ABSTRACT

BACKGROUND: Important evidence has been constantly produced and needs to be converted into practice. Professional consumption of such evidence may be a barrier to its implementation. Then, effective implementation of evidence-based interventions in clinical practice leans on the understanding of how professionals value attributes when choosing between options for dental care, permitting to guide this implementation process by maximizing strengthens and minimizing barriers related to that. METHODS: This is part of a broader project investigating the potential of incorporating scientific evidence into clinical practice and public policy recommendations and guidelines, identifying strengths and barriers in such an implementation process. The present research protocol comprises a Discrete Choice Experiment (DCE) from the Brazilian oral health professionals' perspective, aiming to assess how different factors are associated with professional decision-making in dental care, including the role of scientific evidence. Different choice sets will be developed, either focusing on understanding the role of scientific evidence in the professional decision-making process or on understanding specific attributes associated with different interventions recently tested in randomized clinical trials and available as newly produced scientific evidence to be used in clinical practice. DISCUSSION: Translating research into practice usually requires time and effort. Shortening this process may be useful for faster incorporation into clinical practice and beneficial to the population. Understanding the context and professionals' decision-making preferences is crucial to designing more effective implementation and/or educational initiatives. Ultimately, we expect to design an efficient implementation strategy that overcomes threats and potential opportunities identified during the DCEs, creating a customized structure for dental professionals. ABSTRACT

The aim of this study was to report the clinical findings and the screening of mutations of amelogenin gene of a 7-year-old boy with amelogenesis imperfecta (AI). The genomic DNA was extracted from saliva of patient and his family, followed by PCR and direct DNA sequencing. The c.261C>T mutation was found in samples of mother, father, and brother, but the mutation was not found in the sequence of the patient. This mutation is a silent mutation and a single-nucleotide polymorphism (rs2106416). Thus, it is suggested that the mutation found was not related to the clinical presence of AI. Further research is necessary to examine larger number of patients and genes related to AI.

ABSTRACT

Complete and partial removable dentures have been used successfully in numerous patients with oligodontia and/or anodontia. However, there is little information in the literature regarding the principles and guidelines to prosthetic rehabilitation for growing children. This case report describes the management of a young child with oligodontia as well as the treatment planning and the prosthetic rehabilitation technique.

ABSTRACT

O objetivo deste estudo foi investigar o efeito da Terapia Fotodinâmica antimicrobiana (TFDa) sobre o Enterococcus faecalis (E. faecalis), in vitro, utilizando a clorofila (CL) como agente fotossensibilizador (FS) e o diodo emissor de luz (LED) como fonte de luz. A cultura pura de E. faecalis foi ativada em caldo de BHI a 37oC por 24h. O cultivo do microrganismo foi centrifugado a 3000rpm por 15min, e o pellet re-suspenso em 0,85% de solução salina. As concentrações da bactéria foram ajustadas para 107UFC mL-1 (unidades formadoras de colônias por mililitro). Diferentes tipos de solventes para a CL foram testados: éter, álcool de cereais, Tween 80 e P-123. 100l do inóculo e o mesmo volume da solução teste foram inseridos em cada poço da placa de microtitulação. A mistura foi agitada e aguardou-se 1min. 25l da suspensão foi removida e diluições seriadas foram realizadas. Alíquotas de cada diluição foram espalhadas na superfície da placa, armazenadas em microaerofilia e incubadas na estufa por 24h a 37oC. O número de UFC foi contado em cada placa. Para o experimento da TFDa, os grupos foram divididos em: controle (G1); TBO 1min (G2); CL+Tween 1min (G3); CL+Tween 5min (G4); CL+P-123 1min (G5); CL+P-123 5min (G6); CL+Tween 1min + LED 1min (G7); CL+Tween 1min + LED 5min (G8); CL+Tween 5min + LED 1min (G9); CL+Tween 5min + LED 5min (G10); CL+P-123 1min + LED 1min (G11); CL+P-123 1min + LED 5min (G12); CL+P-123 5min + LED 1min (G13); CL+P-123 5min + LED 5min (G14); TBO 1min + LED 1min (G15); TBO 1min + LED 5min (G16); LED 1min (G17); LED 5min (G18).Um volume de 100l da suspensão bacteriana foi inserido em poços da placa e o mesmo volume da solução do FS foi adicionado. A CL foi solubilizada em soluções aquosas de Tween ou P-123. Cada poço foi agitado e o tempo de préirradiação foi de 1 ou 5min. The aim of this study was to investigate the effect of Antimicrobial Photodynamic Therapy (aPDT) against Enterococcus faecalis (E. faecalis), in vitro, using chlorophyll (CL) as photosensitizer (FS) agent and light-emitting diode (LED) as light source. Pure culture of E. faecalis was cultivated in BHI broth at 37oC for 24h. The culture was centrifuged at 3000rpm for 15min, and the pellets were resuspended in 0.85% saline solution. The bacterial concentrations were adjusted to 107CFU mL-1 (colony forming units per milliliter). Different types of solvents for CL were tested: ether, grain alcohol, Tween 80 and P-123. 100l of inoculum and the same volume of the test solution were inserted into each well of the microtitre plate. The mixture was mixed and 1 min was awaited. 25l of the suspension was removed and serial dilutions were performed. Aliquots of each dilution were spread on the surface of the plates, stored in microaerophilic conditions and incubated for 24h at 37oC. The number CFU was counted in each plate. For aPDT experiment, the groups were divided into: control (G1); TBO 1min (G2); CL+Tween 1min (G3); CL+Tween 5min (G4); CL+P-123 1min (G5); CL+P-123 5min (G6); CL+Tween 1min + LED 1min (G7); CL+Tween 1min + LED 5min (G8); CL+Tween 5min + LED 1min (G9); CL+Tween 5min + LED 5min (G10); CL+P-123 1min + LED 1min (G11); CL+P-123 1min + LED 5min (G12); CL+P-123 5min + LED 1min (G13); CL+P-123 5min + LED 5min (G14); TBO 1min + LED 1min (G15); TBO 1min + LED 5min (G16); LED 1min (G17); LED 5min (G18). A volume of 100l of bacterial suspension was inserted into the well of microtitre plate and the same volume of FS was added. CL was solubilized in aqueous solution of Tween and P-123. ABSTRACT

