ABSTRACT

OBJECTIVE@#To develop a digital breast tomosynthesis (DBT) imaging system with optimizes imaging chain.@*METHODS@#Based on 3D tomography and DBT imaging scanning, we analyzed the methods for projection data correction, geometric correction, projection enhancement, filter modulation, and image reconstruction, and established a hardware testing platform. In the experiment, the standard ACR phantom and high-resolution phantom were used to evaluate the system stability and noise level. The patient projection data of commercial equipment was used to test the effect of the imaging algorithm.@*RESULTS@#In the high-resolution phantom study, the line pairs were clear without confusing artifacts in the images reconstructed with the geometric correction parameters. In ACR phantom study, the calcified foci, cysts, and fibrous structures were more clearly defined in the reconstructed images after filtering and modulation. The patient data study showed a high contrast between tissues, and the lesions were more clearly displayed in the reconstructed image.@*CONCLUSIONS@#This DBT imaging system can be used for mammary tomography with an image quality comparable to that of commercial DBT systems to facilitate imaging diagnosis of breast diseases.

ABSTRACT

Objective To know the situation of extramarital sexual behaviors and HIV infection in middle-aged and elderly people in Chongqing,and provide reference for AIDS prevention and treatment.Methods From October to December 2017,a multi-stage sampling method was used to recruit middle-aged and elderly people aged ≥50 years who lived in Dazu and Hechuan districts of Chongqing for at least one year,with a sample size of 410.Face-to-face questionnaires survey and HIV antibody test were conducted.Results A total of 408 people were surveyed,including 313 males and 95 females aged 50-88 (64.93 ± 9.03) years.The HIV infection rate was 1.47% (6/408),with the rate of 1.28% (4/313) in males and 2.11% (2/95) in females.The awareness rate of AIDS related knowledge was 37.50% (153/408).And 18.87% (77/408) of subjects surveyed reported extramarital sexual behaviors,7.60% (31/408) reported extramarital sexual behaviors in the past half year,the constant condom use rate was 19.35% (6/31).The results of multivariate logistics model analysis on extramarital sexual behaviors showed that the prevalence in males were 39.51 times higher than that in females (OR=39.51,95%CI:5.03-310.30),4.60 times higher in those who were unmarried,divorced or widowed than that in the married or cohabitants (OR=4.60,95%CI:1.50-14.05),2.03 times higher in those with outside activities than those with individual activities (OR=2.03,95%CI:1.08-3.81) and 3.94 times higher in those with self-evaluation of emptiness of living state than that in those with engaged life (OR=3.94,95%CI:1.86-8.36).Conclusions The prevalence of extramarital sexual behavior in middle-aged and elderly people in some counties and districts in Chongqing is high.The factors such as gender,marital status,leisure activities,and self-evaluation of living state were related to the prevalence of extramarital sexual behaviors in this population.The condom use rate in extramarital sexual behavior was low.It is necessary to take effective interventions in this population.

Objectives To analyze and understand the risk factors related to HIV new infections among men who have sex with men (MSM).Methods A longitudinal observational study among MSM was conducted to collect information on HIV related behaviors and sero-conversion.Univariate and multivariate generalized estimating equations (GEE) were used to discuss the risk factors for HIV new infection.Results A total number of 4 305 MSM were followed during 2013-2015.Among those self-reported MSM who are seeking partners on the Interner tended to have higher proportion on receptive anal intercourse and consistent condom use during anal intercourse than the subgroups seeking their partners in gay bars or bathrooms.HIV incidence among followed MSM during the study period appeared as 4.3/100 person years,with adjusted RR (aRR) of HIV infection for receptive anal intercourse as group 2.20 (95% CI:1.49-3.24) times than that of insertion anal intercourse group.Those who used rush-poppers (aRR=1.55,95%CI:1.10-2.17),unprotected anal intercourse (aRR=2.24,95% CI:1.62-3.08),and those with syphilis infection (aRR=2.95,95% CI:2.00-4.35) were also risk factors for HIV new infections.After controlling other factors,the relationship between the ways of seeking partners and HIV new infection was not statistical significant.Conclusion Risk factors for HIV new infection among MSM appeared complex and interactive,suggesting that further studies are needed to generate tailored strategies for the prevention of HIV epidemic among MSM population.

