ABSTRACT

Herpesvirus reactivates from a latent infection in older adults and critically ill and immunocompromised individuals. Herpes zoster ophthalmicus (HZO) is a latent infection that affects the fifth cranial nerve. It is an infrequent cause of increased intraocular pressure. We present the case of a 50-year-old man with a reactivation of latent varicella-zoster virus infection involving the ophthalmic branch of the fifth cranial nerve. The patient was initially managed as an outpatient with an antiviral, but his clinical evolution worsened and required urgent surgical decompression. Lateral canthotomy was performed with cantholysis of the inferior crus of the lateral canthal tendon. Only partial decompression was achieved, so cantholysis of the upper crus was performed with significant tissue tension release. The patient evolved well and was discharged after 6 days without symptoms for outpatient management.

Introducción: La ortoplastia fusiona conceptos y técnicas comunes a la traumatología y a la cirugía plástica, pero no siempre se logra la cooperación entre estas especialidades. Por ello el cirujano ortopédico debe aprender técnicas de cobertura cutánea que utilicen escasos recursos. Este trabajo presenta una alternativa quirúrgica para la cobertura de defectos complejos en la región esternoclavicular mediante un colgajo miocutáneo del pectoral mayor. Objetivo: Exponer el resultado funcional de la cobertura cutánea mediante la aplicación de un colgajo miocutáneo pectoral mayor contralateral. Presentación de caso: Paciente masculino de 21 años que presentó necrosis, exposición ósea y subluxación de la articulación esternoclavicular izquierda después de una cirugía vascular. Acudió a consulta de ortopedia para solucionar este defecto de partes blandas. Conclusiones: El colgajo miocutáneo del pectoral mayor es una técnica quirúrgica muy versátil y segura para la cobertura de defectos en la parte superior del tórax y el cuello con resultado funcional y estético satisfactorios.


Introduction: Orthoplasty merges concepts and techniques common to traumatology and plastic surgery; but achieving the cooperation between these specialties is not always easy. Therefore, the orthopedic surgeon must learn skin coverage techniques that use few resources. This paper presents a surgical alternative for the coverage of complex defects in the sternoclavicular region by means of a pectoralis major myocutaneous flap. Objective: To expose the functional result of skin coverage by applying a contralateral pectoral major myocutaneous flap. Case report: The case of a 21-year-old male patient who had necrosis, bone exposure, and subluxation of the left sternoclavicular joint after vascular surgery. He went to an orthopedic clinic to solve this soft tissue defect. Conclusions: The pectoralis major myocutaneous flap is a very versatile and safe surgical technique for covering defects in the upper part of the thorax and neck with satisfactory functional and aesthetic results.

Introducción: Existen numerosas técnicas para la cobertura cutánea de las pérdidas de sustancia del pulpejo digital. Se destacan la cicatrización dirigida, los injertos de piel total o parcial, los colgajos de avances en V-Y y rectangulares, los neurovascularizados de circulación directa o retrógrada, y los trasplantes microquirúrgicos parciales de artejo. Objetivo: Presentar un caso con pérdida de sustancia compleja del pulpejo digital y la aplicación de un colgajo neurovascularizado de circulación retrógrada. Presentación del caso: Paciente masculino de 35 años. Sufrió herida a nivel de la falange distal del dedo índice de la mano derecha y se le diagnosticó una amputación traumática en zona 2. La cicatrización dirigida fracasó y mantuvo exposición ósea, por tanto, se le realizó un colgajo neurovascularizado de circulación retrógrada. Conclusiones: El colgajo neurovascularizado de circulación retrógrada constituye una opción para las pérdidas complejas de sustancia del pulpejo digital porque aporta resultados satisfactorios.


Introduction: There are numerous techniques for skin coverage in the substance loss of the digital ball. These include targeted scarring, total or partial skin grafts, V-Y and rectangular breakthrough flaps, neurovascularized direct or retrograde circulation, and joint´s partial microsurgical transplants. Objective: To present a case with loss of complex substance of the digital ball and the application of a neurovascularized flap of retrograde circulation. Presentation of the case: 35-year-old male patient. He suffered an injury at the level of the distal phalanx of the index finger of the right hand and was diagnosed with a traumatic amputation in zone 2. The targeted scarring failed and maintained bone exposure, therefore, a neurovascularized flap of retrograde circulation was performed. Conclusions: The neurovascularized retrograde circulation flap is an option for complex losses of substance of the digital ball because it provides satisfactory results.

