ABSTRACT

BACKGROUND: Skin cancer is one of the most frequent types of cancer, and cutaneous squamous cell carcinoma (cSCC) constitutes 20% of non-melanoma skin cancer (NMSC) cases. PTCH1, a tumor suppressor gene involved in the Sonic hedgehog signaling pathway, plays a crucial role in neoplastic processes. METHODS: An analytical cross-sectional study, encompassing 211 cSCC patients and 290 individuals in a control group (CG), was performed. A subgroup of samples was considered for the relative expression analysis, and the results were obtained using quantitative real-time PCR (qPCR) with TaqMan® probes. The functional, splicing, and disease-causing effects of the proposed variants were explored via bioinformatics. RESULTS: cSCC was predominant in men, especially in sun-exposed areas such as the head and neck. No statistically significant differences were found regarding the rs357564, rs2236405, rs2297086, and rs41313327 variants of PTCH1, or in the risk of cSCC, nor in the mRNA expression between the cSCC group and CG. A functional effect of rs357564 and a disease-causing relation to rs41313327 was identified. CONCLUSION: The proposed variants were not associated with cSCC risk in this Mexican population, but we recognize the need for analyzing larger population groups to elucidate the disease-causing role of rare variants.

ABSTRACT

OBJECTIVE: Identify psychosocial factors influencing food waste mitigation and explore motivations and strategies for successful conservation among self-identified food conservers. METHODS: Mixed-methods study consisting of an online survey estimating food waste production and psychosocial factors and a focus group to explore waste mitigation strategies and motivations. RESULTS: Sampled 27 self-identified conservers (female, aged 18-30 years, White/Asian). Mean household food waste was 6.6 cups/wk (range, 0.0-97.9 cups/wk; median 1.3 cups). Reported waste mitigation strategies include proactive mitigation and adaptive recovery measures in each phase of the food management continuum. Conservers reported various intrinsic and extrinsic motivations to reduce food waste and viewed barriers as manageable. CONCLUSIONS AND IMPLICATIONS: Food conservers act on high intentions to reduce waste by consistently employing both proactive waste mitigation and adaptive food recovery measures. Future research is needed to determine if these findings hold in larger, more diverse samples and link specific behaviors to waste volume.

ABSTRACT

BACKGROUND: Basal cell carcinoma (BCC) represents about 80% of all cases of skin cancer. The PTCH1 is a transmembrane protein of the Sonic Hedgehog signaling pathway that regulates cell proliferation. Genetic variants in PTCH1 gene have been previously described in association with BCC development. In addition, PTCH1 mRNA and protein expression analysis are also significant to understand its role in skin cancer physiopathology. METHODS: An analytical cross-sectional study was performed, and a total of 250 BCC patients and 290 subjects from the control group (CG) were included, all born in western Mexico. The genotypes and relative expression of the mRNA were determined by TaqMan® assay. The protein expression was investigated in 70 BCC paraffin-embedded samples with PTCH1 antibodies. Semi-quantitative analysis was performed to determine the expression level in the immunostained cells. RESULTS: We did not find evidence of an association between PTCH1 rs357564, rs2297086, rs2236405, and rs41313327 genetic variants and susceptibility to BCC. Likewise, no statistically significant differences were found in the comparison of the mRNA level expression between BCC and CG (p > 0.05). The PTCH1 protein showed a low expression in 6 of the analyzed samples and moderate expression in 1 sample. No association was found between genetic variants, protein expression, and demographic-clinical characteristics (p > 0.05). CONCLUSION: The studied PTCH1 variants may not be associated with BCC development in the Western Mexico population. The PTCH1 mRNA levels were lower in patients with BCC compared to the control group, but its protein was underexpressed in the tissue samples.

