ABSTRACT

Pleural tuberculosis occurs in 30% of patients with tuberculosis, and the percentage of patients with tuberculosis pleural effusions is comparable to human immunodeficiency virus HIV-positive and HIV-negative individuals, although pleural tuberculosis is rare in HIV-positive patients with CD4+ counts < 200 cells/mm(3). Pleural tuberculosis in HIV-positive patients is likely to happen in young patients, and is more frequent in intravenous drug abusers, with more acid-fast bacilli identifiable in pleural tissue. We report a rare case of pleural tuberculosis in a severely immunosuppressed HIV-positive patient, presented as two parasternum pleural-cutaneous fistula.

ABSTRACT

PURPOSE: The aim this study was to evaluate the clinical outcome of patients needing intra-operative blood transfusion by tissue perfusion markers. METHODS: A prospective single center cohort study. Adult patients needing blood transfusion during the intra-operative period were recruited. RESULTS: This study included 61 patients. At the time of blood transfusion the hemoglobin level was 8.4+/-1.8 g/dL. Scv02 has been the best tissue perfusion marker to determine mortality, compared with hematemetric values and other tissue perfusion markers, with a cut-off point at ROC curve equal to 80% (AUC=0.75; sensitivity=80%; specificity=65.2%). Patients who received blood transfusion and had Scv02 80% (N=32), had lower mortality rates (12.5% vs. 47.1%; p=0.008) and lower incidence of postoperative complications (58.9% vs. 72.9%; p=0.06). Blood transfusion with a Scv02 80%, are associated with worse clinical outcomes.

OBJECTIVE: To analyze the prevalence of cigarette smoking in individuals with severe mental illnesses in a large urban centre of a middle income country. METHODS: Cross-sectional study carried out in São Paulo. The sample (N=192) comprised individuals diagnosed with severe mental illnesses who had contact with public psychiatric care services from September to November 1997 and were aged between 18 and 65 years. Prevalence of daily tobacco smoking in the 12 months previous to the interview and characteristics associated were studied. RESULTS: Out of 192 subjects with severe mental illnesses interviewed, 115 (59.9 percent; 95 percent CI: 52.6 percent; 66.9 percent) reported smoking cigarettes on a daily basis. Male gender, marital status separated or widowed, irregular use of neuroleptic drugs and history of ten or more psychiatric admissions were independently associated with cigarette smoking. CONCLUSIONS: The prevalence of cigarette smoking in the present sample was higher than that found in the general Brazilian population. Mental care services should implement non-smoking policies and mental health providers need to help patients with severe mental illness who want to quit smoking.


OBJETIVO: Analisar a prevalência de tabagismo em amostra de indivíduos com transtornos mentais graves em um grande centro urbano de um país emergente. MÉTODOS: Estudo transversal realizado no município de São Paulo. A amostra (N=192) foi composta por indivíduos com diagnóstico de transtorno mental grave que tiveram contato com serviços psiquiátricos do setor público de setembro a novembro de 1997 e tinham idade entre 18 e 65 anos. O uso diário de cigarro nos 12 meses anteriores à entrevista e as características associadas a esse consumo foram investigados. RESULTADOS: Dos 192 indivíduos com transtornos mentais graves entrevistados, 115 (59,9 por cento; IC 95 por cento: 52,6 por cento;66,9 por cento) referiram ter feito uso diário de cigarros. Ser do sexo masculino, ser separado ou viúvo, fazer uso irregular de neurolépticos e ter história de dez ou mais internações anteriores mostraram-se independentemente associados ao tabagismo. CONCLUSÕES: A prevalência de tabagismo na amostra de indivíduos com transtornos mentais graves foi maior que a encontrada na população geral brasileira. Serviços de saúde mental devem promover políticas anti-fumo e profissionais de saúde mental devem ajudar os portadores de transtornos mentais graves que desejarem parar de fumar.

OBJECTIVE: To investigate the social adjustment of individuals with severe mental illness living in the community in a large urban center of a developing country, and the characteristics associated with poor social functioning. METHOD: A cross-sectional study was performed in the city of Sao Paulo. Eligible subjects were residents of a defined geographic area, aged between 18 and 65, with a diagnosis of functional psychosis who had had contact with any public psychiatric service during a defined period. Structured assessments were used to obtain information on social-demographic characteristics, diagnosis (ICD-10), psychiatric symptoms (PANSS), and social adjustment (DAS). RESULTS: One hundred and eighty-eight subjects were included, of whom, 120 (63.8%) had some degree of impairment in social functioning. The most frequently affected areas of social functioning were work performance and sexual role. Twenty-four patients (12.8%) showed poor or very poor social adjustment in the month prior to the interview. Negative symptoms, number of previous admissions and general symptoms showed statistically significant associations with global social adjustment scores. CONCLUSIONS: The proportion of patients showing any degree of impairment in social adjustment was as high as in more developed societies. In order to successfully implement the new mental health policy in Brazil, better provision of community-based mental health services for those with severe mental illnesses is needed.

