ABSTRACT

La investigación tuvo por Objetivo: Determinar la relación entre la adherencia al tratamiento y la salud física en los pacientes diabéticos del programa de diabetes de un público del Perú. Material y Métodos: Se utilizó la metodología descriptiva, transversal y de enfoque cuantitativo, de diseño no experimental-correlacional. La muestra fue de 50 pacientes, empleándose la técnica de la encuesta y como instrumento un cuestionario estructurado validado y confiabilidad Alfa de Cronbach de 0.79. Resultados: La adherencia al tratamiento fue regular 86% y bueno 14% y por otro lado, la salud física estuvo medianamente afectada 54%, y nada afectada en el 46%. Conclusiones: Existe relación entre la adherencia al tratamiento y la salud física en los pacientes diabéticos del programa de diabetes (r = 0.322, p = 0.004).


The research was Objective: To determine the relationship between treatment adherence and physical health in diabetic patients in the diabetes program of a public in Peru. Material and Methods:The descriptive, transversal methodology and quantitative approach, with a non-experimental-correlational design, were used. The sample was 50 patients, using the survey technique and as an instrument a validated structured questionnaire and Cronbach's Alpha reliability of 0.79. Results: Adherence to treatment was fair in 86% and good in 14%, and on the other hand, physical health was moderately affected in 54%, and not at all affected in 46%. ABSTRACT

OBJECTIVE: To gain an understanding of the potential role of endoplasmic reticulum (ER) stress in the endometrial compartment during early pregnancy, a highly understudied area. DESIGN: This study examined the regulation of interferon-ß (IFNß) in response to ER stress in human decidualized and nondecidualized endometrial cells (human endometrial stromal cells [HESCs]) in vitro. In vivo, we examined ER stress and the IFNß levels locally in the mouse endometrium before and after implantation at embryonic day (E)1, E3, and E6. SETTING: The study was performed in a reproductive sciences laboratory for Human Growth and Development. PATIENT(S): None. INTERVENTION(S): None. MAIN OUTCOME MEASURE(S): Quantitative polymerase chain reaction, Western blotting, and immunohistochemical analysis allowed us to test the action of endogenous ER stress activation in the endometrial compartment likely triggered by implantation and its ability to increase the endometrial IFNß levels. RESULT(S): In vitro, we observed a significant difference in the IFNß levels in HESCs, in response to ER stress activation, where decidualized HESCs exhibited a threefold increase in the IFNß levels compared with nondecidualized HESCs. Apoptotic caspase-3 activation was also isolated to the decidualized cells as a result of ER stress-dependent suppression of nuclear factor-kappa beta-regulated antiapoptotic factors, XIAP and MCL-1. In vivo, mouse endometrial IFNß was present in F4/80-positive macrophages at all time points examined. After implantation (E6), the mouse luminal epithelial cells robustly coexpressed both IFNß and the ER stress marker immunoglobulin heavy chain binding protein (BiP). CONCLUSION(S): These analyses demonstrate that both in vivo and in vitro, differentiated and decidualized endometrial cells undergoing ER stress have the capacity to produce increased IFNß levels; therefore, ER stress activation in the endometrial compartment may play a vital role in promoting successful implantation events.

ABSTRACT

Fungal endophthalmitis is one of the leading causes of vision loss worldwide. Post-operative and traumatic injuries are major contributing factors resulting in ocular fungal infections in healthy and, more importantly, immunocompromised individuals. Among the fungal pathogens, the Aspergillus species, Aspergillus fumigatus, continues to be more prevalent in fungal endophthalmitis patients. However, due to overlapping clinical symptoms with other endophthalmitis etiology, fungal endophthalmitis pose a challenge in its diagnosis and treatment. Hence, it is critical to understand its pathobiology to develop and deploy proper therapeutic interventions for combating Aspergillus infections. This review highlights the different modes of Aspergillus transmission and the host immune response during endophthalmitis. Additionally, we discuss recent advancements in the diagnosis of fungal endophthalmitis. Finally, we comprehensively summarize various antifungal regimens and surgical options for the treatment of Aspergillus endophthalmitis.

ABSTRACT

La distrofia muscular de Duchenne es una enfermedad grave, incapacitante y progresiva que afecta a 1 de cada 3.500 recién nacidos varones alrededor del mundo. El diagnóstico deberá confirmarse mediante pruebas genéticas para identificar la mutación en el gen DMD, o bien por biopsia muscular e inmunotinción para demostrar la ausencia de distrofina. Aunque actualmente continúa siendo una enfermedad incurable, no significa que no tenga tratamiento. Éste debe ser multidisciplinario, buscando la funcionalidad del paciente y evitando o corrigiendo las complicaciones, principalmente cardiorrespiratorias y esqueléticas. Se han evaluado e implementado múltiples propuestas con la finalidad de mejorar la calidad de vida en estos pacientes. Los esteroides a largo plazo han demostrado importantes beneficios para los pacientes, prolongan la deambulación, reducen la necesidad de cirugía de columna, mejoran la función cardiorrespiratoria, y aumentan la supervivencia y la calidad de vida. En este documento se presentan las recomendaciones con base en la experiencia del grupo de trabajo y de los expertos de ámbito mundial sobre el diagnóstico y el tratamiento con esteroides para los pacientes con distrofia muscular de Duchenne (AU)


Duchenne muscular dystrophy is a severe, debilitating and progressive disease that affects 1 in 3,500 live male births in the world. The diagnosis should be confirmed by genetic testing to identify the mutation in the DMD gene or muscle biopsy and immunostaining to demonstrate the absence of dystrophin. Although up to now continues to be an incurable disease, this does not mean it has no treatment. Treatment should be multidisciplinary, looking for the functionality of the patient and avoiding or correcting complications, mainly cardio-respiratory and skeletal. Many proposals have been evaluated and implemented with the aim of improving the quality of life for these patients. The longterm steroids have shown significant benefits, such as prolonging ambulation, reduce the need for spinal surgery, improve cardiorespiratory function and increase survival and the quality of life. This document presents the recommendations based on the experience of the working group and experts worldwide on the diagnosis and treatment with steroids for patients with Duchenne muscular dystrophy (AU)

ABSTRACT

A Hipertensão arterial é um dos principais problemas de saúde do município de Conselheiro Lafaiete na população adulta. Pela importância no controle e prevenção das complicações desta doença como infarto agudo do miocárdio, hemorragia cerebral e outras, o objetivo foi promover maior adesão a utilização do tratamento não medicamentoso no posto de saúde Djalma Rodriguez Fernandez da comunidade Buarque de Macedo, modificando hábitos e estilos de vida como: alimentação saudável, controle do peso corporal, pratica de exercícios físicos, eliminar hábitos tóxicos, etc, desse modo reduzir a prescrição de anti-hipertensivos orais. Para seu desenvolvimento foi utilizado o método do Planejamento Estratégico Situacional PES, com os pacientes diagnosticados com HAS, elevando o nível de informação da população mediante palestras e atividades educativas, tendo como resultado maior numero de pacientes com tratamento não medicamentoso e qualidade de vida.