BACKGROUND: Some cytokines are strongly implicated in the development of squamous cell carcinoma (SCC) such as the Macrophage migration inhibitory factor (MIF). The haplotype -794 (CATT)5-8 /-173G>C in MIF gene polymorphisms has been associated with some types of cancer. The aim of this study is to establish the possible association between the presence of this haplotype in the MIF gene and its subsequent soluble levels with the susceptibility of SCC in western Mexican population. METHODS: This study included 175 SCC patients and 175 age-sex-matched individuals as a reference group (RG) from western Mexico. Genomic DNA was extracted from peripheral blood leukocytes. Polymorphisms were genotyped by endpoint PCR and PCR-RFLP, and the determination of MIF serum levels was measured by ELISA. Clinical characteristics were evaluated by a group of dermatologists. RESULTS: Analysis of [-794(CATT)5-8 /-173G>C] MIF gene polymorphisms showed that the 5C (OR = 2.7, p = 0.02) and the 7G (OR = 3.39, p < 0.01) haplotypes are associated with susceptibility in SCC. MIF soluble levels in SCC patients showed a median of 13.93 ng/mL, whereas the reference group showed 6.000 ng/mL. CONCLUSIONS: Our findings suggest that 5C and 7G [-794(CATT)5-8 /-173G>C] MIF gene haplotypes are associated with susceptibility to SCC and that SCC patients present increased soluble levels of MIF.

ABSTRACT

Duddingtonia flagrans is a nematophagous fungus employed as a biocontrol agent of gastrointestinal nematodes in ruminants. After oral ingestion and passage through the digestive tract of animals, this microorganism captures the nematodes in the feces. The drastic conditions of ruminant digestive tract could affect fungi chlamydospores and therefore biocontrol activity. The aim of this study was to evaluate in vitro the effect of four ruminant digestive segments on the concentration and nematode predatory ability of a Colombian native strain of D. flagrans. The sequential four-step methodology proposed evaluated conditions of the oral cavity, rumen, abomasum, and small intestine such as pH (2, 6, 8), enzymes (pepsin, pancreatin), temperature (39 °C), and anaerobiosis comparing short (7 h) and long (51 h) exposure times. The results showed that the nematode predatory ability of the fungi is affected by sequential exposure to gastrointestinal segments and this effect depends on the exposure time to those conditions. After short exposure (7 h) through the four ruminant digestive segments, the fungi had a nematode predatory ability of 62%, in contrast, after long exposure (51 h) the nematode predatory ability was lost (0%). Moreover, the number of broken chlamydospores was higher in the long-exposure assay.

RESUMEN Durante la pandemia por la COVID-19 la atención odontológica fue recomendada solo en casos de urgencia y dolor. La reducción de citas dentales puede afectar la economía de los dentistas, principalmente la de aquellos que no cuentan con seguridad financiera. Los odontólogos por necesidades económicas requieren brindar atención a pesar del riesgo de ser infectados por SARS-COV-2. Objetivo : Describir la frecuencia de la atención odontológica y el riesgo de complicación por COVID-19 en odontólogos de Ixtlahuaca, Estado de México, México. Material y métodos : Estudio transversal, muestreo no probabilístico desarrollado desde agosto hasta septiembre de 2020. Se aplicó un cuestionario por redes sociales para identificar la frecuencia de la atención odontológica. Resultados: La muestra estuvo conformada por 121 odontólogos; el promedio de edad fue de 32,3 años. La mayor parte de la muestra estuvo conformada por mujeres (74,4 %). Los odontólogos mostraron un riesgo medio a desarrollar complicaciones por COVID-19 (92,5 %). Los odontólogos de práctica privada realizaron más tratamientos no urgentes durante el semáforo epidemiológico rojo y naranja, mostrando diferencias estadísticamente significativas (p ≤ 0,05). El 30 % de la muestra presentó un peso no saludable. Conclusiones : Los odontólogos tienen un riesgo medio a enfermar gravemente por COVID-19. El riesgo se encuentra asociado al peso no saludable. Los odontólogos realizaron tratamientos no urgentes a pesar de las recomendaciones gubernamentales. Los profesionales de práctica privada realizaron una mayor cantidad de tratamientos no urgentes. Los dentistas brindan atención a pesar del riesgo epidemiológico por COVID-19 debido a las necesidades económicas.


ABSTRACT During the COVID-19 pandemic, dental care was recommended only in urgent and severe pain cases. Reduced dental appointments may affect the financial situation of dentists, especially those who are not financially secure. Due to economic necessity, dentists are required to provide care despite the risk of being infected with SARS-COV-2. Objective: To describe the frequency of dental care and the risk of complications due to COVID-19 among dentists in Ixtlahuaca, State of Mexico, Mexico. Material and methods: Cross-sectional study, non-probabilistic sampling carried out from August to September 2020. A survey was carried out through social networks to identify the frequency of dental care. Results: The sample consisted of 121 dentists with an average age of 32.3 years. Women comprised most of the sample (74.4%). Dentists had a medium risk of suffering complications due to COVID-19 (92.5%). Private practice dentists performed more non-urgent treatments during the red and orange epidemiological risk periods, showing statistically significant differences (p≤0.05). Unhealthy weight was present in 30% of the sample. Conclusions: Dentists have a medium risk of becoming seriously ill from COVID-19. The risk is associated with unhealthy weight. Dentists performed non-urgent treatments despite government recommendations. Private practitioners performed a greater number of non-urgent treatments. Dentists provide care despite epidemiological risk related to COVID-19 due to economic necessity.