ABSTRACT

Little is known about brain aging or dementia in nonindustrialized environments that are similar to how humans lived throughout evolutionary history. This paper examines brain volume (BV) in middle and old age among two indigenous South American populations, the Tsimane and Moseten, whose lifestyles and environments diverge from those in high-income nations. With a sample of 1,165 individuals aged 40 to 94, we analyze population differences in cross-sectional rates of decline in BV with age. We also assess the relationships of BV with energy biomarkers and arterial disease and compare them against findings in industrialized contexts. The analyses test three hypotheses derived from an evolutionary model of brain health, which we call the embarrassment of riches (EOR). The model hypothesizes that food energy was positively associated with late life BV in the physically active, food-limited past, but excess body mass and adiposity are now associated with reduced BV in industrialized societies in middle and older ages. We find that the relationship of BV with both non-HDL cholesterol and body mass index is curvilinear, positive from the lowest values to 1.4 to 1.6 SDs above the mean, and negative from that value to the highest values. The more acculturated Moseten exhibit a steeper decrease in BV with age than Tsimane, but still shallower than US and European populations. Lastly, aortic arteriosclerosis is associated with lower BV. Complemented by findings from the United States and Europe, our results are consistent with the EOR model, with implications for interventions to improve brain health.

INTRODUCTION: We evaluated the prevalence of dementia and mild cognitive impairment (MCI) in indigenous Tsimane and Moseten, who lead a subsistence lifestyle. METHODS: Participants from population-based samples ≥ 60 years of age (n = 623) were assessed using adapted versions of the Modified Mini-Mental State Examination, informant interview, longitudinal cognitive testing and brain computed tomography (CT) scans. RESULTS: Tsimane exhibited five cases of dementia (among n = 435; crude prevalence = 1.2%, 95% confidence interval [CI]: 0.4, 2.7); Moseten exhibited one case (among n = 169; crude prevalence = 0.6%, 95% CI: 0.0, 3.2), all age ≥ 80 years. Age-standardized MCI prevalence was 7.7% (95% CI: 5.2, 10.3) in Tsimane and 9.8% (95% CI: 4.9, 14.6) in Moseten. Cognitive impairment was associated with visuospatial impairments, parkinsonian symptoms, and vascular calcification in the basal ganglia. DISCUSSION: The prevalence of dementia in this cohort is among the lowest in the world. Widespread intracranial medial arterial calcifications suggest a previously unrecognized, non-Alzheimer's disease (AD) dementia phenotype.

Introducción: El tumor de células gigantes de hueso es uno de los tumores menos frecuentes y su comportamiento local es agresivo. Objetivo: Presentar un caso afectado por un tumor de células gigantes tratado con resección en bloque y artrodesis de la articulación de la muñeca. Presentación del caso: Paciente masculino de 29 años de edad, con antecedentes relativos de salud, que presentaba desde hacía 5 meses aumento de volumen e impotencia funcional al realizar flexo extensión activa de la muñeca izquierda, con empeoramiento progresivo. Se diagnosticó tumor óseo de células gigantes con marcada actividad proliferativa estromal en el extremo distal del radio, se realizó resección en bloque y artrodesis del extremo distal del radio con márgenes oncológicos y transferencia del flexor radial corto al extensor común y del palmar menor al extensor y abductor del pulgar. Conclusiones: La resección en bloque y artrodesis es una de las opciones a tener en cuenta en la cirugía reconstructiva de la extremidad superior con un resultado satisfactorio(AU)


Introduction: Giant cell tumor of bone is one of the least frequent tumors and the local behavior is aggressive. Objective: To report a case with giant cell tumor treated with en bloc resection and arthrodesis of the wrist joint. Case report: A relative healthy 29-year-old male patient had had increased volume and functional impotence when performing active flexor extension of the left wrist for 5 months, with progressive worsening. A giant cell bone tumor with marked stromal proliferative activity was diagnosed in the distal end of the radius. En bloc resection and arthrodesis of the distal end of the radius were performed with oncological margins and the transfer of the flexor radialis brevis to the common extensor and the palmaris minor to the extensor and abductor pollicis. Conclusions: En bloc resection and arthrodesis is one of the options to consider in reconstructive surgery of the upper extremity with a satisfactory result(AU)

