ABSTRACT

Cash transfer programs are strategies used by countries, intended for impoverished families, which play an essential role in promoting access to public services such as health, education, and social protection. Programs may also promote food and nutrition security. The Brazilian Cash Transfer Program ("Bolsa Familia") (BFP) aims to alleviate immediate poverty and combat hunger. The aim of this study is to characterize the nutritional and breastfeeding status of children under two years old among both beneficiaries and non-beneficiaries of BFP. Data from the Brazilian Food and Nutritional Surveillance System, available in the primary healthcare service system of Goiania, Brazil, in 2013 were collected. The following variables were evaluated: sex, weight, height/length, age, and breastfeeding status. Data from 4,567 children under 24 months old were assessed, of which 2.72% (n= 124) were BFP beneficiaries. Beneficiaries had a lower odd of receiving breast milk compared to non-beneficiaries (OR= 0.46, 95% CI:0.31; 0.66, p= 0.0001). Regarding nutritional status, 18.14% (n= 790) of children were diagnosed with nutritional deviation, and overweight was the most prevalent (n=352, 8.04%). Beneficiaries presented a lower odd of developing stunting when compared to non-beneficiaries of BFP (OR= 0.44, 95% CI:0.25; 0.77, p= 0.006). Being a BFP beneficiary was a protective factor for the stunting in children under 24 months old in Goiania, Brazil. However, measures to promote and support breastfeeding should be intensified in primary healthcare service, aimed primarily at children in social vulnerability.


Los programas de transferencias en efectivo son estrategias utilizadas por los países, destinadas a familias en situación de pobreza y pobreza extrema, que juegan un papel fundamental en la promoción del acceso a los servicios públicos como salud, educación y protección social, además de promover la seguridad alimentaria y nutricional. El Programa Brasileño de Transferencias en Efectivo ("Bolsa Familia") (BFP) tiene como objetivo aliviar la pobreza inmediata y combatir el hambre. El objetivo de este estudio es caracterizar el estado nutricional y de lactancia de los niños menores de dos años, tanto beneficiarios como no beneficiarios del BFP. Se recogieron datos del Sistema de Vigilancia Alimentaria y Nutricional Brasileña, disponibles en el sistema de atención primaria de salud de Goiânia, Brasil, en 2013. Se evaluaron las siguientes variables, sexo, peso, talla, edad y estado de lactancia. Se evaluaron datos de 4.567 menores de 24 meses, de los cuales el 2.72% (n=124) eran beneficiarios del BFP. Las beneficiarias presentaron menos posibilidades de recibir leche materna en comparación con las no beneficiarias (OR= 0.46, IC95%:0,31; 0,66, p= 0,0001). En cuanto al estado nutricional, el 18,14% (n= 790) de los niños fueron diagnosticado con desviación nutricional, siendo el sobrepeso el más prevalente (n= 352, 8, 04%). Los beneficiarios presentaron menos probabilidad de desarrollar talla baja en comparación con los no beneficiarios del BFP (OR= 0.44, IC95%: 0.25; 0.77, p= 0.006). Ser beneficiario del BFP fue un factor de protección para la baja talla en menores de 24 meses en Goiania, Brasil. Sin embargo, se deben intensificar las medidas de promoción y apoyo a la lactancia materna en los servicios de atención primaria de salud, dirigidas principalmente a los niños en situación de vulnerabilidad social.

