or CO as bailout surgery during difficult laparoscopic cholecystectomy between 2014 and 2022. Univariable and multivariable logistic regression models were used to identify prognostic factors for morbimortality. RESULTS: A total of 675 patients were included. Of the 675 patients (mean [SD] age 63.85 ± 16.00 years; 390 [57.7%] male) included in the analysis, 452 (67%) underwent CO and 223 (33%) underwent SLC. Overall, neither procedure had an increased risk of major complications (89 [19.69%] vs 35 [15.69%] P.207). However, CO had an increased risk of bile duct injury (18 [3.98] vs 1 [0.44] P.009), bleeding (mean [SD] 165.43 ± 368.57 vs 43.25 ± 123.42 P < .001), intestinal injury (20 [4.42%] vs 0 [0.00] P.001), and wound infection (18 [3.98%] vs 2 [0.89%] P.026), while SLC had a higher risk of bile leak (15 [3.31] vs 16 [7.17] P.024). On the multivariable analysis, Charlson comorbidity index (odds ratio [OR], 1.20; CI95%, 1.01-1.42), use of anticoagulant agents (OR, 2.56; CI95%, 1.21-5.44), classification of severity of cholecystitis grade III (OR, 2.96; CI95%, 1.48-5.94), and emergency admission (OR, 6.07; CI95%, 1.33-27.74) were associated with presenting major complications. CONCLUSIONS: SLC was less associated with complications; however, there is scant evidence on its long-term outcomes. Further research is needed on SLC to establish if it is the safest in the long-term as a bailout procedure.

ABSTRACT

Acute pancreatitis is the recurrent reason for gastrointestinal admission in a clinical urgent setting, it happens secondary to a wide array of pathologies out of which biliary disease stands as one of the most frequent causes for its presentation. Approximately 20% of pancreatitis are of moderate or severe severity. Currently, there is not a clear recommendation on timing for cholecystectomy, either early or delayed. CHISPA is a randomized controlled, parallel-group, superior clinical trial. An intention-to-treat analysis will be performed. It seeks to evaluate differences between patients taken to early cholecystectomy during hospital admission (72 hours after randomization) versus delayed cholecystectomy (30±5 days after randomization). The primary endpoint is major complications associated with laparoscopic cholecystectomy defined as a Clavien-Dindo score of over III/V during the first 90 days after the procedure. Secondary endpoints include recurrence of biliary disease, minor complications (Clavien-Dindo score below III/V), days of postoperative hospital stay, and length of stay in an intensive therapy unit postoperatively (if it applies). The CHISPA trial has been designed to demonstrate that delayed laparoscopic cholecystectomy reduces the rate of complications associated to an episode of severe biliary pancreatitis compared to early laparoscopic cholecystectomy.Trial registration number: NCT06113419.

ABSTRACT

INTRODUCTION AND IMPORTANCE: Acute cholecystitis is responsible for 44 % of emergency admissions to the emergency services with multiple complications such as empyema a necesitatis (EN). EN has a close relation with cholecystitis when the perforation of the gallbladder (GB) can lead to the formation of a biliary fistula. Patients can be asymptomatic, with late consultations, thus being a diagnostic challenge. Different techniques are described for cholecystitis and secondary abscess, therefore, the choice of the appropriate procedure should be the best one to reduce the high associated morbidity. CASE PRESENTATION: We present a case of an 89-year-old patient, admitted for a sensation of a mass in the right hypochondrium with abdominal pain. He was taken to the operating room, finding a vesicular plastron with piocholecyst and perforation into the abdominal wall with abscess and fasciitis. Subtotal cholecystectomy was performed laparoscopically and an open approach in the abdominal wall, drainage of the abscess and debridement, leaving a negative pressure system. CLINICAL DISCUSSION: EN affects elder patients with high rates of morbidity, also GB empyema, which is related with its perforation and posterior fistulization, its external spontaneous perforation is much less frequent. Fistulas originated from the biliary tract are well described in the literature, with low incidence. They are related with improved diagnostic investigations and earlier implemented treatment by antibiotics and surgery. CONCLUSION: Biliary EN represents a very unusual complication of acute cholecystitis, its atypical presentation represents a diagnostic challenge, with very few cases documented and high mortality rates. Its management represents a challenge for the general surgeon, finding different approaches and surgical behaviors to take.