Conventional radiographic images are frequently used to detect supernumerary teeth. However, recent developments in 3D imaging systems have enabled dentists to better visualize supernumerary teeth, with better contrast and more details. Cone beam computed tomography (CBCT) has improved diagnosis and treatment planning of patients with supernumerary teeth. The decision to use CBCT should be based on the diagnostic information required. This article presents three case reports of patients with supernumerary teeth to demonstrate the need for accurate diagnosis and treatment planning based on a comprehensive evaluation using CBCT.

ABSTRACT

Mucoceles are common disorders of the minor salivary glands, occurring specially on the lower lip. Their occurrence in newborn babies has rarely been reported. In addition, the surgical removal of oral lesions in babies has often been performed with general anesthesia, which option is usually related to the patients' ages, irrespective of the lesion's size and the access to anatomic region where it is located. The present report, however, details the treatment of a mucocele excision performed in an 8-month-old baby with local anesthesia. This case illustrates well that knowledge of the lesion and dentist's experience in the attendance of babies is important, as often only conservative treatment with local anesthesia is required. Therefore, clinicians and parents should be reassured that early surgical intervention under local anesthesia will provide a swift and satisfactory resolution to the problem.


Mucocele é uma ocorrência comum nas glândulas salivares menores, que acontece, principalmente, no lábio inferior. Em bebês, tem sido raramente reportada na literatura. Em adição, a remoção cirúrgica de lesões orais em bebês tem sido realizada sob anestesia geral, cuja opção geralmente se refere à idade do paciente, sem ter relação com o tamanho da lesão e o acesso à região anatômica onde está localizada. O presente caso clínico, no entanto, descreve a excisão da mucocele realizada em um bebê de 8 meses de idade, sob anestesia local. Esse caso ilustra bem que o conhecimento da lesão e a experiência do dentista no atendimento de bebês é muito importante, já que somente um tratamento conservador sob anestesia local é necessário. Portanto, clínicos e pais devem ser alertados para o fato de que uma intervenção cirúrgica precoce sob anestesia local irá proporcionar uma resolução rápida e satisfatória do problema.

ABSTRACT

The present study aimed to compare the fluoride (F-) release pattern of a nanofilled resin-modified glass ionomer cement (GIC) (Ketac N100 - KN) with available GICs used in dental practice (resin-modified GIC - Vitremer - V; conventional GIC - Ketac Molar - KM) and a nanofilled resin composite (Filtek Supreme - RC). Discs of each material (n=6) were placed into 4 mL of deionized water in sealed polyethylene vials and shaken, for 15 days. F- release (µg F-/cm²) was measured each day using a fluoride-ion specific electrode. Cumulative F- release means were statistically analyzed by linear regression analysis. In order to analyze the differences among materials and the influence of time in the daily F- release, 2-way ANOVA test was performed (α=0.05). The linear fits between the cumulative F- release profiles of RC and KM and time were weak. KN and V presented a strong relationship between cumulative F- release and time. There were significant differences between the daily F- release overtime up to the third day only for GICs materials. The daily F- release means for RC were similar overtime. The results indicate that the F- release profile of the nanofilled resin-modified GIC is comparable to the resin-modified GIC.

