ABSTRACT

The case at hand involves a 25-year-old woman suffering from a gradual enlargement of a gluteal mass over a five-year period. This rare medical condition is classified as superficial CD34-positive fibroblastic tumor (SCPFT), an intermediate form of mesenchymal malignancy, originally identified in medical literature in 2014. Initial physical examination revealed a 10 x 10 cm lump in the right upper gluteal region. Magnetic resonance imaging demonstrated a well-circumscribed lesion within the right gluteus maximus muscle. The patient had the lump removed surgically. Examination under a microscope revealed tumor cells with extensive pleomorphism, high cytokeratin (CK) and CD34 positivity, and a low Ki67 index, all of which are consistent with SCPFT. Postoperatively, the patient showed marked improvement, and ongoing monitoring was initiated due to the potential for local recurrence. This case reinforces the importance of considering SCPFT in the differential diagnosis of soft tissue masses, emphasizing the role of immunohistochemical staining in reaching a correct diagnosis. Given the rarity of SCPFT, more studies are necessary to refine our understanding and treatment approaches.

ABSTRACT

The most common malignant tumors of the uterus are endometrioid adenocarcinomas (EA). Their prognosis depends on the qualitative characteristics of the neoplastic cells and their stroma. The neovascularization of EA tissues and level of microvascular density (MVD) influence tumor progression. Our study aims to establish the relationship between MVD in EA tissue and the histological and immunohistochemical features of tumors. Materials and methods: The authors studied 30 cases of endometrial ЕА and compared their histological and immunohistochemical characteristics with the MVD of tumor tissues. Results: Our study indicated that MVD in EA tissue depends on the grade of the tumors and their FIGO stage. Increased MVD was correlated with a depression of E-cadherin and PR expression and enhanced expression of VEGF and Ki-67. MVD enhancement during VEGF overexpression is a manifestation of the functional activity of these proteins. The increase in MVD was accompanied by more frequent metastasis of the EA to the lymph nodes. Conclusion: EA progression is accompanied by qualitative and quantitative variations of parenchymal and stromal patterns of tumors. Dedifferentiation of EA leads to overexpression of VEGF, which becomes diffuse in tumors cells, resulting in an increase of adenocarcinomas' MVD and their metastatic potential. Correlations between histological and immunohistochemical features of EAs indicate the synchronicity of the occurrence and progression of morphological and immunological anaplasia, which can be used in predicting the course of the disease.

ABSTRACT

The emergence of genome-wide association studies (GWAS) has identified genetic traits and polymorphisms that are associated with the progression of nonalcoholic fatty liver disease (NAFLD). Phospholipase domain-containing 3 and transmembrane 6 superfamily member 2 are genes commonly associated with NAFLD phenotypes. However, there are fewer studies and replicability in lesser-known genes such as LYPLAL1 and glucokinase regulator (GCKR). With the advent of artificial intelligence (AI) in clinical genetics, studies have utilized AI algorithms to identify phenotypes through electronic health records and utilize convolution neural networks to improve the accuracy of variant identification, predict the deleterious effects of variants, and conduct phenotype-to-genotype mapping. Natural language processing (NLP) and machine-learning (ML) algorithms are popular tools in GWAS studies and connect electronic health record phenotypes to genetic diagnoses using a combination of international classification disease (ICD)-based approaches. However, there are still limitations to machine-learning - and NLP-based models, such as the lack of replicability in larger cohorts and underpowered sample sizes, which prevent the accurate prediction of genetic variants that may increase the risk of NAFLD and its progression to advanced-stage liver fibrosis. This may be largely due to the lack of understanding of the clinical consequence in the majority of pathogenic variants. Though the concept of evolution-based AI models and evolutionary algorithms is relatively new, combining current international classification disease -based NLP models with phylogenetic and evolutionary data can improve prediction accuracy and create valuable connections between variants and their pathogenicity in NAFLD. Such developments can improve risk stratification within clinical genetics and research while overcoming limitations in GWAS studies that prevent community-wide interpretations.