INTRODUCTION: Children with haemophilia (CwH) have lower bone mineral density in the spine (trabecular bone) than healthy children. There are few studies focusing on bone mineral density in long bones (cortical bone). AIM: To evaluate bi-laterally the distal third of radius and midshaft tibias using quantitative ultrasound (QUS) and assess the speed of sound (SoS). METHODS: A cross-sectional study where 91 CwH and 91 age-matched healthy boys were included. Joint evaluation was determined with the Haemophilia Joint Health Score 2.1 and SoS values. The Z scores were measured with the Sunlight Omnisense 8000 S equipment. RESULTS: Ninety-one CwH (haemophilia A) were evaluated (26 mild form, 26 moderate, and 39 severe). Most patients were treated with on-demand factor replacement and had higher total HJHS scores according to severity (4.8, 14.8 and 14.1, respectively). Patients with moderate and severe disease showed a statistical difference in SoS values for both radius and tibias compared to controls. QUS Z-scores ≥-2 were more frequent in radius and tibias in CwH, but were statistically significant only in tibias when compared to controls (in 30% mild, 46% moderate, 28% severe, respectively). CONCLUSIONS: There was diminished cortical bone density in radius and tibias of CwH compared to healthy controls. Changes predominated in tibias, more frequently affected according to the severity of haemophilia. Early intervention with factor replacement combined with physical activity are key aspects to promote bone health.

Introduction: There is increasing evidence that immunohistochemical expression of p53, Ki-67, and Bcl-2 is associated with aggressive (aBCC) and less aggressive (nBCC) histological subtypes and may have a prognostic role. Aim: To investigate the clinicopathological features and immunohistochemical expressions of p53, Ki-67, and Bcl-2 in cutaneous basal cell carcinoma focusing on histological subtypes. Their roles and possible interactions in the development and progression of BCC are discussed. Material and methods: A total of 50 BCC samples from 50 patients from Western Mexico between June 2018 and June 2019 were included. Paraffin-embedded samples were immunostained with p53, Ki-67, and Bcl-2 antibodies. Semi-quantitative analysis was performed to determine the intensity and positivity of immunostained cells. Parametrical and non-parametrical tests were performed according to the sample's distribution. Results: Samples included 21 nBCC and 29 aBCC. The statistical analysis showed statistical association when grouped as non-aggressive and aggressive subtypes for p53 (p = 0.04) and Bcl-2 (p < 0.01). An inverse negative correlation was found between age and Bcl-2 expression. No statistical association was found between Ki-67 immunoreactivity and any of the other variables. Conclusions: We found that a high expression of Bcl-2 and a low expression of p53 was associated with more indolent histopathological features of BCC and therefore better outcomes. These findings suggest that examination of p53 and Bcl-2 expression in BCC patients may provide valuable prognostic information. These biomarkers may play a role in the development and progression of some cases of BCC.

Introduction: Hair straightening products like the Brazilian Keratin Treatment (BKT) contain high concentrations of formaldehyde, and its use is associated with adverse effects. In 2016, seven cases of eczema-like psoriasiform skin reaction secondary to BKT were described for the first time. We aim to investigate the clinical characteristics and dermatoscopic findings of patients with psoriasiform skin reactions due to BKT. Materials and Methods: A cross-sectional study was performed from October 2017 to June 2020. Patients who developed erythema and scales on the scalp following the use of BKT were included. Age, sex, number of BKTs, time elapsed between BKT and the skin reaction, pull test, and dermatoscopic findings were investigated. Descriptive and inferential statistics were used. Results: We found 43 patients with a mean age of 35 ± 10 years, predominantly females in 42 (98%) cases. The mean number of BKTs was 2 ± 2. The mean length of time elapsed to present the reaction was 12 ± 17 months and this was related to the number of BKTs (P = 0.01). The pull test was positive in 37 (86%) patients. The most frequent dermatoscopic findings were perifollicular scales in 42 (98%), red patches in 35 (81%), and peripilar desquamation resembling the outer skin of an onion bulb in 32 (74%). Conclusion: Hair straightening products are widely used and the psoriasiform skin reaction that develops afterward might be underdiagnosed. It is important to investigate the background of BKT in conjunction with the clinical and dermatoscopic findings suggestive of this cutaneous reaction.

