ABSTRACT

A review of our records between 1970 and 1986 identified 22 patients with transitional cell carcinoma of the bladder who were less than 31 years old. Of these patients 7 were less than 20 years old (group 1) and 15 were 20 to 30 years old (group 2). The tumors usually were of low grade and low stage. Patients in group 1 had no recurrences, whereas 6 patients (40 per cent) in group 2 had recurrences. Upstaging occurred in 2 patients with tumor recurrence. It would appear that while transitional cell carcinoma of the bladder in patients less than 20 years old has a more favorable prognosis, in patients 20 to 30 years old the prognosis is poorer and similar to that observed in older patients.

ABSTRACT

Continence following urinary diversion depends on factors related to the reservoir and outlet. The reservoir should possess good compliance and no or atmost low pressured phasic contractions. The outlet should provide adequate outflow resistance to allow expulsion of urine under voluntary control and at convenient intervals. Overcoming high intra-reservoir pressures by producing high outlet resistance may endanger the upper urinary tracts. There is evidence that a low pressure reservoir may be constructed from any part of the bowel as long as detubularization of the segment is performed. All types of intussuscepted valves possess an inherent tendency to dessuscept despite various techniques which are employed to stabilize them. The causes are discussed and a recently described technique to overcome this problem is reviewed.

ABSTRACT

Ninety-five adult patients with primary vesicoureteral reflux were reviewed concerning the presenting symptoms, radiological findings, associated renal damage, treatment and results of follow-up. We found a relatively high incidence of reflux in adult males. Although urinary tract infection and flank pain were the most common presentation, others had a wide variety of complaints not directly attributable to reflux. Renal anomalies were present in 37 patients. We found surgical correction of reflux useful in preventing infection, and recommend it especially in young women who may become pregnant.

ABSTRACT

Solcoderm, a new topical agent, was used in the treatment of genital and anal cutaneous condylomata in 11 patients. This is a relatively inexpensive form of treatment, which may be used in any office practice setting with no need for anesthesia and very few side effects. The success rate of 100% warrants its further use in the treatment of this disease entity.

ABSTRACT

The records of 105 patients who had undergone unilateral reimplantation for ipsilateral reflux were reviewed. Patient age, grade of preoperative reflux, completely duplicated collecting systems, preoperative findings of contralateral dilatation of the lower ureter or radiographic evidence of pyelonephritis had no predictive value in regard to the appearance of postoperative contralateral reflux. Of the 105 patients 16 had postoperative contralateral reflux: neurogenic problems were found later in 3, reflux resolved spontaneously within 18 months postoperatively in 9, 1 remains in followup and 3 have undergone repeat operations, for a rectified rate of treatment failure of 3 of 102 (2.9 per cent).
