ABSTRACT

The anticonvulsant carbamazepine is widely used to treat affective disorders and behavioural disorders in non-epileptic children. We report an elevated plasma level of carbamazepine-10,11-epoxide in a cystinuric child after daily medication with 300 mg carbamazepine while the serum level of carbamazepine was in the therapeutic range. The concentrations of carbamazepine and its epoxide derivative were determined by HPLC. The formation of a glutathione conjugate of carbamazepine-10,11-epoxide is raised as a hypothesis.

ABSTRACT

The relationship between neurological and cognitive features and the outcome of a 28-day neuroleptic treatment were studied in 98 chronic schizophrenic patients admitted for relapse. Handedness, neurological soft signs, and the cognitive rating on the Mini Mental State assessed in a stable state did not correlate with the neuroleptic response. The findings do not provide a basis for predicting the short-term outcome of the treatment.

ABSTRACT

The differences in the psychopathologic status of 26 complaint and 32 noncomplaint schizophrenic patients were evaluated on the basis of rating scales used at discharge. Noncomplaint patients had significantly higher scores for grandiosity, lack of feeling of illness and insight into it. Their score for global psychopathological state and disturbance in social adjustment was also significantly higher. The compliant patients had significantly higher score for self-rated depression.

ABSTRACT

The frequency of stressful life events, the characteristic psychopathological features, and the course of relapse were studied in 22 schizophrenic patients who relapsed while on maintenance neuroleptic medication and 27 noncompliant patients off treatment. The patients who relapsed while on neuroleptics experienced more independent life events, mainly losses and family conflicts, in the months preceding admission. Patients who relapsed while not on drugs were characterized by a more severe psychopathological picture dominated by positive symptoms at admission. Schizophrenic patients who relapsed on medication showed greater improvement and required shorter hospitalization.

ABSTRACT

The predictive value of the subjective response after 4 hours to an oral test dose of haloperidol and the predictive value of early clinical improvement during a 28-day, fixed-dose regimen with haloperidol were tested in 33 newly admitted schizophrenic patients. A significant (p less than .01) correlation was found between subjective response and therapeutic outcome on Day 28. Five of the 7 patients with dysphoric responses to the test dose had poor therapeutic outcomes. Powerful relationships were found between therapeutic outcome on Day 28 and clinical improvement observed from Day 2 on and between the outcome at discharge and improvement measured on Days 5 and 6.