Beverages can provide improved nutrient intake and hydration, but also pose concerns related to overnutrition or contamination for children and adolescents who are in a time of critical growth. This narrative review aims to understand the impact of milk, 100% juice, and water consumption on health-related outcomes in youth. The literature review conducted used PubMed, Web of Science, and CABI global. Forty-five research articles met the quality criteria and were included. Health organization and governmental resources were also reviewed to identify current intake and consumption recommendations. All beverages in this review were associated with a variety of desirable and undesirable findings that spanned over 40 different health outcomes. Most studies that assessed milk lacked clear distinction between milk type (flavored vs. unflavored) or fat percentage, making it difficult to understand the impact of milk consumption. The relationship between milk intake and anthropometric-related outcomes were mixed within and across studies. Water was consistently associated with better hydration, while 100% juice and flavored milk intake was associated with more desirable dietary patterns or nutrients that children are currently not consuming adequate amounts of. The implications of these findings were discussed in the context of the National School Lunch Program (NSLP), while considering the impact of issues such as contaminated water and lactose intolerance. This review suggests that water may be an optimal default beverage option in the NSLP to promote hydration and accommodate beverage preferences for those with lactose intolerance.

ABSTRACT

KDM4 proteins are a subfamily of histone demethylases that target the trimethylation of lysines 9 and 36 of histone H3, which are associated with transcriptional repression and elongation respectively. Their deregulation in cancer may lead to chromatin structure alteration and transcriptional defects that could promote malignancy. Despite that KDM4 proteins are promising drug targets in cancer therapy, only a few drugs have been described as inhibitors of these enzymes, while studies on natural compounds as possible inhibitors are still needed. Natural compounds are a major source of biologically active substances and many are known to target epigenetic processes such as DNA methylation and histone deacetylation, making them a rich source for the discovery of new histone demethylase inhibitors. Here, using transcriptomic analyses we determined that the KDM4 family is deregulated and associated with a poor prognosis in multiple neoplastic tissues. Also, by molecular docking and molecular dynamics approaches, we screened the COCONUT database to search for inhibitors of natural origin compared to FDA-approved drugs and DrugBank databases. We found that molecules from natural products presented the best scores in the FRED docking analysis. Molecules with sugars, aromatic rings, and the presence of OH or O- groups favor the interaction with the active site of KDM4 subfamily proteins. Finally, we integrated a protein-protein interaction network to correlate data from transcriptomic analysis and docking screenings to propose FDA-approved drugs that could be used as multitarget therapies or in combination with the potential natural inhibitors of KDM4 enzymes. This study highlights the relevance of the KDM4 family in cancer and proposes natural compounds that could be used as potential therapies.

BACKGROUND: Initial publications of COVID-19 (2019 coronavirus disease) focused on the adult population until March 2020, when the first series in children was reported. Our objective was to analyze the spatiotemporal behavior of the pediatric population with COVID-19 in the state of Jalisco. METHODS: We conducted a cross-sectional study including subjects < 18 years of age with SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) infection confirmed by reverse transcription-polymerase chain reaction, registered in the RADAR platform. We investigated the prevalence, incidence rate, age, sex, outpatient or inpatient status, distribution of cases by time, municipality of residence, and geographical region. Descriptive statistics were used for data analysis. RESULTS: Of 58,231 subjects studied, 1,515 were children (3%): 768 males (51%) and 747 females (49%). The mean age was 12 ± 5 years; outpatients predominated (94%). The Central region concentrated the largest cases with 1,257 (82%) and was the second-highest incidence rate, behind the Occidental Coastal-Mountain region. The most affected municipality was Guadalajara. The distribution of new cases increased proportionally to mobility: after the holiday weekend in May, it rose from 28 to 161 cases; after the opening of beaches and recreational sites in June and July, to 539; and after the opening of movie theaters in August, to 673 cases. CONCLUSIONS: Although with a lower incidence, the pediatric population is not exempt from SARS-CoV-2 infection. We observed an increase in cases as restrictions on social activities diminished.