ABSTRACT

Objective@#To analyze the spatial clustering characteristics of HIV/AIDS among men who have sex with men (MSM) in Chongqing from January 2004 to December 2015 and understand the HIV/AIDS related behaviors among MSM by interview.@*Methods@#Data related to MSM who were infected with HIV and whose present address were in Chongqing, were collected from Information System on the HIV/AIDS Prevention and Control. Information included the age when the information was inputted, address, occupation, education level, and marital status. The total number of MSM who were infected with HIV and reported was 6 604 in Chongqing. Those with unknown address were ruled out. The spatial autocorrelation analysis and the local spatial autocorrelation analysis were carried out by using ArcGIS 10.3. In addition, in November 2015 and May 2016, using a convenience sampling, we conducted one-on-one interviews among 23 MSM in the Chongqing Center for Disease Control and prevention. Inclusion criteria: Receiving voluntary counseling and testing in the urban area of Chongqing and willing to participate in the interview by oral informed consent; male and self-described as MSM. The content of the interview included basic information, sexual orientation, sexual role, the main place of making friends, the main place of sexual behavior, a long-term experience in other provinces and drug abuse.@*Results@#The HIV/AIDS reported number in Chongqing from 2004 to 2015 showed an uptrend, except in 2010. The age distribution of 6 604 cases of HIV positive patients was mainly concentrated in the 15-34 years old, about 68.5% (4 522 cases). There was a positive spatial autocorrelation in MSM, except 2005 (Moran's I=-0.046, P=0.823), form 2004 to 2015, Global Moran's I values were 0.308, 0.254, 0.335, 0.683, 0.673, 0.558, 0.620, 0.673, 0.685, 0.654 and 0.649, respectively; all P values were <0.01. The result of local spatial autocorrelation analysis showed that high-high accumulation area development in Chongqing city was divided into two stages in 2004-2015, which were Yuzhong, Jiangbei, and Shapingba district in 2004-2007 and the expanded Jiulongpo, Nan'an, and Yubei district in 2008-2015. Qualitative interviews results revealed that the age of the respondents was 20-44, and the mainly way of making friends were using mobile phone App and internet (17 participants). Most of the participants (11 participants) were making friends in the bar. The majority of respondents would ask the friends or themselves to use condoms when meeting with them the first time (19 participants), and 8 of respondents reported that they would not use condoms when their old friends refused to use condoms.@*Conclusion@#The HIV infected MSM mainly aged between 15-34 years old and the spatial distribution of HIV/AIDS among MSM was clustered in economically developed main area in Chongqing. MSM began to make friends on the Internet, and could not adhere to using condom, which indicated that we should focus on internet intervention to find more efficacious interventions.

ABSTRACT

Objective To analyze the epidemic characteristics of HIV/AIDS among older adults in Chongqing,and to provide scientific evidence for developing prevention and control measures.Methods The data were collected through the national AIDS reporting information system,SPSS 17.0 was used to analyze the data.Results From 2001 to 2015,a total of 10495 cases had been reported accumulatively,and HIV/AIDS cases was accounting for 31.1%.The study was showed that the main infection people was man and the main professional was the main farmer.Moreover,the main infection was heterosexual transmission and accounting for 91.5%.Conclusion The epidemic of HIV/AIDS aged over 50 was on the rapid rise from 2001 to 2015 in Chongqing,the publicity and education should be focused on them,to improve the relevant awareness and reduce their risk behavior.

ABSTRACT

Objective To know basic characteristics of MSM infected with HIV treated by highly active antiretroviral thera‐py(HAART) ,and evaluate therapeutic effect based on the level of CD4+ T cell counts .Methods Basic database of patients were collected from the national AIDS comprehensive data information system ,patients were observed by the retrospective cohort study , and effect indicators were analyzed based on the level of CD4+ T cell counts .The data was analyzed by SPSS18 .0 software .Results A total of 1 758 cases were recruited in this study and the average age was (33 .5 ± 11 .1)years .The component ratio of the CD4+T cell counts 350/μL were 7 .7% ,27 .6% ,54 .9% ,9 .7% ,respectively .The CD4+ T counts increased and kept among 150 to 250/μL ,250 to 350/μL ,350 to 450/μL ,450 to 550/μL ,respectively after 12 months ,treat‐ment .The rates of viral inhibition were 70 .2% ,81 .7% ,84 .2% ,83 .3% ,respectively after 6 months′treatment .And the cumulative survival rates were 0 .81 ,0 .93 ,0 .98 ,1 .0 after 36 ,months treatment ,respectively .Conclusion HAART on MSM achieves remarka‐ble results .And the early treatment was more beneficial to improve immune function ,reduce AIDS mortality and control the HIV/AIDS epidemic of MSM .

