ABSTRACT

In experimental study (40 dogs) variant of precisional (microsurgical) suture and anastomosis was developed, which was successfully carried out in clinical practice in 72 patients with various diseases of gastro-intestinal tract. For precisional formation of gastro-intestinal and bowel anastomoses magnifying optics, atraumatic suturing material and microsurgical instruments were used. Separately were sutured by knot sutures gastric or bowel serosa (external layer) and musculosubcutaneos layer of these organs together with mucous membrane tangentially, without its penetrating puncture (internal layer). The developed precisional method of formation of the anastomoses promotes healing of gastro-intestinal wound by primary intention and may be recommended for wider practical application.

ABSTRACT

Experiments were conducted on 24 dogs to elaborate two variants of precision (microsurgical) double-row suture in operations on the large intestine. The suggested variants of the suture were successfully applied in operations on the large intestine in 48 patients. The first variant of a double-row sero-serous and musculo-intramucous suture is technically simpler and preferable in formation of end to side, side to side ileotransfercal anastomoses, and endo-to end anastomoses of the large intestine. The variant of sero-serous and musculo-intramucous suture with double stitching of the submucous coat on each side of the created anastomosis is preferable in complicated restorative-re constructive interventions. Both variants (the second to a greater measure) ensure sufficient strength and air-tightness of the formed anastomoses, exact approximation of the layers of the edges of the intestine, and quicker healing of the intestinal wound by first intention. The suggested sutures make it possible to decline the formation of preventive relieving colostomas and transanal draining of large intestinal anastomoses.

ABSTRACT

The authors discuss the literature data and analyse three personal cases. They show that inflammatory pseudotumors of the abdominal cavity, especially those arising from the mesocolon, are distinguished with great difficulty from true malignant tumors macroscopically. In none of their personal cases was the correct diagnosis established during surgery and extended or combined operations were carried out. The authors claim that interventions of a lesser volume can be undertaken only when skilled emergency biopsy of different areas of the new growth yields negative results.