OBJETIVO: Investigar o ajustamento social em indivíduos com transtornos mentais graves vivendo na comunidade de um grande centro urbano de um país em desenvolvimento e as características associadas a um ajustamento social pobre. MÉTODO: Um estudo de corte transversal foi conduzido em São Paulo. Indivíduos elegíveis deviam residir em áreas definidas da cidade de São Paulo, ter tido contato com os serviços psiquiátricos do setor público em período de três meses, idade entre 18 e 65 anos e diagnóstico de psicose funcional. Entrevistas estruturadas foram utilizadas para obter dados sociodemográficos, diagnóstico (CID-10), sintomas psiquiátricos (PANSS) e ajustamento social (DAS). RESULTADOS: Cento e oitenta e oito indivíduos foram incluídos e 120 (63,8%) apresentaram algum grau de desajustamento social. As áreas mais afetadas foram performance no trabalho e sexual. Vinte e quatro indivíduos (12,8%) apresentaram ajustamento social pobre ou muito pobre no mês anterior à entrevista. Número de internações anteriores, sintomas gerais e negativos de esquizofrenia mostraram-se associados ao ajustamento social global. CONCLUSÕES: A proporção de indivíduos apresentando qualquer grau de desajustamento social é comparável à encontrada em países mais desenvolvidos. Para a adequada implementação das novas políticas públicas em Saúde Mental no Brasil, há a necessidade de melhor provisão de serviços aos indivíduos com transtornos mentais graves.

ABSTRACT

BACKGROUND: The purpose of our study was to evaluate the desire of the patients on being informed about diagnosis of severe diseases, the desire to have their families informed about this situation and to participate in therapeutic decisions. METHODS: 363 patients (outpatients and inpatients) of a General Internal Medicine division of a University Hospital were interviewed. The questionnaire contained specific questions on their desire to be informed of the diagnosis in case of cancer or AIDS and on their desire to have their families informed as well. Specific questions on whether they wanted to be informed of and participate of the therapeutic discussion process in case of abdominal tumors were also included. RESULTS: 96.1% of men and 92.6% of women showed the desire of being informed in case of cancer diagnosis and 87.7% of men and 84.2% of women wanted to have their families informed, 94.2% of men and 91% of women wanted to know the diagnosis of AIDS. While 86% of women and 76.6% of men wanted be informed in the case of a diagnosis of an abdominal tumor, only 58.5% of women and 39.6% of men wanted to give their opinion about in the case of different therapeutic alternatives. The desire to participate in therapeutic decisions was significantly lower (p<0.05) in men, people older than 60 years and inpatients. CONCLUSIONS: Our results showed that the great majority of the population that seeks for medical support in a Brazilian university hospital wishes to be informed on this health condition, even in case of serious illness. In addition, there are intense familiar bonds that make patients want to have their families also informed.

OBJETIVO: Avaliar o desejo de pacientes serem informados sobre diagnóstico de doenças graves, de informação às suas famílias e de participação nas decisões terapêuticas. MÉTODOS: 363 pessoas atendidas no ambulatório ou internadas na enfermaria de um serviço universitário de Clínica Geral foram entrevistadas. O questionário continha perguntas sobre desejo de ser informado e de que familiares também fossem informados em casos de diagnósticos de câncer e síndrome da imunodeficiência adquirida (Aids) e de ser informado e participar de decisões terapêuticas em caso de tumores abdominais. RESULTADOS: Homens (96,1 por cento) e mulheres (92,6 por cento) mostraram desejo de serem informados do diagnóstico de câncer e 87,7 por cento dos homens e 84,2 por cento das mulheres desejaram que sua família também fosse informada; 94,2 por cento dos homens e 91 por cento das mulheres afirmaram querer saber do diagnóstico de Aids. Enquanto 86 por cento das mulheres e 76,6 por cento dos homens mostraram desejo de serem informados das opções terapêuticas em caso de tumor abdominal, apenas 58,5 por cento das mulheres e 39,6 por cento dos homens desejaram opinar sobre o tratamento. O desejo de participar das decisões terapêuticas foi menor nos homens, nas pessoas com mais de 60 anos e em quem estava internado (p<0.05). CONCLUSÕES: A grande maioria da população que procura um hospital universitário deseja ser informada sobre suas condições de saúde, incluindo eventuais diagnósticos de doenças graves. Por outro lado, existem vínculos familiares intensos, sendo que os pacientes desejam, também, que suas famílias sejam informadas.