ABSTRACT

RESUMO Problematizou-se a percepção dos atores dos cursos da área da saúde da Universidade Federal de Goiás quanto ao impacto do Programa Nacional de Reorientação da Formação Profissional em Saúde articulado ao Programa de Educação pelo Trabalho para a Saúde (Propet-Saúde) na reestruturação curricular. Realizou-se um estudo transversal com a participação de 300 atores de sete cursos, entre estudantes, tutores, preceptores, professores e coordenadores de graduação. Para isto, foi construído e validado um instrumento atitudinal do tipo Likert com duas dimensões: D1: reestruturação curricular e os princípios do Propet-Saúde e D2: projeto pedagógico do curso e o Propet-Saúde. A validação de conteúdo do instrumento considerou as equivalências semântica, cultural e conceitual, atribuição de pontos (de 1 a 4) e randomização das assertivas. A pontuação gerou três intervalos interpretativos: de 1 a 1,99 (insatisfação); 2 a 2,99 (relativa satisfação) e 3 a 4 (satisfação). Foi feito pré-teste e, após a inserção das contribuições, o instrumento foi aplicado. Após a aplicação, o instrumento foi validado estatisticamente quanto à dispersão e confiabilidade. Foi realizada análise fatorial considerando função e curso. A dispersão por meio da correlação linear evidenciou perda de quatro asserções (11,77%) denotando excelente validação de conteúdo; a confiabilidade foi de 0,93 (p < 0,01), evidenciando ótima densidade estatística. As médias gerais de D1 e D2 foram 3,09 e 3,11 respectivamente, indicando satisfação entre os respondentes. Conclui-se que os participantes reconhecem importantes avanços, como a reestruturação curricular a partir dos programas, inserção dos alunos já no início do curso em cenários de prática, estímulo à interdisciplinaridade, diversificação de práticas avaliativas e uso de metodologias ativas. Na análise fatorial, verificou-se que os cursos de Medicina e Biomedicina mostram-se mais refratários às contribuições dos programas Pró-Saúde e Pet-Saúde. Outros desafios foram apontados, como carga horária excessiva, que dificulta a integração entre cursos e o desenvolvimento da competência em administração/gestão ainda incipiente. A junção dos programas na forma Propet-Saúde deve ser fortalecida, pois evidenciou grande assertividade na organização curricular, bem como na integração ensino-serviço, aspecto central para o fortalecimento do Sistema Único de Saúde (SUS).


ABSTRACT This study problematized the perception of stakeholders in the health care field at the Federal University of Goiás regarding the impact of the National Program for Professional Training and Reorientation in Health under the Program for Education for Work in Health (Propet-Saúde) on curricular restructuring. This cross-sectional study included 300 students, tutors, preceptors, teachers and graduation coordinators from seven courses. A Likert-type instrument was constructed and validated with the following dimensions: D1: curricular restructuring and the principles of Propet-Saúde and D2: the pedagogical project of the Propet-Saúde course. The instrument was validated considering semantic, cultural and conceptual equivalence, assignment of points (from 1 to 4), and randomization of the assertions tested. The score generated three interpretive intervals: from 1 to 1.99 (dissatisfaction); 2 to 2.99 (relative satisfaction) and 3 to 4 (satisfaction). The instrument was applied following a pre-test and the placement of the contributors. The instrument was then statistically validated for dispersion and reliability. Factorial analysis was performed considering function and course. Dispersion through linear correlation showed a loss of 4 assertions (11.77%), signifying excellent content validation. Reliability was 0.93 (p < 0.01), showing excellent statistical density. The general averages of D1 and D2 were 3.09 and 3.11 respectively, indicating satisfaction among the respondents. In conclusion, the participants recognize important advances such as curricular restructuring from these programs, placement of students already at the beginning of the course into practice scenarios, motivation toward interdisciplinarity, diversification of evaluation practices, and use of active methodologies. After factorial analysis, it was shown that medical and biomedical courses are more resistant to the contributions of the Pró-Saúde and PET-Saúde Programs and other hurdles were identified, such as excessive workload that hinders the integration of courses and the development of still-evolving administration and management skills. The interaction of the programs in Propet-Saúde should be strengthened, as it demonstrated significant assertiveness in curricular organization as well as in the teaching and service integration, a central aspect for the strengthening of Brazilian Public Health.