ABSTRACT

INTRODUCTION AND IMPORTANCE: Paraneoplastic neurologic syndromes are a group of neurologic disorders that can affect any part of the nervous system and are mediated by immune pathogens produced by cancer. These disorders occur distant to a malignant tumor and are not caused by metastasis, nutritional disorders or side effects of therapy related to the tumor. CLINICAL FINDINGS: We present the case of a 47-year-old male patient who was admitted to the emergency department due to 1 month of neurological impairment including generalized tonic-clonic movements. He was admitted to the institution and was taken to multiple neurologic tests, all of which were normal, including a negative panel for onconeural antibodies. He persisted with seizures and was taken to a 24-hour video electroencephalogram which showed features consistent with moderate encephalopathy and focal epileptiform activity, which evolved into status epilepticus. Suspecting immune - mediated encephalitis, a therapeutic trial was started with methylprednisolone and plasma exchange, and a positron emission tomography was indicated. The positron emission tomography showed in the brain regions of marked hypometabolism and hypermetabolic thickening of gastric infiltrative aspect fundocorporal topography. Upper gastrointestinal endoscopy revealed in the subcardial region a mass-like lesion with an ulcer-infiltrative appearance, pathology showed an adenocarcinoma. CONCLUSION: Autoimmune encephalitis as a paraneoplastic neurological syndrome of a gastric adenocarcinoma have been documented in few patients in the literature. It is important to describe and recognize clinical findings in this cases to be able to suspect malignancy and thus have early diagnosis, start treatment promptly and avoid irreversible neurological sequelae.

ABSTRACT

Este artículo analiza, desde la perspectiva del principio de justicia distributiva, los procesos de obtención de componentes anatómicos para trasplante. Se examinan, a partir de la revisión de la literatura, la consulta de fuentes de datos estadísticos y el recurso de narrativas fílmicas, documentales y ficcionales, algunos factores que inciden en la brecha existente entre la oferta y demanda, el fenómeno del turismo de trasplantes, la obtención forzada de componentes anatómicos y algunos de los mitos divulgados por medios de comunicación que impactan negativamente la donación de órganos, tanto a nivel global como en el contexto del Sistema de Salud en Colombia. Se examinan también las nociones de justicia distributiva (equidad y utilidad) que legitiman los criterios de obtención, distribución y asignación de órganos y tejidos, partir de la categorización de estos bio-objetos como bienes sociales


This chapter analyzes, from the perspective of the principle of distributive justice, the processes of obtaining anatomical components for transplantation. Based on the review of the literature, the consultation of sources of statistical data and the use of film narratives are examined, some factors that affect the gap between supply and demand, the phenomenon of transplant tourism, forced obtaining of anatomical components and some of the myths disclosed by the media that negatively impact organ donation, both globally and in the context of the Health System in Colombia. The notions of distributive justice (equity and utility) that legitimize the criteria for obtaining, distributing and assigning organs and tissues are also examined, based on the categorization of these bio-objects as social goods

