ABSTRACT

Avulsion fractures of the calcaneal tuberosity are rare injuries. Several surgical treatment options have been described. The size of the calcaneal fragment is the limiting factor in choosing the method for restoration. Finding the right type of fixation modality remains challenging in this rare kind of injury. In the case presented one Mitek Super Anchor was used to reattach the small fragment of the tuberosity to the calcaneus. Stable fixation with bony reunion and excellent functional outcome were achieved by this technique within 10 weeks. Therefore, we recommend the use of an anchor system for the treatment of small fragment calcaneal avulsion fractures.

ABSTRACT

Two cases of fractures of the humeral neck in association with an axillary artery injury are reported. Even now, there are still only 14 well-documented cases in the literature. Trauma, pattern of injuries and treatment are analyzed. The treatment is focused on preservation of hand function, which is more important than shoulder motion. Neurovascular damage causes an impairment of hand function. The priorities of treatment (reperfusion versus osteosynthesis) are dictated by the degree of ischemia. Long-term disability is determined by brachial plexus injury.

ABSTRACT

We report a case of lethal air embolism during diagnostic arthroscopy using air to distend an acutely injured knee. Air had escaped from the joint through an intra-articular fracture and entered the venous system. During arthroscopy, pressure within the joint may be 5 to 10 times higher than venous pressure, so any medium may escape. We advise that the use of air, especially in freshly injured joints, should be abandoned in favour of saline or carbon dioxide.

ABSTRACT

We analysed the cases of renal carcinoma undergoing operation or autopsy from 1970 to 1978 in respect of their metastasizing to the adrenals. Of the 31 cases undergoing autopsy 10 adrenal metastases were found mostly as a result of a widespread metastasis. Of the operated 74 patients 37 underwent nephrectomy alone and 37 nephrectomy together with prophylactic adrenalectomy. The histologic evaluation of the 37 removed adrenals revealed metastases only in 4 cases. Only one of them proved to be a single metastasis, the other occurring together with other metastatic lesions. The value of the adrenalectomy will be discussed.
