ABSTRACT

The diabetic nephropathy is one of the most prevalent microvascular complications with type 2 diabetes mellitus. The most accurate and widely used marker for diabetic nephropathy is microalbuminuria and it is also regarded as conventional method. However, it is not a sensitive or specific nephropathy biomarker. Therefore, it is of interest to evaluate the role of podocalyxin to predict early onset of nephropathy in patients with type 2 diabetes mellitus. This cross - sectional study is conducted on 150 subjects. Among these 150 T2DM patients (Group 2: T2DM with normoalbuminuria and Group 3: T2DM with microalbuminuria) and 50 were age, gender and BMI matched healthy controls (Group 1). The biochemical and experimental parameters was analyzed. T2DM patients have higher levels of urine podocalyxin. This level was significantly elevated in patients with T2DM with microalbuminuria than normoalbuminuria. Urinary podocalyxin levels and HbA1c were found to be positively correlated. Thus, urinary podocalyxin is useful as early predictable marker for nephropathy in patients with type 2 diabetes mellitus.

ABSTRACT

Diabetic nephropathy is a one of the risk factor for end stage renal disease in patients with type 2 diabetes mellitus. Urinary micr oalbumin currently marker using for diagnosis of nephropathy in type 2 diabetes mellitus due to its larger variability and less specificity. Urinary nephrin is podocyte protein can be act as a early predictable and prognostic marker than micro albumin for nephropathy in type 2 diabetes mellitus. This case-control prospective observational study included 60 type 2 diabetes mellitus and 30 controls after applying criteria. Basic biochemical, clinical and experimental data were measured and recorded. There was a significantly elevated level of urinary nephrin in type 2 diabetes mellitus patients when compared to controls. The urinary nephrin significantly elevated in normo albuminuria group only when compared to urinary ACR and it is positive association with kidney damage. This study concluded significantly elevated levels of urinary nephrin might be used for early diagnosis and prognostic marker for nephropathy in type 2 diabetes mellitus.

ABSTRACT

Type 2 Diabetes Mellitus (T2DM) patients with coronary artery disease (CAD) coupled with hypertension (HTN) are leading cause of morbidity and mortality. Association of Interleukin 6 and uric acid levels can be used for early detection of CAD in patients with T2DM. This cross sectional study included 120 study participants, were sub grouped into three Group 1: controls (n=30), Group 2: CAD, HTN without T2DM (n=30) and Group 3: CAD, HTN with T2DM (n=30). Basic biochemical, clinical and experimental parameters were analysed and recorded for all the study subjects. The statistical analysis was done by using the SPSS 20.0. Individuals with CAD, HTN with T2DM had higher BMI, weight, SBP, and DBP levels than patients CAD, HTN without diabetes and controls. In CAD, HTN with T2DM study participants has been found to have higher serum levels of inflammatory cytokines like IL-6 and uric acids when compared to CAD, HTN without T2DM and controls. IL6 was significantly positive association with uric acid and negative association with age and HDL. These findings demonstrated a statistically significant positive connection between IL-6, and uric acid concentrations. The identified elevated levels of inflammatory cytokines and uric acids in type 2 diabetic and hypertensive CAD patients may aid in the identification of patients at higher risk.