INTRODUCCIÓN: Las publicaciones iniciales de COVID-19 (enfermedad por coronavirus de 2019) se enfocaron en población adulta, hasta marzo de 2020, cuando se informó la primera serie en niños. Nuestro objetivo fue analizar el comportamiento espacio-temporal de la población pediátrica con COVID-19 en el estado de Jalisco. MÉTODOS: Se llevó a cabo un estudio transversal en el que se incluyeron sujetos < 18 años con infección por SARS-CoV-2 (coronavirus tipo 2 del síndrome respiratorio agudo grave) confirmada por reacción en cadena de la polimerasa con retrotranscriptasa, registrados en la plataforma RADAR. Se investigó la prevalencia, tasa de incidencia, edad, sexo, paciente ambulatorio u hospitalizado, distribución de casos por tiempo, municipio de residencia y región geográfica. Se utilizó estadística descriptiva para el análisis de los datos. RESULTADOS: De 58,231 sujetos estudiados, se encontraron 1,515 pacientes pediátricos (3%): 768 de sexo masculino (51%) y 747 de sexo femenino (49%). La media de edad fue de 12 ± 5 años; predominaron los pacientes ambulatorios (94%). La región Centro concentró la mayor cantidad de casos con 1,257 (82%) y fue la segunda con mayor tasa de incidencia, detrás de la región Costa-Sierra Occidental. El municipio más afectado fue Guadalajara. La distribución de nuevos casos incrementó al aumentar la movilidad: después del puente vacacional de mayo subió de 28 a 161 casos; tras la apertura de playas y sitios de recreación en junio y julio, a 539 casos, y posterior a la apertura de cines en agosto, a 673 casos. CONCLUSIONES: Aunque con una incidencia menor, la población pediátrica no está exenta de la infección por SARS-CoV-2. Se observó un incremento de los casos a medida que disminuyeron las restricciones para las actividades sociales.

Introducción: la leptospirosis es una enfermedad infecciosa producida por espiroquetas del género Leptospira. Se disemina a través de la orina de animales domésticos, con mayor frecuencia por roedores. En República Dominicana se necesitan pruebas confiables para el diagnóstico en etapas tempranas de la enfermedad. En el presente trabajo se aborda la validez diagnóstica del PCR en Tiempo Real y del IGM (INMUNODOT) en comparación con la prueba de Microaglutinación (MAT). Materiales y métodos: se realizó un estudio observacional, descriptivo, de corte transversal con 69 pacientes admitidos en el Hospital Regional "José María Cabral y Báez" con diagnóstico presuntivo de leptospirosis desde el 2010 hasta el 2012. Resultados: del total de los casos (69), la mayoría fueron del sexo masculino (94.2 %), menores de 49 años (79.4 %) y provenían de la provincia de Santiago (58 %). La mortalidad fue de 52.1 % de los cuales 52.3 % fueron reportados positivos para Leptospira y 47.6 % resultaron negativos según el MAT. Al comparar los resultados de PCR en relación al MAT se obtuvo una sensibilidad de 27.3 % y una especificidad de 80 %. Los resultados del PCR y del Immunodot fueron equivalentes. Conclusión: en el presente trabajo la realización del PCR en sangre, después del 5to día de inicio de la enfermedad, no demostró ser mejor que la Inmunodot para la detección temprana de la enfermedad, al contrastarlos con el resultado del MAT. En República Dominicana, los casos hospitalizados con diagnóstico presuntivo de leptospirosis permanecen con muy alta mortalidad. Por tanto, es prioritario optimizar el diagnóstico y el tratamiento de estos casos. En este estudio, los casos confirmados con Leptospirosis que fallecieron indican que debe actualizarse el protocolo de tratamiento y asegurarse que pueda implementarse. Por otra parte, los casos no confirmados fallecidos exigen investigar otras causas de enfermedad como la Infección por Hanta Virus.