ABSTRACT

Background: Atrial fibrillation is the most common arrhythmia in post-industrialized populations. Older age, hypertension, obesity, chronic inflammation, and diabetes are significant atrial fibrillation risk factors, suggesting that modern urban environments may promote atrial fibrillation. Objective: Here we assess atrial fibrillation prevalence and incidence among tropical horticulturalists of the Bolivian Amazon with high levels of physical activity, a lean diet, and minimal coronary atherosclerosis, but also high infectious disease burden and associated inflammation. Methods: Between 2005-2019, 1314 Tsimane aged 40-94 years (52% female) and 534 Moseten Amerindians aged 40-89 years (50% female) underwent resting 12-lead electrocardiograms to assess atrial fibrillation prevalence. For atrial fibrillation incidence assessment, 1059 (81% of original sample) Tsimane and 310 Moseten (58%) underwent additional ECGs (mean time to follow up 7.0, 1.8 years, respectively). Findings: Only one (male) of 1314 Tsimane (0.076%) and one (male) of 534 Moseten (0.187%) demonstrated atrial fibrillation at baseline. There was one new (female) Tsimane case in 7395 risk years for the 1059 participants with >1 ECG (incidence rate = 0.14 per 1,000 risk years). No new cases were detected among Moseten, based on 542 risk years. Conclusion: Tsimane and Moseten show the lowest levels of atrial fibrillation ever reported, 1/20 to ~1/6 of rates in high-income countries. These findings provide additional evidence that a subsistence lifestyle with high levels of physical activity, and a diet low in processed carbohydrates and fat is cardioprotective, despite frequent infection-induced inflammation. Findings suggest that atrial fibrillation is a modifiable lifestyle disease rather than an inevitable feature of cardiovascular aging.

ABSTRACT

Indigenous communities worldwide share common features that make them especially vulnerable to the complications of and mortality from COVID-19. They also possess resilient attributes that can be leveraged to promote prevention efforts. How can indigenous communities best mitigate potential devastating effects of COVID-19? In Bolivia, where nearly half of all citizens claim indigenous origins, no specific guidelines have been outlined for indigenous communities inhabiting native communal territories. In this Public Health article, we describe collaborative efforts, as anthropologists, physicians, tribal leaders, and local officials, to develop and implement a multiphase COVID-19 prevention and containment plan focused on voluntary collective isolation and contact-tracing among Tsimane forager-horticulturalists in the Bolivian Amazon. Phase 1 involves education, outreach, and preparation, and phase 2 focuses on containment, patient management, and quarantine. Features of this plan might be exported and adapted to local circumstances elsewhere to prevent widespread mortality in indigenous communities.

Background: Carotid disease, measured as carotid intima-media thickness (CIMT) and carotid plaque (CP), is associated with major adverse cardiac and cerebrovascular events (MACCE) in people without the previous atherosclerotic disease; however, there are few published data in patients undergoing coronary angiography. The aim of the study is to determinate if the carotid disease is associated with MACCE after coronary angiography. Methods: A total of 390 consecutive patients underwent coronary angiography after exercise echocardiography and carotid ultrasonography between 2002 and 2013. MACCE was defined as stroke, myocardial infarction due to atherosclerosis progression or death due to a stroke or cardiac event. Results: Two patients were lost (0.5%). During a mean follow-up of 6.0 years (standard deviation of 2.9), 52 patients (13.4%) suffered MACCE. 1, 5, and 10 years, event-free survival was 96.4% (1.0), 88.7% (1.7), and 81.4% (2.8), respectively. Event rates at 10 years were higher in the CP group (23.2% vs. 10.2%, p = 0.013) and in the CIMT > 0.9 mm group (25.9% vs. 13.3%, p = 0.023). Multivariate analysis showed smoking habit (hazard ratio [HR] 2.51, 95% confidence interval [CI] 1.36-4.62, p = 0.003), glomerular filtration rate (HR 0.98, 95% CI 0.98-0.99), aortic stenosis (HR 2.99, 95% CI 1.24-7.21, p = 0.014), incomplete/no coronary revascularization (HR 1.97, 95% CI 1.06-3.67, p = 0.033), insulin treatment (HR 2.63, 95% CI 1.30-5.31, p = 0.006), and CP (HR 2.36, 95% CI 1.02-5.44, p = 0.044) as predictors of MACCE. Conclusions: CP is an independent predictor of MACCE in patients undergoing coronary angiography.


