ABSTRACT

Introducción : el objetivo de este estudio fue el evaluar la relación entre la inclinación de los incisivos al finalizar tratamiento de ortodoncia con la estabilidad de la alineación de dientes anteriores usando índice PAR. Métodos: estudio analítico de corte transversal, en 47 pacientes que finalizaron ortodoncia, con radiografía lateral inicial y final, se evaluó el ángulo formado entre plano (Silla-Nasion) (U1-NS) e inclinación del incisivo superior y el ángulo entre inclinación axial del incisivo inferior y plano mandibular (Go-Gn). Se aplicó el índice PAR del sector anterior a modelos pretratamiento (T0), postratamiento (T1) y de seguimiento (T2). El análisis estadístico se realizó mediante distribuciones de frecuencias y porcentuales, prueba T, Anova I, Anova II y Manova; significancia P= 0.05. Resultados: no se encontró asociación entre la inclinación del incisivo superior e inferior, la estabilidad en alineación y el puntaje ponderado del PAR entre T2-T1 (P>0.05). El PAR disminuyó 75.29% de T0 a T1 y 58.79% de T0 aT2, con recidiva de 16,5%. No hubo asociación entre tipo de retenedor y puntaje ponderado del PAR. De T0 a T2 hubo asociación entre la interacción de la inclinación del incisivo superior (P=0.03) e inferior (P=0.04), con el puntaje total ponderado del índice PAR. Conclusión: no hubo asociación entre la modificación de la inclinación de los incisivos con la estabilidad del sector anterior. Al terminar ortodoncia se presentó un nivel de corrección alto en el sector antero-superior e inferior, sin embargo, hubo recidiva de 16.5%.


Introduction: the objective of this study was to evaluate the relationship between incisor inclination at the end of orthodontic treatment with the anterior teeth alignment stability using PAR index. Methods: analytical cross-sectional study, the angle formed between the plane (Silla-Nasion) (U1-NS) and the inclination of the upper incisor and the angle between axial inclination of the lower incisor and mandibular plane (Go-Gn), were measured in 47 initial and final lateral radiographs of patients who finished orthodontic treatment. The anterior sector PAR index was applied to pretreatment (T0), posttreatment (T1) and follow-up (T2) casts. Statistical analysis was performed using frequency and percentage distributions, T test, Anova I, Anova II and Manova; significance p = 0,05. Results: no association was found between upper and lower incisor inclination, alignment stability and PAR weighted score between T2-T1 (p> 0,05). The PAR decreased 75,29% from T0 to T1 and 58,79% from T0 to T2, with a recurrence of 16,5%. There was no association between retainer type and PAR weighted score. From T0 to T2 there was an association between the interaction of the incisor inclination of upper (p = 0,03) and lower (p = 0,04), with the weighted total score of the PAR index. Conclusion: there was no association between the modification of the incisor inclination with the stability of the anterior sector. At the end of orthodontic treatment there was a high level of correction in the anterior-superior and inferior sector, however, there was a recurrence of 16,5%.

ABSTRACT

BACKGROUND: Telehealth has become a ubiquitous tool for healthcare providers. However, concepts of telehealth are missing from many Doctor of Nursing Practice (DNP) curricula. This quality improvement project aimed to evaluate telehealth education implementation into a Doctor of Nursing Practice curriculum. METHODS: The Iowa Model of Evidenced-Based Practice to Promote Quality Care guided the quality improvement project. An online module regarding telehealth was developed based on the American Telemedicine Association (ATA), the Health Resources and Services Administration (HRSA), and HealthIT.gov and was integrated into an existing course. Students completed a pre and posttest regarding telehealth knowledge and levels of satisfaction with the online module. RESULTS: Thirty-seven students participated in the program. Mean knowledge scores increased from 4.9 to 8.6, and the modal for satisfaction and self-confidence was strongly agree. CONCLUSION: As telehealth is now a mainstream method of clinical practice, schools of nursing must adjust the curriculum to prepare nurses for this form of care delivery. An online module is a sustainable method of providing the foundation of knowledge for future expansion. Practicing via telehealth presents unique strengths and challenges. Nurses must be informed of the nuances and special considerations involved to safely practice using this distinctive care modality.

