ABSTRACT

UNLABELLED: The aim of our study was to identify electrocardiographic predictors of response to cardiac resynchronization therapy (CRT) in patients with dilated cardiomyopathy (DCM). METHODS: 46 patients with DCM (76% men; mean age 48.8+/-12 years; mean NYHA class 3.09+/-0.3; mean QRS duration 167+/-30.1 ms; left ventricular ejection fraction 23.5+/-6%) had CRT implanted. Assessment of clinical, laboratory and echocardiographic (including TDI) and 12-lead surface ECG parameters was performed at baseline, in 24 hours post CRT implantation and after 6 months of follow-up. Multivariate logistic regression analysis selected the following independent predictors of response to CRT: two pre-implantation - baseline RS width in lead V1 ≥ 113.5 ms (OR 0.94, 95% CI 0.814-1.00; p=0.000), BNP ≤ 456 g/ml (OR 0.92, 95% CI 0.797-1.00; p=0.001); and two post-implantation: shortening of QRS width - ΔQRS ≥ -24.5 ms (OR 0.80, 95% CI 0.586-1.00; p=0.01) and shortening of RS V1 width - ΔRSV1 ≥ -21.8 ms (S: 0.87, 95% CI 0.72-1.00; p= 0.002) in 24 hours after CRT.

ABSTRACT

Clinical-electrophysiological investigation has been performed in 30 patients with acute lacunar cerebral infarct. Patients with atrial fibrillation, severe extrasystole weren't included to the study. The control group includes 7 health volunteers. EKG registration within 5 minutes with time domain, frequency domain analysis have been made. Brain attack on the type of lacunar IS associates with evident HRV disturbances. Centralization of the heart rate autonomic control, caused by suprasegmental part of the vegetative nervous system hyperactivity occurs against a background relative sympaticothonia and decrease of parasympathetic nervous system. In case of right carotid stroke there is significant lowering of standard deviation of the RR intervals (p=0,024), triangular index (p<0,001), total power (p=0,016), increase of mode of the histogram distribution of RR intervals (p=0,002) and the proportion derived by the number of RR intervals, that match the value of mode (p=0,001).

ABSTRACT

The results are provided on the development and introduction of automated working places on the basis of personal computers for the prevention, follow-up and treatment of patients with arterial hypertension (AH) and ischemic heart disease (IHD) at an industrial enterprise. The process of identifying persons with AH and IHD is automatized. Automated working places on ECG processing, assessment of central and cerebral hemodynamic parameters are introduced. Information load has been developed in the form of a computer game for identifying persons with early stages of AH. A base has been created for storage of the results of regular medical check-ups. The complex has been tested during 2 years in the settings of a health unit at the Lenin tractor plant in Minsk. As a result of comprehensive measures, provisional functional diagnosis expenditures were reduced by 30-70%. Losses due to temporary disability as a result of circulatory diseases were reduced by 26%.

ABSTRACT

A modified method for the determination of red cell membrane permeability for sodium and potassium monovalent ions is described. The described modification essentially cuts down the time of investigation and accelerates potassium ion transport. Instead of ouabain, an imported reagent, strophanthin K glycoside manufactured in the USSR is used in the test.