Introducción: La enfermedad carotídea, definida como grosor de íntima media (GIMC) y placa (PC), se asocia con eventos adversos cardiacos y cerebrovasculares (EACC) en sujetos sin aterosclerosis previa; sin embargo hay pocos datos en pacientes sometidos a coronariografía. El objetivo del estudio es determinar si la enfermedad carotídea se asocia a EACC en pacientes remitidos a coronariografía. Métodos: Entre 2002 y 2013 390 pacientes fueron sometidos a coronariografía tras ecocardiograma de esfuerzo y ecografía carotídea. Se definió EACC como accidente cerebrovascular, infarto de miocardio por progresión aterosclerótica o muerte por accidente cerebrovascular o causa cardiaca. ABSTRACT

Objetivo: Describir las complicaciones más frecuentes y la carga económica asociada con la varicela en el Perú. Material y métodos: Estudio multicéntrico de revisión de historias clínicas de pacientes de 1 a 14 años con diagnóstico de varicela entre 2011 y 2016. El uso de recursos de atención médica (URAM) asociados con la varicela, los costos unitarios y la pérdida de trabajo se utilizaron para estimar los costos directos e indirectos, presentados en USD ($). Los datos de costos y URAM se combinaron con estimaciones de carga de enfermedad para calcular el costo total anual de la varicela en el Perú. Resultados: Se incluyeron un total de 179 niños con varicela (101 ambulatorios, 78 hospitalizados). Entre los pacientes ambulatorios, el 5,9 % presentó una o más complicaciones, en comparación con 96,2 % de pacientes hospitalizados. El URAM incluyó el uso de medicamentos de venta libre (72,3 % frente a 89,7 % de pacientes ambulatorios y hospitalizados, respectivamente), medicamentos con receta (30,7 % frente a 94,9 %) y análisis y procedimientos (0,0 % frente a 80,8 %). Los costos directos e indirectos por caso ambulatorio fueron $36 y $62 respectivamente y por caso hospitalizado fueron $548 y $222. El costo anual total asociado con la varicela se estimó en $13 907 146. Conclusión: La varicela está asociada con complicaciones clínicas importantes y elevado URAM en Perú, lo que respalda la necesidad de implementación de un plan de vacunación universal. (AU)


Objective: The purpose of this study was to evaluate the clinical and economic burden associated with varicella in Peru. Healthcare resource utilization (HCRU) associated with varicella, unit costs, and work loss were used to estimate direct and indirect costs, presented in USD ($). The cost and HCRU data was combined with estimates of varicella disease burden to estimate the overall annual costs of management of varicella in Peru. Results: A total of 179 children with varicella (101 outpatients, 78 inpatients) were included. Among outpatients, 5.9% experienced ≥1 complication, compared with 96.2% of inpatients. HCRU estimates included use of over-the-counter (OTC) medications (72.3% vs. 89.7% of outpatient and inpatients, respectively), prescription medications (30.7% vs. 94.9%), tests/procedures (0.0% vs. 80.8%). Among outpatients, direct and indirect costs per case were $36 and $62, respectively; among inpatients, respective costs were $548 and $222. The total annual cost associated with varicella was estimated at $ 13 907 146. Conclusion: Varicella is associated with substantial clinical complications and high HCRU in Peru, supporting the need for implementation of a routine childhood varicella vaccination plan. (AU)