ABSTRACT

Justificación: El personal de Enfermería juega varios e importantes roles dentro de los equipos multidisciplinarios de terapia nutricional (EMTN) relacionados con la implementación, administración y supervisión de la terapia nutricional (TN). Sin embargo, no se tienen documentos sobre las prácticas corrientes de Enfermería en las TN que se administran en los países de América Latina (AL). Objetivo: Documentar el estado actual de las prácticas de Enfermería comprendidas dentro de las TN conducidas en los países AL. Diseño del estudio: Transversal, descriptivo. Locación del estudio. Cincuenta hospitales de las ciudades-capitales de 8 países de AL. La encuesta se desarrolló durante el mes de Noviembre del 2019. Métodos: Mediante una encuesta anónima distribuida sobre la plataforma Survey Monkey™ (SurveyMonkey LLC, Palo Alto, California, Estados Unidos) entre los profesionales en Enfermería en los hospitales participantes se documentó la existencia de un EMTN (u otra forma de organización hospitalaria), la dedicación exclusiva de los integrantes del mismo a la provisión de la TN, la existencia de legislaciones sobre la provisión de TN, las actividades que realiza el personal de Enfermería dentro del EMTN, la existencia de protocolos propios para la evaluación nutricional del enfermo, la administración de la TN, y la educación nutricional del enfermo y los familiares al alta hospitalaria; el monitoreo de los indicadores de calidad de la TN, el gerenciamiento del riesgo relacionado con la TN; y la formación local en temas de TN. Resultados: El 95 % de las instituciones participantes cuenta con un EMTN, pero el 55 % de los encuestados se dedica en forma exclusiva a la provisión de TN. La mitad de los encuestados conocía las legislaciones nacionales existentes sobre la TN. Rationale: Nursing personnel plays several and important roles within the nutritional therapy multidisciplinary teams (NTMT) regarding implementation, management and supervision of nutritional therapy (NT). However, current Nursing practices in nutritional therapy (NT) administered in Latin American (LA) countries are not well documented. Objective: To document the current state of Nursing practices followed with NT administered in LA countries. Study design: Cross-sectional, descriptive. Study location: Fifty hospitals in the capitol-cities of eight LA countries. The survey was conducted during the month of November 2019. Methods: By means of an anonymous survey distributed through Survey Monkey™ platform (SurveyMonkey LLC, Palo Alto, California, United States) among Nursing professionals dedicated to TN in participating hospitals existence of a NTMT (or any other form of hospital organization) dedicated to NT administration, exclusive dedication of their members to NT provision, existence of legislations on NT provision, activities performed by Nursing personnel as part of the NTMT, existence of proprietary protocols for nutritional assessment of the patient, NT administration, and nutritional education of patients and relatives upon hospital discharge; and monitoring of NT quality indicators, management of risks related with NT, and local formation in NT issues were documented. Results: Ninety-five percent of participating institutions has a NTMT, but 55 % of the respondents are dedicated to NT provision in an exclusive manner. Half of the respondents knew of nationally existent legislations on NT. Sixty-five percent of the surveyed Nursing personnel delivers NT as part of the care of critically ill patients and those convalescing after surgery. INTRODUCTION: Hip fractures are a pathology that have emerged as a major subject over the years, due to increased prevalence and the multiple surgical treatments involved. The characterization and classification of the lesion is essential for proper surgical planning, with anteroposterior (AP), lateral and traction radiograph of the hip, paramount for decision-making. PATIENTS AND METHODS: This is a retrospective concordance study of 64 patients with hip fracture who consulted the hospital between January and July 2017. Four radiographs were taken of each: AP, AP with traction, lateral and lateral with traction. This set of images was evaluated by 9 observers, with different levels of experience, to answer questions regarding the classification, emphasize in potential instability and requirement of other images. A statistical analysis of concordance between and within observers was performed using Cohen's kappa coefficient. RESULTS: Of the 64 patients, 70.6% were women; the average age was 69.5 years. 82.8% presented a secondary fracture from falling from their own height. As the observer's experience increases, the need for traction radiograph decreases; interobserver kappa goes from 0.98 in experts to 0.01 in students. Traction radiograph is important in the diagnosis of potentially unstable fractures. Of the 1,503 radiographs with traction, 636 (42.38%) were classified as potentially unstable. And of the 708 without traction, 560 (79.1%) were classified as potentially unstable. CONCLUSIONS: Traction hip radiograph continues to be a useful tool in training environments to adequately classify an intertrochanteric fracture, considering it is a low-cost, minimal morbidity intervention, and is easily accessible. In similar studies, we found similar findings regarding the usefulness of traction to perform an adequate classification in people in training or in young orthopedists. It also influences to determine the potential instability, and this would modify the choice of the implant.

ABSTRACT

Introduction: Human papilloma virus (HPV) is the mainlycause of cervical cancer. Nutritional management considerationswith antioxidant rich diet, an adequate BMI range are protectivefactors that can modify the disease natural progression. Objective: To evaluate the nutritional status and foodintake in women with HPV. Methods: A cross-sectional study in 75 women of 18 to 35years old, selected by simple availability. A frequency foodintake was applied and analyzed with adequacy percentage.Nutritional status and risk factors as tobacco smoke wereassessed. Results: A 66% of women were obese or overweight.Tobacco was positive in 36%. The energy intake was covered in73%, 84% for lipid, 52% for protein and 66% for carbohydrates.Micronutrient intake was according recommendations.MaritalStatus, Smoking, Firs sexual intercourse age <18, obesity oroverweight, Waist circumference >80cm were risk factorsassociated between group 18 to 28 years vs 29 to 35 years old.Frecuency food intake were low for foods rich in acid folic,carotenoids, vitamin C mainly. Conclusions: This evidence showed the poor nutrimentsintake in the participants related with antioxidant function,that is important for treatment in premalignant lesion forHPV. These results highlight the importance of nutritionalmanagement considerations in prevention for resolution ofhuman papillomavirus premalignant lesions. (AU)

ABSTRACT

This paper presents a batteryless system-on-chip (SoC) that operates off energy harvested from indoor solar cells and/or thermoelectric generators (TEGs) on the body. Fabricated in a commercial 0.13 µW process, this SoC sensing platform consists of an integrated energy harvesting and power management unit (EH-PMU) with maximum power point tracking, multiple sensing modalities, programmable core and a low power microcontroller with several hardware accelerators to enable energy-efficient digital signal processing, ultra-low-power (ULP) asymmetric radios for wireless transmission, and a 100 nW wake-up radio. The EH-PMU achieves a peak end-to-end efficiency of 75% delivering power to a 100 µA load. In an example motion detection application, the SoC reads data from an accelerometer through SPI, processes it, and sends it over the radio. The SPI and digital processing consume only 2.27 µW, while the integrated radio consumes 4.18 µW when transmitting at 187.5 kbps for a total of 6.45 µW.
