ABSTRACT

Statement of problem Dental restorations are subjected to tensile stresses from oblique or transverse loading of their complex geometric forms, making tensile strength a fundamental mechanical property. Since composite core build-up materials are brittle, the integrity of the post and core depends on their tensile strength and resistance to fracture when utilized with various prefabricated post systems. Therefore, it is essential to determine the tensile strength of the prefabricated metallic and nonmetallic posts used to reinforce the composite resin core. Purpose This study compared the diametral tensile strength (DTS) of three prefabricated post systems with composite core build-up material. Material and methodology Ten composite resin cores from four different groups were formed. The control group was the composite resin core without a post (group 1). Group 2 was composed of composites with metal posts, group 3 was composed of composites with glass fiber posts, and group 4 was composed of composites with carbon fiber posts. All the samples were kept in a humid place for seven days to mimic the conditions in the mouth. DTS was determined by recording the tensile force required to fracture the core material by performing a diametral compression test for tension after a week. The observations were analyzed using a one-way analysis of variance (ANOVA), followed by a post-hoc test. Results The tensile strength of the resin core material was decreased by 28.1%, 20.8%, and 10.4% by using posts made of stainless steel, carbon fiber, and glass fiber, respectively. Among the three post systems, stainless steel had the lowest mean DTS values, while glass fiber had the highest mean DTS values. Conclusion Composite core glass fiber post systems showed higher tensile strength than other post systems.

ABSTRACT

Cardiovascular disease is a leading cause of death in children with chronic kidney disease (CKD). A strong correlation exists between disturbed calcium-phosphate metabolism and cardiac dysfunction. Studies with use of cinacalcet in CKD are few and limited to older children and adults and in improving growth and bone deformities. We present three children with CKD on CAPD with cardiac dysfunction with refractory hyperparathyroidism. Patients were initiated on lowest adult weight-adjusted dose of 0.2 mg/kg/day. Dose was titrated every 30 days to achieve decline in iPTH to a goal of 200- 300 pg/ml. Serum calcium, phosphorus and iPTH levels were checked monthly. Complications of therapy related to cinacalcet monitored. Monthly echocardiography done to monitor cardiac dysfunction. None of them experienced significant adverse effects of cinacalcet therapy.

ABSTRACT

Betel quid (paan) chewing is common in India, especially in Uttar Pradesh. Betel quid has multifaceted relationship with health, including metabolic and psychosocial health. The current recommendations have been released keeping in view the public health and clinical importance of this addictive behavior. The objective of this document is to offer clinical guidance for screening, diagnosis and management of co-occurring betel quid chewing among persons with Diabetes Mellitus (DM). The document aims to provide education and guidance to clinicians engaged in care and management of persons with DM, and improve access to treatment for co-occurring betel quid chewing among persons with DM. The current recommendation grades are based on published evidence, and categorized as strong, intermediate, weak and no evidence. The strength of these recommendations is based on the level of evidence.

ABSTRACT

Gallstone ileus is a mechanical intestinal obstruction caused due to impaction of a large gallstone within the bowel. The ideal treatment of gallstone ileus remains controversial, with the main dilemma being between a one-stage and a two-stage surgical procedure. A 69-year old male patient presented with gallstone ileus. A one-stage procedure with enterolithotomy and primary closure of duodenal fistula was done. His immediate postoperative recovery was uneventful, but after 3 weeks of surgery, he developed respiratory complications and expired of multi-organ failure. In gallstone ileus, patient presents with symptoms of intestinal obstruction. Enterolithotomy alone remains the most common operative method, but the definitive surgical management is still under research. An intraoperative dilemma between a one-stage or two-stage surgery is difficult to resolve in absence of clear guidelines. Hence, more studies are required to come to a consensus in deciding its definitive management.