ABSTRACT

Citrinin (CIT) is a polyketide-derived mycotoxin, which is produced by many fungal strains belonging to the gerena Monascus, Aspergillus, and Penicillium. It has been postulated that mycotoxins have several toxic mechanisms and are potentially used as antineoplastic agents. Therefore, the present study carried out a systematic review, including articles from 1978 to 2022, by collecting evidence in experimental studies of CIT antiplorifactive activity in cancer. The Data indicate that CIT intervenes in important mediators and cell signaling pathways, including MAPKs, ERK1/2, JNK, Bcl-2, BAX, caspases 3,6,7 and 9, p53, p21, PARP cleavage, MDA, reactive oxygen species (ROS) and antioxidant defenses (SOD, CAT, GST and GPX). These factors demonstrate the potential antitumor drug CIT in inducing cell death, reducing DNA repair capacity and inducing cytotoxic and genotoxic effects in cancer cells.

ABSTRACT

Acute appendicitis is one of the most common causes of abdominal pain in children. During the coronavirus disease 2019 (COVID-19) pandemic, a delay in presentation to the emergency department and a higher rate of complicated appendicitis were observed. Traditionally, operative management (laparoscopic or open appendectomy) was thought to be the best treatment strategy for acute appendicitis. However, nonoperative management with antibiotics has gained popularity in managing pediatric appendicitis during the COVID-19 era. The pandemic has posed significant challenges in the management of acute appendicitis. Cancellation of elective appendectomies, delay in seeking care due to fear of contracting COVID-19 infection, and impact of COVID-19 infection in the pediatric population have resulted in higher rates of complications. Furthermore, multiple studies have reported multisystem inflammatory syndrome in children mimicking acute appendicitis, subjecting patients to unnecessary surgery. Therefore, it is imperative to update the treatment guidelines for the management of acute appendicitis in the pediatric population during and after COVID-19 times.

ABSTRACT

Background and Aims: Antibiotic resistance is seen as a worldwide health risk as a result of the overuse of antibiotics. Many countries noted that antibiotic usage was high during the COVID-19 pandemic. The purpose of this study is to evaluate Syrians' knowledge, attitudes, and practice about the use of antibiotics and antibiotic resistance during the COVID-19 epidemic. Methods: A cross-sectional study was conducted using an online questionnaire to collect the data from the Syrian population from February 5 to March 4, 2022. Syrians 18 years or older all over the world were able to participate in this study. A convenience snowball sampling method was used. SPSS version 20.0 was used to analyze the data. To examine the results, binominal logistic regression was used. Statistical significance was defined as a p < 0.05. Results: Out of 2406 respondents, 60.2% knew that transmission of COVID-19 could occur even if the patient has not developed any symptoms, and 91.6% were able to recognize the main clinical symptoms of COVID-19. There was a statistically significant difference between male and female knowledge of COVID-19 (p = 0.002), with males having 3.78 ± 2.1 (2.7-3.87) and females scoring 3.93 ± 2.3 (3.7-4.1). Newly graduated students have more knowledge of COVID-19 than other subtypes of Job (p = 0.0001), and those with medical practice are more knowledgeable than those without (p = 0.0001). Only 16.6% answered that taking antibiotics would not speed up the recovery from all the infections. 65.3% answered correctly that misuse of antibiotics could cause antibiotic resistance. Conclusion: Our study concluded that the Syrian population demonstrated good knowledge of COVID-19 and moderate acceptance of the new norm. Knowledge regarding antibiotic use and resistance and practice of preventive measures was poor, which can encourage the health authorities to develop community education programs to increase public awareness of the usage of antibiotics and safety protocols during the COVID-19 pandemic.

In order to better understand and quantify the effect of instabilities in systems utilizing flow boiling heat transfer, the present study explores dynamic results for pressure drop, mass velocity, thermodynamic equilibrium quality, and heated wall temperature to ascertain and analyze the dominant modes in which they oscillate. Flow boiling experiments are conducted for a range of mass velocities with both subcooled and saturated inlet conditions in vertical upflow, vertical downflow, and horizontal flow orientations. High frequency pressure measurements are used to investigate the influence of individual flow loop components (flow boiling module, pump, pre-heater, condenser, etc.) on dynamic behavior of the fluid, with fast Fourier transforms of the same used to provide critical frequency domain information. Conclusions from this analysis are used to isolate instabilities present within the system due to physical interplay between thermodynamic and hydrodynamic effects. Parametric analysis is undertaken to better understand the conditions under which these instabilities form and their impact on system performance. Several prior stability maps are presented, with new stability maps provided to better address contextual trends discovered in the present study.