ABSTRACT

A Odontologia moderna está voltada para os procedimentos preventivos, mesmo assim a cárie dentária constitui problema frequente na prática clínica odontopediátrica, sendo responsável por destruições coronárias extensas e tratamentos reabilitadores diversos. Nesses casos, o uso de aparelhos protéticos seria o tratamento indicado com a finalidade de restaurar a função mastigatória, normalizar a fonação, a estética, e possibilitar ajuste social e emocional do paciente. Este estudo relata um caso de reabilitação estético-funcional de um paciente de 5 anos de idade. Foi realizada exodontia do segundo molar decíduo superior direito, restauração de elementos superiores e inferiores remanescentes, confecção de coroas de aço nos molares inferiores, controle da cárie e um aparelho protético foi confeccionado para o paciente. O plano de tratamento permitiu restabelecer a função mastigatória, melhorar a fonética e estética da criança, bem como resultou na satisfação, demonstrada pelo paciente e seus familiares


The Modern dentistry is focused on the preventive procedures, even thus dental caries is a frequent problem in Pediatric Dentistry clinical practice, being responsible for extensive dental and coronal destructions and various rehabilitation treatments. In these cases, the use of prosthetic appliances on the treatment would be indicated in order to restore the masticatory function, normalize the speech, aesthetics, and provides social and emotional adjustment of the patient. This case reports a aesthetic-rehabilitation functioning of a 5 years-old patient. We performed extraction of the upper right second deciduous molar, upper and restoration less remaining, making steel crowns on molars, caries control and a prosthetic device was fabricated for the patient. The treatment plan allowed restore chewing function, improve phonetics and aesthetics of the child and resulted in satisfaction, demonstrated by patients and their relatives

Impaction of permanent teeth represents a clinical challenge with regard to diagnosis, treatment plan, and prognosis. There is a close relationship between deciduous teeth and permanent teeth germ, and any injury in the deciduous dentition may influence the permanent teeth eruption. The extent of the damage caused to the permanent teeth germ depends on the patient age at the time of injury, type of trauma, severity, and direction of the impact. Conventional radiographic images are frequently used for diagnosis; however, recent developments in three-dimensional (3D) imaging systems have enabled dentistry to visualize structural changes effectively, with better contrast and more details, close to the reality. The cone-beam computed tomography (CBCT) has been used in the diagnosis and treatment plan of these impacted teeth. The purpose of the present case report is to describe a successful conservative management of a retained permanent maxillary lateral incisor with delayed root development after a trauma through the deciduous predecessor in a 9 year-old patient. After clinical and radiographic examination, a CBCT examination of the maxilla was requested to complement the diagnosis, providing an accurate 3D position of the retained tooth and its relationship to adjacent structures. The proposed treatment plan was the surgical exposure and orthodontic traction of the retained tooth. The lateral incisor spontaneously erupted after 6 months. Therefore, this case report suggests that permanent teeth with incomplete root formation have a great potential for spontaneous eruption because no tooth malposition or mechanical obstacles are observed.

ABSTRACT

Tomografia Computadorizada de Feixe Cônico é uma técnica de aquisição de imagens queoferece diversas vantagens na prática clínica odontológica por produzir imagens tridimensionais da região dentomaxilofacial. Em Odontopediatria, a avaliação de estruturas em diferentes planos é necessária especialmente em casos complexos, nos quais as radiografias convencionais não fornecem com exatidão informações importantes. O presente estudo tem como objetivo relatar três casos clínicos de diferentes alterações buco-dentárias em que a utilização da Tomografia Computadorizada de Feixe Cônico auxiliou na determinação de um correto diagnóstico e definição de um plano de tratamento adequado.


Cone Beam Computed Tomography is a technique that offers many advantages in dentistry because it produces three-dimensional imaging of the dentomaxillofacial structures. In pediatric dentistry, the need to evaluate structures in different planes is noticed especially in complex cases, in which conventional radiographs do not show, in an accurate way, some important information. Therefore, the purpose of this study is to report three cases of different alterations which the use of Cone Beam Computed Tomography helped us for correct diagnosis and adequate treatment planning.

