ABSTRACT

Background: Teledermatoscopy accuracy has been examined in experimental settings and is recommended for primary care despite lacking real-world implementation evidence. A teledermatoscopy service has been provided in Estonia since 2013, where lesions are evaluated based on the patient's or general practitioner's suggestion. Objective: The management plan and diagnostic accuracy of a real-world store-and-forward teledermatoscopy service for melanoma diagnosis were evaluated. Methods: A retrospective study analyzed 4748 cases from 3403 patients using the service between October 16, 2017 and August 30, 2019 by matching country-wide databases. Management plan accuracy was calculated as the percentage of melanoma found that was managed correctly. Diagnostic accuracy parameters were sensitivity, specificity, and positive and negative predictive values. Results: Management plan accuracy for melanoma detection was 95.5% (95% CI, 77.2-99.9). Diagnostic accuracy showed a sensitivity of 90.48% (95% CI, 69.62-98.83) and a specificity of 92.57% (95% CI, 91.79-93.31). Limitations: Matching the lesions was limited to SNOMED CT location standard precision. Diagnostic accuracy was calculated based on a combination of diagnosis and management plan data. Conclusion: Teledermatoscopy for detecting and managing melanoma in real-world clinical practice displays results comparable with those in experimental setting studies.

AIM: To validate mobile-based funduscope for diabetic retinopathy screening in Estonia. METHODS: Quality validation comparison of HEINE® iC2 funduscope and Zeiss Visucam camera with image scoring and diagnostic test accuracy measurement by sensitivity and specificity. Study took place from January 2020 until March 2020 in East-Tallinn Central Hospital's eye clinic. RESULTS: Based on 90 patients, the Zeiss Visucam showed 35.6% DR prevalence while iC2 had 18.9% for images and 17.8% for videos. The average Likert score was 4.7 for Zeiss Visucam and 2.4 for both iC2 images and iC2 videos. The sensitivity of iC2 images was 72.7% (95%CI 49.6-88.4) for grader 1 and 61.9% (95%CI 38.7-81.0) for grader 2, iC2 video sensitivity was 57.1% (95%CI 37.4-75.0) and 65.4% (95%CI 44.4-82.1), respectively. The grader-based specificity for iC2 images was 96.7% (95%CI 80.9-99.8) and 93.5% (95%CI 77.2-98.9). iC2 videos had a 100% (95%CI 91.7-1.0; 92.0-1.0) specificity by both graders. Cohen's kappa agreement was 0.82 and 0.96 for images and videos. CONCLUSION: Mobile-based funduscope iC2 is not valid for DR screening with non-dilated pupils and thus not suitable for clinics that do not have experienced specialist present. Moreover, the screening specialist needs to be experienced fundus photographer with extra multiple day training for funduscope use. As main resolution, mobile-based funduscope was not validated for DR screening in Estonia based on pre-set study criteria. Additional research and development of funduscope algorithm for image stripping from videos is needed for validation as iC2 benefits do not offset the gold standard at the moment.

ABSTRACT

BACKGROUND: Patient Reported Outcome Measures (PROMs) are questionnaires that could be used in palliative care (PC) to evaluate patient well-being and monitor their care. PROMs enable a focus on what is important to patients by putting the patient at the center of care. Adapting an existing PROM requires considering cultural differences, general usability and understandability of translated terms. AIM: To translate and culturally adapt both the patient and staff three and seven day versions of the Integrated Palliative care Outcome Scale (IPOS) into Estonian. The IPOS consist of 10 questions (staff versions 9 questions) and 17 close ended items. The sub aim is to describe the differences and discrepancies found during the adaptation process and compare the results with previous research. METHODS: The translation and adaptation process of IPOS was conducted using recommended guidelines consisting of six phases and included cognitive interviews with patients (n = 11) and palliative care multidisciplinary team members (n = 8). The study was conducted in two major Estonian hospitals. RESULTS: The Estonian IPOS demonstrated face and content validity, acceptance by patients and staff. As a result of expert group review and cognitive interviews with patients and staff, 9 semantic changes were implemented. CONCLUSIONS: Patient and staff versions of the IPOS with a recall of 3 or 7 days were translated and culturally adapted for Estonia. The Estonian IPOS four versions are ready for use in Estonia.

ABSTRACT

PURPOSE: Innovative technologies to deliver health care across borders have attracted both evangelists and sceptics. Our aim was to systematically identify factors that hinder or support implementation of cross-border telemedicine services worldwide in the last two decades. METHODS: Two reviewers independently searched ten databases including MEDLINE and EMBASE, in June 2011 including citations from 1990 onwards when at least an abstract was available in English. We also searched ELDIS and INTUTE databases and Internet search engines to identify grey literature. We included studies which (a) described the use of telemedicine to deliver cross-border healthcare and, or (b) described the factors that hinder or support implementation of cross-border telemedicine services. All study designs were included. Two reviewers independently assessed titles and abstracts of articles identified. Papers were allocated to one of four reviewers who extracted relevant data and validated it. We took a qualitative approach to the analysis, conducting a narrative synthesis of the evidence. RESULTS: 6026 records were identified of which 5806 were excluded following screening of titles and abstracts. We assessed 227 full text articles, excluding 133 because they were fatally flawed or did not meet the inclusion criteria, producing a final sample of 94. They involved 76 countries worldwide, most involving collaborations between high and low or middle income countries. Most described services delivering a combination of types of telemedicine but specialties most represented were telepathology, telesurgery, Emergency and trauma telemedicine and teleradiology. Most link health professionals, with only a few linking professionals directly to patients. A main driver for the development of cross-border telemedicine is the need to improve access to specialist services in low and middle income countries and in underserved rural areas in high income countries. Factors that hinder or support implementation clustered into four main themes: (1) legal factors; (2) sustainability factors; (3) cultural factors; and (4) contextual factors. CONCLUSIONS: National telemedicine programmes may build infrastructure and change mindsets, laying the foundations for successful engagement in cross-border services. Regional networks can also help with sharing of expertise and innovative ways of overcoming barriers to the implementation of services. Strong team leadership, training, flexible and locally responsive services delivered at low cost, using simple technologies, and within a clear legal and regulatory framework, are all important factors for the successful implementation of cross-border telemedicine services.