Introduction: Leptospirosis is an infectious disease caused by spirochaetes of the genus Leptospira. It spread through the urine of domestic animals most frequently in rodents. Need reliable tests to diagnose in early stages of the disease and it has been proposed the use of the PCR in real-time as an option. The present work deals with the diagnostic real-time PCR and IGM (INMUNODOT) in comparison with the Microagglutination (MAT) test. Materials and methods: An observational, descriptive, cross-sectional study with 69 patients admitted as possible Leptospirosis in the Hospital "Jose Maria Cabral y Báez" of Santiago from 2010 to 2012. Results: Of the total cases (69), most were male (94.2%), under 49 years of age (79.4%) coming from the province of Santiago (58%). Mortality was 52.1% of which 52.3% were reported positive for leptospirosis and 47.6% were negative according to the MAT. To compare the results of PCR in relation to the MAT was obtained a sensitivity of 27.3% and a specificity of 80%. The results of the PCR and immunodot were equivalent. Conclusion: In this study, the realization of the PCR in blood after the 5th day of the disease not proved to be better than the Inmunodot for the early detection of the disease. Is necessary to evaluate cases less than 5th day. In the Dominican Republic, hospitalized with a presumptive diagnosis of Leptospirosis cases remain with very high mortality. Therefore, it is important to optimize the diagnosis and treatment of these cases. In this study, confirmed cases with Leptospirosis who died, indicate that you must upgrade the treatment protocol and ensure that it can be implemented. On the other hand, the deceased not confirmed cases require to investigate other causes of disease as the Hanta Virus infection.

BACKGROUND: Lymphocytoma cutis (LC) is a benign reactive lymphoproliferative B-cell process. It has two variants: localized type with solitary lesions and miliarial type with numerous lesions. The objective was to investigate the characteristics of LC with emphasis on the miliarial type. METHODS: Retrospective study, patients with clinical and histopathological diagnosis of LC were included. Age, sex, evolution time, affected site, and type of treatment were investigated. In miliarial-type LC, the histological and immunohistochemical characteristics were also investigated. RESULTS: In an 18-year period, there were 102 patients found with LC: 72 (71%) corresponded to females, the median age was 45 years, the median evolution time was 4 months, and the face was the most predominant affected area in 81 (79%) cases. Localized-type LC corresponded to 88 (86%) cases, and miliarial type in 14 (14%). The most common treatment was surgery, which was used in 32 (31%) patients, all of whom had localized type (P < 0.01). The most frequent treatment for miliarial-type LC was corticosteroids in five (36%, P = 0.32), the predominant histopathological pattern was nodular in 10 (71%) specimens, and immunohistochemistry was performed in 11 (79%), where all were positive for CD20 with polyclonality to kappa and lambda light chains. CONCLUSIONS: The importance of LC lies in that it can be clinically and histopathologically confused with cutaneous lymphoma and that it is a rare entity, with its miliarial variant being rarer still. This study provides information on the clinical-histological characteristics of LC and its immunohistochemistry.

BACKGROUND: The Service Quality in Hospital (SERVQHOS) assesses quality and satisfaction with hospital care received. This study aimed to determine the quality and satisfaction of parents in a tertiary-level pediatric public facility in Mexico. METHODS: We conducted a cross-sectional study in which 425 anonymous surveys were distributed during the discharge of children. The questionnaire evaluates the individual (subjective) and organizational (objective) quality of service: reliability, tangibles, assurance, responsiveness, and empathy, as well as satisfaction on a 5-point scale from 1 (much worse) to 5 (much better). RESULTS: A total of 401 questionnaires were returned (94%). The mean quality score was 3.6 ± 0.7. The best-rated aspects were the medical equipment technology (3.6 + 0.8), the confidence that the staff transmits to patients (3.6 ± 0.8), and the friendliness of the staff when attending patients (3.6 ± 0.8). The worst-rated aspects were the condition of the rooms (3.4 ± 0.8), the waiting time to be attended by a physician (3.3 ± 0.8), and the timeliness of internal consultations (3.3 ± 0.8). The overall population rated as satisfied in 97% of cases. CONCLUSIONS: A high rate of satisfaction was observed concerning both objective and subjective factors. However, the negative aspects of objective quality, such as reliability, should be addressed organizationally without implying economic investment in their resolution.


