ABSTRACT

We evaluated major cytogenetic abnormalities associated with childhood acute lymphoblastic leukemia (ALL) through both fluorescent in situ hybridization and conventional chromosomal analysis for 132 ALL patients diagnosed at St Mary's Hospital in Korea. Chromosome abnormalities have been detected in 92% of patients. Eighteen (14%) patients showed numerical abnormalities only, 50 (38%) patients showed structural abnormalities only, and 53 (40%) patients showed both. The simultaneous trisomies 4, 10 and 17 were observed in 23 (17%) patients. Of the patients with abnormal karyotypes, recurrent structural abnormalities were determined in 103 (78%) cases. t(12;21)(q13;q22) was found in 29 (22%) out of 132 patients, 9p abnormalities in 13 (10%) patients, t(1;19)(q23;p13.3) in 11 (8%) patients, t(9;22)(q34;q11.2) in 11 (8%) patients, and 11q23 abnormalities in 7 (5%) patients. Interestingly, we identified five uncommon translocations such as t(5;12) (q33;p13), t(14;19)(q32;q13.1), t(12;16)(p13;q13), der(1)t(1;12)(p32;p13), and t(5;15)(p15;q11.2). Our study pool is representative of pediatric ALL patients in Korea as it consists of about 20% of patients diagnosed annually in Korea. We believe that the data provided will aid in comparative studies of the treatment outcomes, as well as the type and incidence of chromosomal abnormalities associated with childhood ALL in various Asian nations and Western countries.

ABSTRACT

PURPOSE: The purpose of the paper is to discover the patterns and processes of decision-making in clinical nursing practice. METHODS: A set of think-aloud data from five critical care nurses during 40 to 50 minutes of caregiving in intensive care units were obtained and analyzed by applying the procedures recommended by Ericsson and Simon for protocol analysis. RESULTS: Four thinking processes before acting were identified to constitute various sorts of thoughts in which the nurses were engaged during patient care: reviewing, validation, consideration, rationalization, and action. In addition, three patterns of sequential streaming of thinking (short, intermediate, long) were identified to reveal various ways the nurses dealt with clinical situations involving nursing tasks and responsibilities. CONCLUSION: This study specifies the initial categories of thoughts for each of the processes and various patterns with which these processes are sequentially combined, providing insights into the ways nurses think about problems and address their concerns. The findings suggest that the thinking in clinical practice involves more than focused decision-making and reasoning, and needs to be examined from a broader perspective.

ABSTRACT

Ciprofloxacin (CPFX) and roxithromycin (RXM) induced apoptosis of activated Jurkat T cells in vitro. CPFX showed concentration-dependent acceleration of apoptosis of activated Jurkat T cells by enhancing the expression of FasL and activities of caspase-3 and -8. RXM accelerated cell death, enhanced expression of FasL and caspase-3 but not caspase-8, and did not show the concentration dependency.

ABSTRACT

PURPOSE: The purpose of this study was to compare the nursing activities delineated by interview of nurses with those on nursing notes. METHOD: The participants of interview were 18 nurses working in medical and surgical units of a large hospital in Seoul. Each nurse was asked to choose one patient who demand most nursing care among her patients. The nurse was then interviewed to describe what her nursing activities for the patient was that day. The audio-taped interview was transcribed and the content was analyzed by researchers. Nursing notes of each nurses' patients were copied and the content analyzed by researchers. Finally, themes from the interview data and those from nursing notes were compared. RESULT: Activities related to emotional or psychological nursing, education for patient and families, and problem solving related to treatment or nursing procedure were most often omitted in nursing notes. Most of the documentation in nursing notes were related to physical condition of patients or physician's orders. Nurses described that they will do better recording if they were given less patient care responsibility, had better nursing knowledge, had better recording system, and received more training on nursing record. CONCLUSION: Nursing notes did not reflect nursing activities properly. Few independent nursing roles were documented in the nursing notes. Development of nursing education program and nursing record system is needed for improvement of nursing record.