PURPOSE: This study's purpose was to assess the prevalence of dental erosion in adolescents and to investigate the association between erosion and sociodemographic characteristics. METHODS: This study consisted of a single center cluster random sample of 414 adolescents (12 and 16 years old) of both genders from private and public schools in Bauru, São Paulo, Brazil. Two previously calibrated examiners (kappa=0.85) used the O'Brien (1994) index for assessment of dental erosion on the buccal and palatal surfaces of the permanent maxillary incisors and on the occlusal surfaces of the permanent first molars. Data on gender, type of school, and mean family income were collected by a questionnaire completed by the adolescents. Descriptive statistics were applied to the data, and the associations between erosion and sociodemographic variables were investigated by chi-square test (P<0.05). RESULTS: The prevalence of dental erosion was 20% (N=83), with only enamel being involved (score 1). The labial surface was the most affected (16%). There were no significant differences between the presence of erosion and gender, type of school, and mean family income. CONCLUSIONS: The results indicate that dental erosion restricted to enamel was observed among adolescents in Bauru/Brazil, and there was no correlation between this condition and sociodemographic factors.

BACKGROUND: Dental erosion is a multifactorial disease and is associated with dietary habits in infancy and adolescence. AIM: To investigate possible associations among dental erosion and diet, medical history and lifestyle habits in Brazilian schoolchildren. DESIGN: The sample consisted of a random single centre cluster of 414 adolescents (12- and 16-years old) of both genders from private and public schools in Bauru (Brazil). The O'Brien [Children's Dental Health in the United Kingdom, 1993 (1994) HMSO, London] index was used for dental erosion assessment. Data on medical history, rate and frequency of food and drinks consumption, and lifestyle habits were collected by a self-reported questionnaire. Odds ratios with 95% confidence intervals were used to assess the univariate relationships between variables. Analysis of questionnaire items was performed by multiple logistic regression analysis. The statistical significance level was set at 5%. RESULTS: The erosion present group comprised 83 subjects and the erosion absent group 331. There were no statistically significant correlations among dental erosion and the consumption of food and drinks, medical history, or lifestyle habits. CONCLUSION: The results indicate that there was no correlation between dental erosion and the risk factors analysed among adolescents in Bauru/Brazil and further investigations are necessary to clarify the multifactorial etiology of this condition.

O presente estudo teve como objetivo comparar o padrão de liberação de fluoreto (F-) de um cimento de ionômero de vidro (CIV) nanoparticulado modificado por resina (Ketac N100 - KN) com CIVs disponíveis na prática clínica (CIV modificado por resina - Vitremer - V; CIV convencional - Ketac Molar - KM) e uma resina composta nanoparticulada (Filtek Supreme - RC). Discos de cada material (n=6) foram imersos em 4 mL de água deionizada em frascos de polietileno e agitados durante 15 dias. A liberação de F- (μg F-/cm²) foi medida a cada dia utilizando um eletrodo de F- específico. Os valores de liberação cumulativa de F- foram analisados estatisticamente por análise de regressão linear. Com o objetivo de analisar as diferenças entre os materiais e a influência do tempo na liberação diária de F- foi aplicado o teste ANOVA a dois critérios (α=0,05). A relação entre os padrões de liberação de cumulativo de F- da RC e KM e o tempo foram fracas. Os materiais KN e V apresentaram uma relação forte entre a liberação cumulativa de F- e o tempo. Diferenças significativas foram observadas entre a liberação diária de F- até o terceiro dia somente para os cimentos ionoméricos. Os resultados indicam que o padrão de liberação de F- do CIV modificado por resina nanoparticulado é semelhante ao CIV modificado por resina.

ABSTRACT

Foram avaliados os prontuários de atendimentos de crianças de 0 a 12 anos de idade, atendidas no Setor de Urgência Odontológica (SUO) da Faculdade de Odontologia de Bauru, da Universidade de São Paulo, no período compreendido entre os anos de 2004 a 2008 que continham informações completas sobre o paciente e o tipo de atendimento realizado. Os dados analisados foram: idade, gênero, motivo da visita, exames, tratamentos realizados e tipo de encaminhamento. As crianças foram divididas em dois grupos de acordo com a injúria sofrida: traumáticas e não-traumatícas. Os dados foram tabulados e submetidos a uma análise descritiva. Do total de pacientes atendidos no SUO, nesse intervalo de 60 meses, 1236 (9,05%) eram crianças, sendo que a maoria apresentava idade ente 7 e 12 anos (62,78%). Enquanto as lesões traumáticas foram responsáveis por 18,52% (229) dos atendimentos, as injúrias não-traumáticas foram responsáveis por 81,47% (1007). O tratamento para ambos estava de acordo com o preconizado para esse tipo de serviço. Por meio do presente estudo, pôde-se concluir que a cárie dentária foi a lesão que gerou a maior ocorrência dos atendimentos realizados pelo SUO, o que indica uma maior necessidade de orientação em saúde bucal da população, com ênfase nas ações preventivas.