INTRODUCCIÓN: La prueba de Calidad en el Servicio de Hospital (SERVQHOS) evalúa la calidad y la satisfacción con la atención hospitalaria recibida. El objetivo de este estudio fue determinar la calidad y la satisfacción de los padres de familia en un hospital público pediátrico de tercer nivel en México. MÉTODOS: Se realizó un estudio transversal en el que se distribuyeron 425 encuestas anónimas durante el alta de los pacientes. El cuestionario evalúa la calidad individual (subjetiva) y de la organización (objetiva) del servicio: fiabilidad, tangibles, garantía, capacidad de respuesta y empatía, y satisfacción en una escala tipo Likert de 5 puntos, de 1 (mucho peor) a 5 (mucho mejor). RESULTADOS: Se recibieron 401 encuestas respondidas (tasa de respuesta del 94%). El 97% de los padres calificaron la satisfacción global como satisfechos o muy satisfechos. Los aspectos mejor calificados fueron la tecnología de los equipos médicos (3.6 ± 0.8), la confianza que el personal transmite al paciente (3.6 ± 0.8) y la amabilidad del personal en su trato al paciente (3.6 ± 0.8). Los aspectos peor valorados fueron el estado de las habitaciones (3.4 ± 0.8), el tiempo de espera para ser atendido por un médico (3.3 ± 0.8) y la puntualidad de las interconsultas (3.3 ± 0.8). CONCLUSIONES: Se observó un alto índice de satisfacción relacionado tanto con los factores objetivos como con los subjetivos. Sin embargo, los aspectos negativos de calidad objetiva, tales como la fiabilidad, deben ser atendidos por la organización sin que ello implique una inversión económica para su resolución.

ABSTRACT

Clinical criteria diagnose Parkinson's disease (PD), therefore, it is crucial to find biological elements that could support diagnosis or even act as prognostic tools of PD. The SNCA gene codifies a protein called α - synuclein; several studies associate genetic and biochemical factors of SNCA with PD, including transcript and plasmatic protein levels, however, contradictory evidence indicates inconclusive results. We aim to compare SNCA mRNA expression, plasmatic α-syn protein and rs356219 SNP between PD cases and a control group, and to identify a potential biomarker in Mexican mestizos', focusing on these three components determined in blood. We included 88 PD patients and 88 age-matched controls. We observed higher α-syn protein and decreased SNCA mRNA levels in PD subjects, compared to control group (p = 0.044 and p < 0.001, respectively). A statistically significant difference was found in allelic and genotypic frequencies of SNP rs356219 between PD patients and normal subjects (p = 0.006 and p = 0.023, respectively). Logistic regression analysis determined as optimal predictors of PD the GG genotype of SNP rs356219 (OR 2.49; p = 0.006) in a recessive model and α-syn protein (OR 1.057; p = 0.033). Furthermore, the G allele of SNP rs356219 was associated with higher plasmatic α-syn and mRNA levels in PD subjects. The receiver operating curves (ROC) distinguished PD from healthy controls with good sensitivity and specificity considering the plasmatic α-syn protein (AUC = 0.693, Sensitivity = 66.7 %, Specificity = 63.9 %) or a predictive probability of plasmatic α-syn protein and SNP rs356219 in a single model (AUC = 0.692, Sensitivity = 62.3 %, Specificity = 62.5 %). The performance of this classifier model in PD at early stage (n = 31) increase the discriminant power in both, plasmatic α-syn protein (AUC = 0.779, Sensitivity = 72.7 %, Specificity = 73.9 %) and predictive probability (AUC = 0.707, Sensitivity = 63.6 %, Specificity = 62.5 %). We propose that α-syn protein and SNP rs356219 together may work as a good signature of PD, and they can be suggested as a non-invasive biomarker of PD risk.

ABSTRACT

Duddingtonia flagrans is a biological alternative to the use of anthelmintic drugs in ruminants. This fungus must be ingested by the animal, pass through the cavities of the digestive tract and reach the feces where it develops traps that capture the nematodes. The severe conditions encountered in this process negatively affect the fungus, which is reflected in the low recovery rates compared to the amount administered. The aim of this study was to evaluate independently the in vitro effect of typical physical and chemical conditions of the gastrointestinal cavities of ruminants on the concentration, viability, and the in vitro nematode predatory ability of the chlamydospores of D. flagrans. The factors evaluated individually were pH (2, 6, and 8), temperature (28 ± 2°C and 39 ± 2°C), exposure to artificial saliva, and milling. The results showed that the concentration and viability of D. flagrans were not affected by the action of pH, temperature, milling, or exposure to artificial saliva. Regarding the in vitro nematode predatory ability, a reduction was observed after the milling process and the exposure for 24 h at different pH.