Abstract Introduction: Carotid disease, measured as carotid intima-media thickness (CIMT) and carotid plaque (CP), is associated with major adverse cardiac and cerebrovascular events (MACCE) in people without the previous atherosclerotic disease; however, there are few published data in patients undergoing coronary angiography. The aim of the study is to determinate if the carotid disease is associated with MACCE after coronary angiography. Methods: A total of 390 consecutive patients underwent coronary angiography after exercise echocardiography and carotid ultrasonography between 2002 and 2013. MACCE was defined as stroke, myocardial infarction due to atherosclerosis progression or death due to a stroke or cardiac event. Results: Two patients were lost (0.5%). During a mean follow-up of 6.0 years (standard deviation of 2.9), 52 patients (13.4%) suffered MACCE. 1, 5, and 10 years, event-free survival was 96.4% (1.0), 88.7% (1.7), and 81.4% (2.8), respectively. Event rates at 10 years were higher in the CP group (23.2% vs. 10.2%, p = 0.013) and in the CIMT > 0.9 mm group (25.9% vs. 13.3%, p = 0.023). Multivariate analysis showed smoking habit (hazard ratio [HR] 2.51, 95% confidence interval [CI] 1.36-4.62, p = 0.003), glomerular filtration rate (HR 0.98, 95% CI 0.98-0.99), aortic stenosis (HR 2.99, 95% CI 1.24-7.21, p = 0.014), incomplete/no coronary revascularization (HR 1.97, 95% CI 1.06-3.67, p = 0.033), insulin treatment (HR 2.63, 95% CI 1.30-5.31, p = 0.006), and CP (HR 2.36, 95% CI 1.02-5.44, p = 0.044) as predictors of MACCE. Conclusions: CP is an independent predictor of MACCE in patients undergoing coronary angiography.


Resumen La enfermedad carotídea, definida como grosor de íntima media (GIMC) y placa (PC), se asocia con eventos adversos cardiacos y cerebrovasculares (EACC) en sujetos sin aterosclerosis previa; sin embargo hay pocos datos en pacientes sometidos a coronariografía. El objetivo del estudio es determinar si la enfermedad carotídea se asocia a EACC en pacientes remitidos a coronariografía Métodos: Entre 2002 y 2013 390 pacientes fueron sometidos a coronariografía tras ecocardiograma de esfuerzo y ecografía carotídea. Se definió EACC como accidente cerebrovascular, infarto de miocardio por progresión aterosclerótica o muerte por accidente cerebrovascular o causa cardiaca. Resultados: Durante un seguimiento medio de 6 años (desviación estándar 2, 9) se registraron 2 pérdidas y 52 eventos (13,4%). La supervivencia media libre de eventos a uno, cinco y diez años fue 96.4% (1.0), 88.7% (1.7) y 81.4% (2.8). Hubo mayor número de eventos a 10 años en el grupo de PC (23.2% frente 10.2%, p = 0.013) y GIMC > 0.9 mm (25,9% frente 13.3%, p = 0.023). En el análisis multivariado los predictores de EACC fueron tabaquismo (hazard ratio [HR] 2.51, intervalo de confianza [IC] al 95% 1.36-4.62, p = 0.003), filtrado glomerular renal (HR 0.98 IC95% 0.98-0.99), estenosis aórtica (HR 2.99, IC 95% 1.24-7.21, p = 0.014), revascularización incompleta/no revascularización (HR 1.97, IC 95% 1.06-3.67, p = 0.033), tratamiento con insulina (HR 2.63, IC 95% 1.30-5.31, p = 0.006) y PC (HR 2.36, 95%CI 1.02-5.44, p = 0.044). Conclusiones: La PC es un predictor independiente de EACC en pacientes sometidos a coronariografía.

ABSTRACT

Genital warts (GW) are mucosal or skin lesions caused by sexual transmission of human papillomavirus (HPV). This study estimates the frequency of GW cases in physicians' clinics and physicians' usual practices of GW referral and diagnosis in Peru. Participants in this study were a convenience sample of 100 physicians in five specialties: primary care (17), gynecology (37), urology (10), dermatology (31), and infectious diseases (5). Physicians completed a survey and daily log of all patients aged 18-60 years seen over ten days in their offices located in Peru. The survey recorded GW referral patterns and the daily log recorded patient demographic information and GW diagnosis. Among 12,058 patients, the annual GW prevalence (95% confidence interval [CI]) was 2.28% (2.02-2.56) and cumulative incidence (95% CI) was 1.60% (1.38-1.84). Physicians reported that most GW patients were direct consult (73.5% of male and 67.9% of females) and physicians treated most GW patients themselves (73.4% of males and 76.7% of females). As reported, the most common reasons for referring were 'serious cases requiring more specialized treatment' (73.2% of male and 72.2% of female) and 'lack of resources to treat' (26.8% of male and 27.8% of female). We conclude that GW cases are commonly seen by physicians in Peru.