ABSTRACT

<p><b>OBJECTIVES</b>To investigate the incidence of syphilis infection and to determine the risk factors related to syphilis infection among young men who had sex with men (YMSM), which were documented for developing effective intervention to prevent sexually transmitted diseases among YMSM.</p><p><b>METHODS</b>A cohort study was conducted in 8 cities (Beijing, Shanghai, Kunming,Guiyang, Chongqing, Chengdu, Urumqi and Nanning) from May to December, 2009. A total of 1 037 syphilis-negative YMSM aged 18-25 were enrolled in the cohort and the two follow-up surveys were carried out every six months. The contents of study included sociodemographic characteristics, HIV-related knowledge, sexual behavior and condom use in the 6 months prior to survey. All participants were tested for syphilis with whole blood specimens. Chi-square test was used to compare demographic characteristics of participants in baseline with those of two follow-up, and Cox regression analysis was used to identify risk factors associated with syphilis infection.</p><p><b>RESULTS</b>The rates of participants in 6, 12 months follow-up surveys was 79.85% (828/1 037) and 82.16% (852/1 037) respectively.39 syphilis seroconversions were found in the 12 months follow-up survey. Cumulative observed person-years during follow-up time was 1 106.67. The syphilis incidence rate was 3.5%. The Multivariate Cox regression analysis showed that the education of senior high school (senior high school vs some college or higher, RR = 2.19, 95% CI:1.21-3.98), bisexual orientation (bisexual orientation vs homosexual orientation, RR = 2.19, 95% CI:1.21-3.97), score of HIV/AIDS knowledge <8 (score of HIV/AIDS knowledge <8 vs knowledge = 8, RR = 2.39, 95%CI:1.35-4.21), had two and more sexual partners and inconsistent condom use in the past 6 months (inconsistent condom use vs consistent condom use, RR = 3.10, 95% CI:1.39-6.91) were significantly associated with syphilis seroconversion in the 12-month period.</p><p><b>CONCLUSIONS</b>The syphilis incidence was high and risk behaviors were common among YMSM of China.</p>

ABSTRACT

Objective To study and evaluate the effect of developing the preventive service in venereal clinic .Methods The questionnaire survey on the selected outpatients was performed by the trained medical personnel in the venereal disease clinic of 5 medical institutions in Chongqing .The intervention services were carried out by providing healthy advices and tailor-made preven-tive packs .The healthy knowledge publicity were posted in the waiting area of the venereal clinic .The questionnaire investigation in the re-visiting patients was performed again to understand the awareness of prevention venereal disease knowledge and the condom use .Results 96 .4% of outpatients in this study accepted the AIDS-related knowledge at the first visit .84 .1% of them acquired the knowledge by consulting their doctor ,60 .9% of the patients insisted on using condom in sexual activity after accepting the interven-tion knowledge ,which was much higher than 17 .4% before intervention .Conclusion The STD clinic attenders satisfy the interven-tional preventive service in the venereal clinic ,the improvement of the venereal disease related knowledge and the adverse behaviors is obvious .

ABSTRACT

To meet the need of accurate positioning for biopsy gun in the breast biopsy operation, a new stereotactic biopsy guide device have been developed to adapt to the domestic mammary machine, which can help physician to carry out biopsy operation more accurately and effectively. The guide device has the motion model, measurement model and display model and can realize linear motion and display real-time displacement values in X, Y and Z direction. The experimental results showed that the guide device could be well fixed in the domestic mammary machine, and achieved good accuracy and repeatability in each direction. Depending on the displacement values, physician can change the space of biopsy gun accurately.

Objective To establish a comprehensive evaluation indexing system to appraise the implications of prevention and treatment of HIV/AIDS ,and to calculate the weight of each indicator .Methods Based on the idea of performance and input-out-put ,professional consultation ,and Delphi method was determined as the evaluation index system ,analytical hierarchy process (AHP) was used to calculate the weight value for each indicator .Results The evaluation indexing system had been established af-ter three rounds of professional consultation .It contained two 1st class indicators ,six 2nd class indicators and thirty-one in 3rd class indicators .The weight value of each indicator was calculated .Conclusion The evaluation indexing system that has been established and the weight value quantities are of completeness ,practicality ,operability and logic .They have important value for application in the future .