ABSTRACT

Coordinated gastric smooth muscle contraction is critical for proper digestion and is adversely affected by a number of gastric motility disorders. In this study we report that the secreted protein Mfge8 (milk fat globule-EGF factor 8) inhibits the contractile responses of human gastric antrum muscles to cholinergic stimuli by reducing the inhibitory phosphorylation of the MYPT1 (myosin phosphatase-targeting subunit (1) subunit of MLCP (myosin light chain phosphatase), resulting in reduced LC20 (smooth muscle myosin regulatory light chain (2) phosphorylation. Mfge8 reduced the agonist-induced increase in the F-actin/G-actin ratios of ß-actin and γ-actin1. We show that endogenous Mfge8 is bound to its receptor, α8ß1 integrin, in human gastric antrum muscles, suggesting that human gastric antrum muscle mechanical responses are regulated by Mfge8. The regulation of gastric antrum smooth muscles by Mfge8 and α8 integrin functions as a brake on gastric antrum mechanical activities. Further studies of the role of Mfge8 and α8 integrin in regulating gastric antrum function will likely reveal additional novel aspects of gastric smooth muscle motility mechanisms.

ABSTRACT

Background: Caregivers of children with asthma demonstrate higher levels of anxious and depressive symptoms when compared to caregivers of healthy children. Objectives: The objectives of this study were to: 1) Evaluate feasibility and acceptability of two nurse-led, remotely offered interventions for caregivers of children with asthma; 2) Compare effectiveness of two interventions (a virtual education session and a virtual education session supplemented with a telehealth visit) in relation to caregiver outcomes, and 3) Assess the preliminary effect of the interventions on caregivers' knowledge of asthma, sleep, anxiety and depressive symptoms. Methods: A mixed methods approach was used inclusive of a qualitative, descriptive design and randomized controlled trial design. Caregivers were provided virtual education and telehealth visits and evaluated from pre-posttest. Results: The intervention was found to be feasible and acceptable. Both the virtual education session and telehealth visit were effective. The intervention had a significant effect on caregiver's asthma knowledge and depressive symptoms (p<.05), but did not affect caregiver's sleep or anxiety. Qualitative analysis of the virtual educational session revealed themes of 1) valuable learning experience, 2) more medication education needed, and 3) appreciated remote format. Qualitative analysis of the telehealth visits revealed themes of 1) educational, helpful, and worthwhile and 2) virtual offering was easy and convenient. Conclusions: Remotely conducted, nurse-led interventions such as virtual education sessions and telehealth visits are a feasible, acceptable, and effective way to improve caregiver outcomes.

ABSTRACT

Total anomalous pulmonary venous return (TAPVR), also known as total anomalous pulmonary venous connection, is a congenital cardiovascular malformation that presents itself in the neonatal period, with cyanosis and tachypnea. There are 4 types of TAPVR with the mixed type being the least common. Any type of TAPVR may be associated with obstruction as result of flow redirection through the liver parenchyma before it may return to the heart, but infracardiac is the most common one. We report a case of a 10-hour-old female, with a mixed (cardiac and infracardiac) TAVPR with obstruction, that showed drainage to the coronary sinus and the portal vein, as the other classic findings in TAVPR, made with computed tomography angiography and echocardiogram. The patient was taken to surgical repair, but unfortunately died during the procedure because of multiple complications.

ABSTRACT

La Biopsia por Aspiración con Aguja Fina (BACAF) es el método de elección en el diagnóstico de nódulos tiroideos, mamarios, ganglios linfáticos y glándulas salivares. En piel y tejidos blandos ha sido utilizada con poca frecuencia. Presentamos dos casos de endometriosis extrapélvica, localizada en tejidos blandos y pared abdominal. Se trata de dos mujeres con nódulos dolorosos en la pared abdominal y sobre cicatriz de cirugía ginecológica. El diagnóstico se confirmó en una de las pacientes con biopsia excisional, otra recibió tratamiento médico.


Fine-Needle Aspiration Biopsy is the method of choice for the diagnosis of thyroid, breast, lymph and salivary glands nodules; nevertheless, it’s use has been poor regarding sking and soft tissues evaluation. In the following paper, the authors present two cases of extrapelvic endometriosis, located on soft tissues and abdominal wall, in two women with tender nodes on abdominal wall and over a gyneacologic surgery scar. Diagnosis was confirmed on one of the patients via excisional biopsy; the other one underwent medical management.