BACKGROUND: There is scarcity of data about the prevalence of non-AIDS defining comorbidities among stable HIV-infected patients in Peru. OBJECTIVE: We aimed to describe the most frequent cardiometabolic comorbidities found among ambulatory adults on ARV in Peru. METHODS: A review of records for patients attending regular visits at 5 clinics in Lima-Callao in January-February 2016 is presented. Patients were adults on ARV for >6 months, with no recent AIDS-defining condition. RESULTS: Three hundred and five medical charts were reviewed. Most patients were male (73.1%, n=223) with a mean age of 46.0 years. Mean time from HIV diagnosis was 9.41 yrs. and mean duration of ARV was 7.78 yrs. Most patients were on an NNRTI-based first line regimen (76.4%, n=233), and 12.1% (n=37) were on rescue regimens. Median CD4 count was 614.2 cells/µL and the proportion of patients with viral load <40 c/mL was 90.8% (n=277). Most frequent metabolic diagnoses were dyslipidemia (51.5%, n=157), obesity (11.1%, n=34), and diabetes mellitus (7.2%, n=22). Hypertension was diagnosed in 8.9% (n=27). Other diagnoses of cardiovascular disease were documented in 3.3% (n=10). Pharmacologic treatment was prescribed in 91.3% of patients with diabetes or hypertension, but in only 29.3% of patients with dyslipidemia. CONCLUSION: A high proportion of metabolic comorbidities was found, with dyslipidemia being the most frequent, followed by obesity and diabetes. In contrast, cardiovascular disease was documented less frequently. Medical treatment was started for only a third of dyslipidemia patients. HIV care policies need to consider proper management of chronic comorbidities to optimize long-term outcomes.

Introducción: las lesiones traumáticas de las manos encabezan la lista de accidentes laborales industriales. El tratamiento de estas lesiones tiene como objetivo restablecer la función de la punta del dedo, mejorando la calidad de vida del paciente. Objetivo: evaluar resultados de la aplicación de colgajos homodigitales neurovascularizados en las lesiones con pérdida compleja de sustancia del pulpejo digital de los dedos largos de la mano . Método: se realizó una investigación de tipo observacional, descriptiva de serie de casos en 33 pacientes con lesión del pulpejo digital de los dedos largos de la mano en el Hospital Clínico Quirúrgico Hermanos Ameijeiras en el período comprendido desde el 1ro. de octubre de 2013 hasta el 30 de enero de 2015; se aplicaron colgajos homodigitales neurovascularizados monopediculados de circulación directa (17 pacientes) o indirecta (16 pacientes). Resultados: el sexo más afectado fue el masculino, con una mayor frecuencia en los pacientes entre 36-41 y > 41 años edad. Se constataron 4 complicaciones (12 por ciento de la muestra en estudio). Con la aplicación de las técnicas quirúrgicas se logró 72,7 por ciento de resultados entre excelentes y buenos, así como una adecuada sensibilidad protectora y una cobertura cutánea satisfactoria. La satisfacción de los pacientes fue significativa (93,9 por ciento). Conclusiones: se demostró la eficacia de la aplicación de los colgajos homodigitales neurovascularizados en las pérdidas complejas de sustancia del pulpejo en los dedos largos de la mano(AU)


Introduction: traumatic injuries of the hand are in the top list of industrial working accidents. The treatment of these injuries has as goal to restore the function of fingertips, improving the patients' quality of life. Objective: evaluate results of the application of neurovascular homodigital flaps on cutaneous defects of digital fingertips of the hand large fingers. Method: an observational and descriptive investigation was conducted in a case chain of 33 patients at Hermanos Ameijeiras Hospital from October 2013 to January 2015. Neurovascular homodigital flaps were applied with direct circulation (17 patients) or indirect circulation (16 patients). Results: male sex was more affected, frequency higher was between 36 and 41 years old and over 41 years old. Four complications were confirmed representing 12 percent 72.7 percent obtained good and excellent results with the application of these surgical techniques receiving an appropriate protector of sensibility and satisfactory cutaneous covering. Application of these procedures was 93.9 percent satisfied. Conclusions: the application of neurovascular homodigital flaps was established on cutaneous defects of digital fingertip of the hand large fingers(AU)