The attendance's records of children between 0 to 12 years old assisted at Urgency Dental Service (UDS) from Bauru Dental School, University of São Paulo were assessed in the period among the years 2004 and 2008, which only the records that containing all information completed about the patient or type of treatment were analyzed. The analyzed data were age, gender, main complaint, exams, performed treatmente records and follow-up care. They were divided into two groups in relation to the involved injury: traumatic and non-traumatic. Data were plotted and submitted to a descriptive statistical analysis. Among the total of patients attended at UDS at this period of 60 months, 1236 (9.05%) were children the majority was between 7 and 12 years old (62.78%). While traumatic lesions were the cause of 229 (18.52%) of treatments, the non-traumatic injuries were the cause of 1007 (81.47%). The treatment performed in both situations was done according to the indicated protocol for each case. Through this study it was concluded that dental caries was the most frequent lesion of the treatments provided by UDS, which indicates a higher need for oral health orientation of the population with emphasis in preventive measures.

Foram avaliados os prontuários de atendimentos de crianças de 0 a 12 anos de idade, atendidas no Setor de Urgência Odontológica (SUO) da Faculdade de Odontologia de Bauru, da Universidade de São Paulo, no período compreendido entre os anos de 2004 a 2008 que continham informações completas sobre o paciente e o tipo de atendimento realizado. Os dados analisados foram: idade, gênero, motivo da visita, exames, tratamentos realizados e tipo de encaminhamento. As crianças foram divididas em dois grupos de acordo com a injúria sofrida: traumáticas e não-traumatícas. Os dados foram tabulados e submetidos a uma análise descritiva. ABSTRACT

O objetivo deste estudo foi avaliar a prevalência da erosão dentária em adolescentes e os possíveis fatores etiológicos extrínsecos e intrínsecos envolvidos nesse processo. A amostra consistiu de 414 adolescentes com 12 e 16 anos de idade, de ambos os gêneros, matriculados em escolas públicas ou privadas da cidade de Bauru/SP. O exame clínico foi realizado por dois examinadores previamente calibrados (Kappa = 0.85), que utilizaram o índice de OBrien (1994) para avaliar as faces vestibulares e palatinas dos incisivos superiores e a oclusal dos primeiros molares permanentes. Um questionário foi aplicado para pesquisar sobre dieta, história médica e hábitos dos adolescentes. Os dados foram devidamente analisados através da estatística descritiva e dos testes do Qui-Quadrado e de Mann-Whitney. Foi adotado nível de significância de 5% para que as diferenças sejam consideradas estatisticamente significantes. A prevalência de erosão foi de 20% na amostra estudada, com envolvimento apenas de esmalte. Nenhuma superfície apresentou erosão em dentina (código 2) ou exposição pulpar (código 3). A face vestibular foi a mais acometida (16,1%) e a face oclusal a menos acometida (1,3%). Não houve diferenças estatisticamente significantes entre gênero, condição sócio-econômica e tipo de escola. Nenhum dos fatores de risco pesquisados no questionário apresentou uma associação significativa com a erosão. Os resultados deste estudo sugerem que a erosão dentária foi uma condição observada na amostra estudada e não houve correlação entre esta ocorrência e as condições e hábitos de vida desta população.


The aim of this study was to assess the prevalence of dental erosion in adolescents and determine the possible etiological factors. A sample of 414 adolescents (12- and 16-year-old) from private and public schools in Bauru participated in this study. For the dental examination, two examiners previously calibrated (kappa=0,85) made the evaluation on the labial and palatine surfaces of the maxillary permanent incisors and on the oclusal surface of the first permanent molars for the presence of dental erosion using the OBrien (1994) index. Data on dietary habits, oral hygiene practices and medical background were collected by a structured questionnaire completed by the adolescents. Descriptive statistics were applied to the data and the associations between dental erosion and the variables under study were investigated by Chi-square and Mann-Whitney analysis. The statistical significance level was set at 5%. The prevalence of dental erosion was 20% in this sample, with only enamel been involved. Erosion affecting dentine (score 2) or pulpar exposure (score 3) were not observed. The labial was the most affected surface (16,1%) and the oclusal surface the least (1,3%). There were no statistically significant differences between the presence of erosion and gender, socioeconomic status and type of school. The risk factors investigated in the questionnaire were not associated with the presence of erosion. The results of this study indicate that dental erosion was observed among adolescents in Bauru/SP and there was no correlation between this condition and the life habits of this population.
