ABSTRACT

Parenteral prostanoids are effective for improving outcomes in patients with pulmonary arterial hypertension. However, subcutaneous or intravenous delivery via an external pump places a significant burden on patients. Consequently, the Implantable System for Remodulin© (treprostinil) was developed and is associated with a low rate of complications (United Therapeutics (Research Triangle Park, NC) in collaboration with Medtronic, Inc. (Mounds View, MN)). The current real-world experience study evaluated pulmonary arterial hypertension patients' perceptions of their quality of life, ability to perform activities of daily living, perceptions on the benefits and risks of the implantable system, and their social interactions before and after receiving the implantable system. Pulmonary arterial hypertension patients who had been transitioned from an external infusion pump to the implantable system completed a mix of quantitative and qualitative questions administered online over the course of a six-day period. A total of 20 patients completed the study. All patients reported that their quality of life, confidence out in public, and ability to travel long distances had improved. Over 90% of patients reported that their overall level of independence was better since receiving the implantable system, and most patients indicated that their ability to independently perform specific activities of daily living had improved. Responses to the qualitative questions suggested that the implantable system saved time, improved interpersonal relationships, and increased freedom. Results from this real-world patient experience study suggest this novel delivery system provides improvements in factors that are of substantial importance to patients.

ABSTRACT

PURPOSE: Up to 25% of chronic hepatitis B (CHB) patients eventually develop hepatocellular carcinoma (HCC), a disease with poor prognosis unless detected early. This study identifies a blood-based RNA biomarker panel for early HCC detection in CHB. MATERIALS AND METHODS: A genome-wide RNA expression study was performed using RNA extracted from blood samples from Malaysian patients (matched HCC, CHB, controls). Genes differentiating HCC from controls were selected for further testing using quantitative real-time polymerase chain reaction. Finally, a 6-gene biomarker panel was identified and characterized using a training set (cohort I = 126), and tested against 2 test sets (cohort II = 222; cohort III = 174). The total number of samples used for each group is: HCC + CHB = 143, CHB = 211, control = 168. RESULTS: Our gene panel displays a consistent trend distinguishing HCC from controls in our test sets, with an area under receiver-operating characteristic curve of 0.9 in cohort III. Our independent test set (cohort III) showed that the gene panel had a sensitivity of 70% with a specificity of 92%. The biomarker profile for HCC was consistently detected in a small subgroup of CHB patients, thus potentially predicting early, preclinical cases of cancer that should be screened more intensively. CONCLUSION: The biomarkers identified in this study can be used as the basis of a blood-based test for the detection of early HCC in CHB.

ABSTRACT

PURPOSE: To report the efficacy and safety of percutaneous ultrasonography-guided endoscopic lavage in the management of perinephric abscesses. PATIENTS AND METHODS: This is a retrospective review of patients in a single center who had perinephric abscesses and were treated by percutaneous ultrasonography-guided endoscopic lavage during the period of March 2001 to March 2013. Information including the demographics, comorbid medical conditions, presenting symptoms, size of abscesses, operation information, and postoperative information were retrieved for review. RESULTS: There were 37 patients (40 operations) identified during the study period. The mean age was 56.8 years (21-91 years). Eighteen (48.9%) patients presented with loin pain (48.6%) and/or fever (18, 48.6%), and the mean diameter of the abscesses was 10.8 cm (5-22 cm). Thirty-one (77.5%) procedures were performed under general anesthesia. The mean operative time was 49.4 minutes (15-140 min). Thirty-one (77.5%) patients had their percutaneous track dilated to 32F. Only two of 37 (5.4%) patients needed repeated drainage. The average postoperative drainage time was 8.35 days (3-21 days). The median postoperative hospital stay for our patients was 10 days (4-101 days). There was no mortality related to the abscess in this cohort. CONCLUSION: Percutaneous Ultrasonography-guided endoscopic lavage was shown to be an effective and safe approach for patients with perinephric abscesses.