Introduction: les lésions traumatiques de la main sont à l'avant-garde des accidents du travail. Le traitement de ces lésions est visé à restaurer la fonction de la pointe du doigt, améliorant ainsi la qualité de vie du patient. Objectif: l'objectif de cette étude est d'évaluer les résultats de l'utilisation du lambeau neurovasculaire homodigital dans le recouvrement des lésions avec une grande perte de substance pulpaire des doigts longs de la main. Méthode: une étude observationnelle et descriptive d'une série de 33 patients, souffrant des lésions pulpaires des doigts longs de la main, a été réalisée à l'hôpital Hermanos Ameijeiras pendant la période comprise entre le 1er octobre 2013 et le 30 janvier 2015; des lambeaux neurovasculaires homodigitaux et monopédiculaires à flux artériel antérograde ont été appliqués chez 17 patients et à flux artériel rétrograde chez 16 patients. Résultats: dans ce groupe, les hommes et les tranches d'âge de 36-41 et >41 ans ont été les plus souvent touchés. Quatre complications ont été constatées (dans 12 pourcent des cas). L'utilisation des techniques chirurgicales a obtenu d'excellents à bons résultats (72,7 pourcent), ainsi qu'une appropriée sensibilisation protectrice et une couverture cutanée satisfaisante. La satisfaction des patients a été significative (93,9 pourcent ). Conclusions: on a constaté l'efficacité du lambeau neurovasculaire homodigital pour le recouvrement des pertes de substance pulpaire des doigts longs de la main(AU)

ABSTRACT

Sexual risk-taking and reproductive morbidity are common among rapidly modernizing populations with little material wealth, limited schooling, minimal access to modern contraception and healthcare, and gendered inequalities in resource access that limit female autonomy in cohabiting relationships. Few studies have examined how modernization influences sexual risk-taking and reproductive health early in demographic transition. Tsimane are a natural fertility population of Bolivian forager-farmers; they are not urbanized, reside in small-scale villages, and lack public health infrastructure. We test whether modernization is associated with greater sexual risk-taking, report prevalence of gynecological morbidity (GM), and test whether modernization, sexual risk-taking and parity are associated with greater risk of GM. Data were collected from 2002-2010 using interviews, clinical exams, and laboratory analysis of cervical cells. We find opposing effects of modernization on both sexual risk-taking and risk of GM. Residential proximity to town and Spanish fluency are associated with greater likelihood of men's infidelity, and with number of lifetime sexual partners for men and women. However, for women, literacy is associated with delayed sexual debut after controlling for town proximity. Fifty-five percent of women present at least one clinical indicator of GM (n = 377); 48% present inflammation of cervical cells, and in 11% the inflammation results from sexually transmitted infection (trichomoniasis). Despite having easier access to modern healthcare, women residing near town experience greater likelihood of cervical inflammation and trichomoniasis relative to women in remote villages; women who are fluent in Spanish are also more likely to present trichomoniasis relative to women with moderate or no fluency. However, literate women experience lower likelihood of trichomoniasis. Parity has no effect on risk of GM. Our results suggest a net increase in risk of reproductive morbidity among rapidly modernizing, resource-stressed populations.

ABSTRACT

Since the 1970s, There have been studies of the venom of Latrodectus sp. spiders, in particular the latrotoxin (LTX) of Latrodectus mactans. Many of the studies were aimed at understanding the action of the venom on the muscular system. Now accepted that LTX is able to generate a calcium-permeable membrane pore and modulate the release of synaptic vesicles that activate a receptor and induce cellular changes. Interestingly, when work began with venom obtained from the Latrodectus sp present in Chile, it generated clinical indications similar to the bite of this spider in another country, with some differences in intensity. The purpose of the first studies was to understand the systemic mechanisms of this venom, and other active compounds were studied for biological interest. It was found that these molecules are capable of causing systemic effects such as changes in muscle contraction; of generating vascular relaxation and synaptic and cellular modulation; and of altering potassium conductance channels. Based on this evidence, we suggested biotechnological applications to characterize low molecular-weight compounds obtained from the Chilean Latrodectus venom and exploring the effects on the electrophysiology in oocytes and neurons, and the contraceptive effect on spermatozoa.


