ABSTRACT

OBJECTIVE: To assess possible role of psychosomatic aspects and antipsychotic medical treatment in ethio-pathogenesis of endometrial cancer. DESIGN: Prospective study and revue of literature. SETTING: Department of Obstetrics and Gynecology, Faculty of Medicine and Dentistry, University Hospital, Olomouc; Department of Health Care Science, Faculty of Humanities, Tomas Bata University in Zlín. METHODS: A case-control study was conducted on 40 patients who underwent surgical treatment for histopathologicly confirmed diagnosis of endometrial cancer. Patients on admition were examined by means of the standardised Minnesota personality inventory MMPI-2. Antipsychotic medical treatment in patients history was recorded. RESULTS: Our study confirmed signinicantly increas in the scale of depression, anxiety, psychastenia, social introversion, low self-respect, A-type behaviour and hostility. On the other hand there were no changes in the scale of hysteria and lie. A possible role of antipsychotics is discussed. ABSTRACT

OBJECTIVE: To present experience with surgical treatment of various gynaecological diseases in patients belonging to the Church of Jehova´s Witness. DESIGNE: Retrospective study. SETTING: Department of Obstetrics and Gynaecology, Palacky University Olomouc, Institute of Health Care Studies, Faculty of Humanity Studies, Thomas Bata University, Zlín. METHODS AND RESULTS: The study included 24 patients belonging to the Church of Jehova´s Witness who reject blood tranfusion. The operations in these patients were performed for malignant as well as nonmalignant gynaecological disorders which could not be treated by conservative therapeutic procedures. The operation records were analysed and evaluated according to a set of criteria including the type of surgical procedure, estimated amount of blood loss, postoperative complications and the outcome of surgical treatment. CONCLUSION: Jehovas´s Witnesses represent a risk group of patients considering their refusal of blood transfusion. The indication for the operation and its performing is responsible decision which always inherits a certain degrese of risk. On the other hand, when adhering to the principles of bloodless surgery, the therapeutic results are very good and in the properly indicated cases the scope of risk is acceptable.

ABSTRACT

OBJECTIVE: Evaluation of selected scales of the Minnesota Multiphasis Personality Inventory-2 (MMPI-2) within the frame of psychosomatic aspects of gynecological oncology. DESIGN: Assessment of actual emotional state and its comparison with the general personality traits of patients before a surgery for gynecologic malignancy. SETTING: Department of Obstetrics and Gynecology, University Hospital Medical Faculty PU, Olomouc. METHODS: The study group included 40 patients before the operation and histological confirmation of the diagnosis of oncological disease in whom the indicators of emotional status and personality structure was assessed. The standardized MMPI-2 was used for differenciation of clinical depression, anxiety and psychastenic traits. The ability of patients to participate in the study and adequately express themselves about their emotional status was examined to estimate the validity of administered data. RESULTS: Significantly higher scores in the selected scales of the standardized MMMPI-2 were found in our study group compared to general population. The most marked differences were observed in anxiety and clinical manifestation of depresive symptoms. CONCLUSION: Based on the configuration of the personality scales it is therefore possible to accept the research hypothesis that patients in this group are predisponised to anxiousness as a personality trait even before the final confirmation of their suspected oncological diagnosis.

ABSTRACT

Beta 2-microglobulin (beta 2-MG) is used as a tumour marker in some malignant diseases. The authors assessed the concentration of beta 2-MG in serum of 30 patients with malignant ovarian tumours and in 29 patients with other benign gynaecological diseases, in particular benign ovarian tumours. Although raised values of beta 2-MG were found in ovarian carcinomas, as compared with the control group of non-malignant gynaecological diseases, a statistically significant increase was recorded only in carcinomas grade III and IV.