ABSTRACT

OBJECTIVE: To evaluate the effect of chromium supplementation on the glucose homeostasis and anthropometry of type 2 diabetic patients. MATERIAL AND METHODS: Fifty-six individuals with type 2 Diabetes were randomized on a double blind clinical trial into three groups: placebo (NC0), 50µg (NC50) and 200µg (NC200) of chromium nicotinate. Glucose homeostasis, anthropometry and physical activity intensity were evaluated at the beginning, at day 45 and at day 90. Energy intake was evaluated at the beginning, between the beginning and 45 days, and between days 45 and 90 of the study. RESULTS: There were no differences within or between groups for HOMA-IR, waist circumference, body fat percentage, lean body mass percentage and total energy intake during the trial. There was an increase of the HOMA-ß in group NC0 (p=0.0349) and a decrease of 1.08kg in group NC50 (p=0.0048) at 90 days. The relation between body mass index, body fat percentage and insulin sensitivity did not change in the placebo and supplemented groups (p>0.05). In the effect of the intervention, for each 1cm increase in waist circumference there was an increase of 1.90±0.63 in HOMA-IR (p=0.0087) and 16.31±5.27% in HOMA-ß (p=0.0073) in group NC200. No difference was seen in the intensity of physical activity within the groups and in the comparison between the supplemented groups (NC50 and NC200) and placebo (NC0) at 90 days. There was an increase in energy expenditure in physical activity at 90days (p=0.0371) of intervention in the group subjects NC50. As for total energy intake, there were no differences within or between the groups during the study. CONCLUSION: 50µg and 200µg supplementation with chromium nicotinate for 90days did not promote improvements in glucose homeostasis and anthropometry in individuals with type 2 diabetes mellitus.

ABSTRACT

OBJECTIVE: To investigate the effect of chromium nicotinate supplementation on insulin sensitivity, glycemic control, and lipid profile in subjects with type 2 diabetes. MATERIALS AND METHODS: In a double-blind clinical trial, 56 overweight individuals with type 2 diabetes were randomized into 3 groups: placebo (NC0), 50 µg (NC50), and 200 µg (NC200) of chromium as chromium nicotinate. Chromium status, sensitivity to insulin, glycemic control, and lipid profile were assessed at the beginning of the study and 45 days and 90 days after. RESULTS: In the beginning, most subjects showed low concentrations of serum chromium (71.88%), regular levels of urinary chromium (80.65%), and insulin resistance (73.80%). The serum chromium concentrations did not differ among the groups over time (p = 0.2549). The changes in serum chromium and urine concentrations did not relate to changes in fasting glucose (p > 0.05). At 90 days of intervention, there was no significant difference between groups in fasting glucose, glycosylated hemoglobin, homeostasis model assessment insulin resistance (HOMA-IR), total cholesterol, and low-density lipoprotein (LDL); there was increase in homeostasis model assessment ß-cell function (HOMA-ß; p = 0.0349) and high-density lipoprotein (HDL; p = 0.0425) in the NC0 group and a reduction of triglycerides in the NC0 (p = 0.0177) and NC50 (p = 0.0336) groups. CONCLUSION: Supplementation at 50 and 200 µg of chromium as chromium nicotinate did not promote glycemic control, increase insulin sensitivity, or change the lipid profile of subjects with diabetes.

ABSTRACT

As barras de cereais são produtos alimentícios obtidos da compactação de cereais, frutas secas, castanhas, aromas e ingredientes ligantes, que são utilizadas como opção de lanche rápido e saudável. Este trabalho teve como objetivo avaliar a aceitabilidade, aparência e qualidade nutricional de barras de cereais, adicionadas de diferentes concentrações de um fruto do cerrado processado como passa (murici-passa). Foram preparadas cinco formulações de barra de cereal com proporções crescentes de murici-passa em substituição à banana-passa [BCM0 –100% banana (controle), BCM25 – 25% murici e 75% banana,BCM50 – 50% murici e 50% banana, BCM75 – 75% murici e 25% banana, BCM100 – 100% murici]. As formulações foram analisadas quanto aos teores de umidade, cinzas, proteínas, lipídeos e carboidratos totais por diferença, fibra alimentar e aceitabilidade. As barras de cereais apresentaram em média 10,00%de umidade, 76,00% de carboidratos totais, 4,70% de lipídeos, 7,30% de proteínas e 1,20% de cinzas. A adição do murici-passa às barras de cereais aumentou o teor de fibras e favoreceu a análise de aparência, e a aceitabilidade foi satisfatória na proporção 75:25 de banana-passa e murici-passa. A adição de murici-passa proporcionou sabor diferenciado e incrementou a aparência e a qualidade nutricional das barras de cereais, contribuindo para valorização desse fruto regional na alimentação humana.