BACKGROUND/OBJECTIVE: Psoriatic arthritis (PsA) is a chronic inflammatory illness. Approximately, 15% of psoriasis patients have undiagnosed PsA. In Mexico, we found no related studies. Our objective was to investigate the clinical-epidemiological characteristics of PsA in psoriasis patients in western Mexico. METHODS: A cross-sectional study including Mexican patients with clinical and histopathological diagnosis of psoriasis. Physical examination, rheumatoid factor analysis and radiographies of axial and peripheral skeleton were performed. The prevalence of PsA using the CASPAR criteria, age, sex; clinical variants of PsA, psoriasis type and the Psoriasis Area and Severity Index (PASI), were assessed. Descriptive and inferential statistics were used. RESULTS: Of 90 patients with psoriasis, 48 met the criteria for PsA, with a prevalence of 53%, and average age of 50 ± 15 years. Predominating were, the female sex in 29 (60%), the axial variant of PsA in 24 (50%), and psoriasis plaques in 40 (83%). The average PASI was 12 ± 11. All cases were rheumatoid factor negative. These variables were not significantly different when comparing subjects with and without PsA, except for the female sex (60% vs. 7%; P < 0.001). CONCLUSIONS: Patients with psoriasis should intentionally be evaluated jointly Dermatologists and Rheumatologists searching joint involvement given the high prevalence of PsA previously undiagnosed.

ABSTRACT

Parkinson's disease (PD), a common neurodegenerative disorder, has a complex etiology where environmental and genetic factors intervene. While a number of genes and variants have been identified in recent decades as causative or protective agents of this condition, a limited number of studies have been conducted in mixed populations, such as Mexican Mestizos. The historical convergence of two founding groups and three ethnicities, and the increasing north-to-south gradient of Native American ancestry in Mexico resulted in a subpopulation structure with considerable genetic diversity. In this work, we investigate the influence of 21 known susceptibility variants for PD. Our case-control study, with a cohort of 311 Mexican Mestizo subjects, found a significant risk association for the variant rs1491942 in LRRK2. However, when stratification by ancestry was performed, a risk effect for MTHFR rs1801133 was observed only in the group with the highest percentage of European ancestry, and the PD risk effect for LRRK2 rs1491942 was significant in subjects with a higher ratio of Native American ancestry. Meta-analyses of these SNP revealed the effect of LRRK2 rs1491942 to be even more significant than previously described in populations of European descent. Although corroboration is necessary, our findings suggest that polymorphism rs1491942 may be useful as a risk marker of PD in Mexican Mestizos with greater Native American ancestry. The absence of associations with the remaining known risk factors is, in itself, a relevant finding and invites further research into the shared risk factors' role in the pathophysiological mechanisms of this neurodegenerative disorder.

ABSTRACT

PURPOSE: To explore a telehealth-based lifestyle therapeutics (THBC) program on weight loss (WL) and program satisfaction in an employer population. DESIGN: This study was a collaboration between inHealth Lifestyle Therapeutics and a large national employer group including 685 participants (296 women [64% obese] and 389 men [62% obese]). MEASURES: Percent WL and subjective rating (Perceived Program Value measured by a questionnaire) were assessed. INTERVENTION: Average number of visits was 3.1 ± 0.4; each visit ranged between 20 and 45 minutes. ANALYSIS: This study utilized a 2 × 2 block design using analysis of variance techniques based on sex (male and female) and initial body mass index (BMI) category (overweight and obese) tested at P ≤ .05. RESULTS: There was no statistical difference in %WL between by sex (F1,681 = 0.398, P = .528) nor an interaction between sex and BMI (F1,681 = 0.809, P = .369). There was a statistically significant difference in %WL from pre to post program across initial BMI category (F1,681 = 13.707, P ≤ .001) with obese participants losing an average of 1.1% (0.5%-1.6%) more than overweight participants (overweight 2.5% [2.1%-3.0%] vs obese 3.6% [3.2%-3.9%]). Obese participants were 1.15 (1.07-1.25) times more likely to lose weight compared to overweight participants. Analysis of variance power analysis indicated sufficient power on minimum factor combination n = 106 ( Effect Size = 0.282). CONCLUSION: Results support the efficacy THBC in supporting WL with no reported differences between men and women, while having a high perceived value for employee participants.