ABSTRACT

Investigation of two-phase flow dynamic behavior and instabilities has traditionally centered on phenomena present in boiling flows due to the safety critical nature of boiling in a variety of cooling applications. Analysis of pressure signals in condensing systems reveal the presence of relevant oscillatory phenomena during flow condensation as well, which may impact performance in applications concerned with precise system control. Towards this end, the present study presents results for oscillatory behavior observed in pressure measurements during flow condensation of FC-72 in a smooth circular tube in vertical upflow, vertical downflow, and horizontal flow orientations. Dynamic behavior observed within the test section is determined to be independent of other components within the flow loop, allowing it to be isolated and interpreted as resulting from physical aspects of two-phase flow with condensation. The presence of a peak oscillatory mode (one of significantly larger amplitude than any others present) is seen for 72% of vertical upflow test cases, 61% of vertical downflow, and 54% of horizontal flow. Relative intensities of this peak oscillatory mode are evaluated through calculation of Q Factor for the corresponding frequency response peak. Frequency and amplitude of peak oscillatory modes are also evaluated. Overall, vertical upflow is seen to exhibit the most significant oscillatory behavior, although in its maximum case amplitude is only seen to be 7.9% of time-averaged module inlet pressure, indicating there is little safety risk posed by oscillations under current operating conditions. Flow visualization image sequences for each orientation are also presented and used to draw parallels between physical characteristics of condensate film behavior under different operating conditions and trends in oscillatory behavior detected in pressure signals.

ABSTRACT

Historically, study of two-phase flow instabilities has been arguably one of the most challenging endeavors in heat transfer literature due to the wide range of instabilities systems can manifest depending on differences in operating conditions and flow geometry. This study utilizes experimental results for vertical upflow boiling of FC-72 in a rectangular channel with finite inlet quality to investigate Density Wave Oscillations (DWOs) and assess their potential impact on design of two-phase systems for future space missions. High-speed flow visualization image sequences are presented and used to directly relate the cyclical passage of High and Low Density Fronts (HDFs and LDFs) to dominant low-frequency oscillations present in transient pressure signals commonly attributed to DWOs. A methodology is presented to determine frequency and amplitude of DWO induced pressure oscillations, which are then plotted for a wide range of relevant operating conditions. Mass velocity (flow inertia) is seen to be the dominant parameter influencing frequency and amplitude of DWOs. Amplitude of pressure oscillations is at most 7% of the time-averaged pressure level for current operating conditions, meaning there is little risk to space missions. Reconstruction of experimental pressure signals using a waveform defined by frequency and amplitude of DWO induced pressure fluctuations is seen to have only moderate agreement with the original signal due to the oversimplifications of treating DWO induced fluctuations as perfectly sinusoidal in nature, assuming they occur at a constant frequency value, and neglecting other transient flow features. This approach is nonetheless determined to have potential value for use as a boundary condition to introduce DWOs in two-phase flow simulations should a model be capable of accurately predicting frequency and amplitude of oscillation.

ABSTRACT

Formulation components of organophosphate insecticidal preparations might affect their toxic action in animals. The objective of this study was to examine and compare the acute toxicity and cholinesterase inhibition in seven to 14-day-old chicks dosed orally with dichlorvos and diazinon in standard veterinary and agricultural formulations. The acute (24 h) oral median lethal doses (LD50) of the formulations were determined using the up-and-down method. Respective LD50 of dichlorvos of the veterinary and agricultural formulations in chicks were 11.1 mg kg(-1) and 6.51 mg kg(-1) and those of diazinon 6.4 mg kg(-1) and 6.7 mg kg(-1). Plasma and brain cholinesterase activities were measured by electrometry after in vivo and in vitro exposure to organophosphates. The chicks showed signs of cholinergic toxicosis within one hour of dosing. Dichlorvos (8 mg kg(-1)) and diazinon (4 mg kg(-1)) in the veterinary and agricultural formulation significantly reduced both plasma and brain cholinesterase activities in the chicks. The veterinary formulation of dichlorvos reduced plasma ChE by 60% and agricultural by 40% and brain ChE by 93% and 87%, respectively. In contrast, ChE inhibition by diazinon in the agricultural formulation of diazinon was stronger than by the veterinary formulation; 72% vs. 64% in plasma and 97% vs. 80% in the brain, respectively. The highest in vitro inhibitions were observed with dichlorvos in the agricultural formulation (50%) in the brain samples and with diazinon in the agricultural formulation (52%) in the plasma samples. While they exist, differences between formulations cannot be taken as a rule and further investigations should inventory the toxicity of standard veterinary and agricultural organophosphate formulations in addition to the known data for pure forms.