Desde los años 70, se han realizado estudios con el veneno de arañas Latrodectus sp, en particular la latrotoxina (LTX) de Latrodectus mactans. Muchos de estos estudios estuvieron enfocados a entender la acción del veneno sobre el sistema muscular. Hoy en día es aceptado que la LTX es capaz de generar un poro de membrana permeable a calcio y modular la liberación de vesículas sinápticas que activan un receptor e inducen cambios celulares. Interesantemente, cuando comenzamos a trabajar con el veneno obtenido de Latrodectus sp. presente en Chile, ésto generó indicaciones clínicas similares a la picadura de esta araña en otros países, con algunas diferencias en su intensidad. El propósito de estos primeros estudios fue entender los mecanismos sistémicos de este veneno y además otros compuestos activos fueron estudiados para interés biológico. Se ha encontrado que estas moléculas son capaces de causar efectos sistémicos así como cambios en la contracción muscular; generar relajación vascular y modulación sináptica y celular; y de alterar los canales de conductancia de potasio. Basados en estas evidencias, nosotros sugerimos usar aplicaciones biotecnológicas para caracterizar los compuestos de bajo peso molecular obtenidos del veneno de Latrodectus Chilena y explorar los efectos sobre la electrofisiología en ovocitos y neuronas, y el efecto anticonceptivo sobre los espermatozoides.

The morphology and size of spermatozoa make it difficult to study the functional properties of the plasma membrane, however, some studies have revealed the presence of a number of ion channels in this cell. We measured the calcium (Ca(++)) influx induced by depolarization of the plasma membrane and by venom isolated from the Chilean black widow spider (Latrodectus mactans), and functional changes in the presence of either high potassium or total venom. Our results indicate that the venom increased the Ca(++) influx, with an EC50 of 6.1 microg/mL and triggering the acrosome reaction in 43.26% of the cells. The application of potassium (10 mM K(+)) or total venom (10 microg/mL) did not affect the morphology or DNA stability of the sperm. The effects induced by high K(+) and venom suggest that direct blocking of K(+) currents alters the passive properties of the plasma membrane, leading to the entry of Ca(++). These results show the importance of functional changes induced by depolarizing the spermatozoa and by venom. This venom possesses one or more molecules that may be used as pharmacological tools for studies on spermatozoa and have potential applications in reproductive biotechnology.

The morphology and size of spermatozoa hinder the study of the functional properties of the spermatozoa plasma membrane. However, some studies have revealed the presence of a number of ion channels in this cell. We set out to measure the endogenous currents and to study the effect of the venom of the Chilean black widow spider (Latrodectus mactans). By patch-clamping bovine spermatozoa our results indicate the presence of an outwardly rectifying current, sensitive to changes in K(+) concentration (30-140 mM) and to tetraethylammonium (TEA, 10-100 mM). The application of the venom (7.5 microg/ml) blocks these K+ currents and then alters the passive properties of the plasma membrane. This leads to the entry of Ca(++), reflected by a change in basal fluorescent units (5+/-2 at 35+/-10 FAU). The Ca(++) influx follows a reduction in the membrane conductance (control 22+/-2; venom 10+/-1 pS), as calcium channels open in accord with voltage dependence.

ABSTRACT

Existen varios métodos que solucionan la tenosinovitis estenosante de los dedos largos de la mano, comúnmente conocida como dedo en resorte. Este estudio muestra los resultados de la aplicación de la vaginotomía percutánea para tratar esta afección. Se evaluó según el método de Strickland, 1 437 pacientes, que presentaron 2 192 dedos en resorte. En cuanto al resultado de la cirugía, se analizó el tiempo quirúrgico, la duración del dolor postoperatorio, la recuperación de la función motora, el tiempo de reincorporación al trabajo y las complicaciones. Se obtuvo un 98 por ciento de resultados satisfactorios. Los tiempos promedios de las variables descritas fueron significativamente cortos y no se mostraron complicaciones serias. La vaginotomía percutánea solucionó satisfactoriamente la morbilidad que produce el dedo en resorte. Es un proceder seminvasivo, fácil, rápido, bien tolerado y de bajo costo que aporta una recuperación funcional óptima y un retorno inmediato a las actividades cotidianas
