ABSTRACT

The use of recreational cannabis affects almost all countries in the world, being illegal in most of them. Cannabis formulations include several compounds, with cannabidiol (CBD) being one of the best studied. One of its effects is its action in adenosine (anti-arrhythmic) receptors. We present the case of a 39-year-old male with chronic anxiety and regular inhaled cannabis consumption who reported to the emergency department (ED) after a first use of inhaled CBD. While in the ED, the patient presented with four episodes of syncope. EKG was performed and an extreme bradycardia with a sinus pause of 13.8 seconds was observed. Marijuana consumption is associated with various clinical manifestations, such as bradycardia, sinus pauses, and orthostatic hypotension. The tropism of this toxic for adenosine receptors can induce severe bradycardia, which can be life-threatening.

ABSTRACT

Mycobacterium chelonae is a non-tuberculous mycobacterium that can cause skin infections in immunocompetent individuals. We report a case of skin infection by this agent in a woman with dyslipidaemia, that culminated in statin-induced rhabdomyolysis due to the combination of clarithromycin, ciprofloxacin and simvastatin. LEARNING POINTS: Skin infection with Mycobacterium chelonae is an increasing global problem among immunocompetent individuals.Statin-induced rhabdomyolysis is an important and avoidable end-result of drug-drug interaction.Inhibition of cytochrome P450 isoenzyme 3A4 and of organic anion transporting polypeptide 1B1 are two important examples of statin interference with metabolism, and clarithromycin can inhibit both.

ABSTRACT

Serious electrical injuries are rare but may have life-threatening consequences. Voltage exposure injuries are divided into low voltage injury (LVI) or high voltage injury (HVI). An LVI current can result in severe injury, depending on the length of exposure, the size of the individual, the cross-sectional area in contact with the electrical source, and environmental humidity. The authors present a 31-year-old male with accidental electrocution with low voltage current and cardiopulmonary arrest. A detailed revision by organs and systems is presented. LVI is uncommon and can occur with a variety of clinical presentations, rarely presenting with direct lung injury. Early recognition and support are the cornerstones of treatment.

ABSTRACT

Listeria monocytogenes is a ubiquitous, rapidly growing, gram-positive bacterium causing infections in humans and animals. It is responsible for a variety of symptoms depending on the infection site and the integrity of the host's immune system. Case reports of skin and soft tissues infections by Listeria are rare. The authors present a case of a 65-year-old diabetic male with recurrent skin abscess diagnosed with a perianal abscess due to Listeria monocytogenes associated with lumbar spine osteitis. At the time of this publication and to our knowledge, this case represents the first Listeria monocytogenes infection involving skin and bone in a diabetic man with recurrent skin abscess.