ABSTRACT

Objective To explore more satisfactory results in correcting the aesthetically short nose combined with low and blunt nasal tip by using autologous cartilage grafts.Methods External rhinoplasty approach was preferred.Nasal septal cartilage and auricular cartilage or rib cartilage of patients were adopted,as necessary,various shapes of grafts were carved,such as septal extention graft,columella strut and shift graft.Then we used these autologous cartilage grafts with suture and removal techniques to correct the aesthetically short nose combined with low and blunt nasal tip.The length of the nose (n-prn) and the projection of nasal tip (sn-prn) were measured before and after the operation.Paired t-test was adopted to evaluate the results.Results Thirty-one patients accepted the nasal tip rhinoplasty with autologous cartilage whose nasal tip was over rotation,blunt and low.All the patients were followed up for 3 months.Thirty patients satisfied with the results,accounting for 96.7 % of the total.()ne (3.3 %) patient dissatisfied with the result because of postoperative asymmetry nostrils.Nose length before and after surgery was significantly different (P<0.05).Nasal tip projection before and after surgery was also significantly improved (P<0.05).Conclusions It is an effective method to use autologous cartilage graft for correcting the aesthetically short nose combined with low and blunt na sal tip,with low complications.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of topical application of leptin in promoting burn wound healing in rats.</p><p><b>METHODS</b>Four parallel second-degree burn wounds induced on the back of 18 Wistar rats were divided into leptin treatment group (treated with topical application of 400 ng/ml leptin dissolved in PBS) and control group (treated with PBS). The time of wound healing was recorded, and the wound area that was not healed was measured at 3, 7, 11, 15, and 19 days after burns. The tissue at the peripheries of the wound was sampled at 7, 14 and 21 days after burns for pathological examination with HE staining and immunohistochemistry for proliferating cell nuclear antigen (PCNA) to evaluate the proliferation of keratinocytes.</p><p><b>RESULTS</b>Compared with the control group, leptin-treated wounds showed a shorter time (by 2-3 days) of wound healing, and significant differences were found between the two groups in healing at 7, 11, 15, and 19 days after burns. HE staining and immunohistochemistry revealed a faster rate of epidermis growth and a greater thickness of the cuticular layer in leptin-treated wounds at 7, 14 and 21 days after burns. PCNA positivity in the keratinocytes was stronger in leptin-treated wounds than in the control wounds at 7 and 14 days, but no such distinct difference was noted at 21 days between the two groups.</p><p><b>CONCLUSION</b>Topical application of leptin can promote re-epithelization in burn wounds to shorten the wound healing time of burns.</p>

ABSTRACT

Objective To observe the effects of leptin on the proliferation and differentiation of human preadipocyte in vitro, and to explore the possible mechanism of the generation of obesity regulated by leptin. Methods The human preadipocytes were isolated from human subcutaneous adipose tissue of abdomen and cultured in vitro. The effects of leptin (0-1 000 ng/ml) on the proliferation, lipid accumulation and the mRNA expression of PPAR-γ2 and C/EBP-α, which are the differentiation and transcription factor of human preadipocyte, were analyzed by the methods of MTT, cell counting, extracting stained intracytoplasmic lipid with oil red O and RT-PCR. Results Leptin (1 000 ng/ml) could stimulate the proliferation, lipid accumulation and the mRNA expression of PPAR-γ2 and C/EBP-α (P<0. 05). There were not obvious effects on the proliferation and lipid accumulation in the groups of lower (10 ng/ml) and common (100 ng/ml) concentration (P>0. 05). Conclusion Leptin in higher concentration can stimulate the proliferation and differentiation of preadipocye in vitro, which indicates that leptin may regulate the generation of obesity through acting on the proliferation and differentiation of preadipocyte at the pathologic state of leptin resistance and high leptin concentration in serum.

ABSTRACT

[Objective] To investigate the antibiotic resistance and nosocomial infections status of Staphylococcus aureus(SAU)and provide scientific evidence for disease diagnosis and reasonable use of antibiotics.[Methods]The clinical isolates were identified by VITEK-2 compact.AST-GP61 was used to detect the antibiotic susceptibility.[Results] Among 310 S.aureus isolates,SAU was isolated from ICU and department of dermatology,from sputum/throat and secretion.The drug sensitivity rate to glycopeptide antibiotics and linezolid was 100%,the incidence of oxacillin-resistant S.aureus was 53.1%.[Conclusions] S.aureus showed multidrug resistance to the commonly used antibiotics,while curing infections caused by oxacillin-resistanct S.aureus,Vancomycin,teicoplanin and linezolid were the best choice.
