ABSTRACT

The exposome has an impact on skin from life-long exposure. Acute short-term exposure to exposome stressors can also alter skin functions such as skin physical barrier and immune defenses, leading to skin dryness, sensitivity, flares of inflammatory skin conditions, or viral reactivations. Probiotics are defined as live microorganisms, which, when administered in adequate amounts, confer a health benefit on the host. An extract produced by lysing Vitreoscilla filiformis (VfeV) cultured in Vichy volcanic mineralizing water (VVMW) has properties of probiotic fractions. In this review, we present in vivo and ex vivo studies with a dermocosmetic formulation containing 80% VVMW, 5% VfeV, 4% niacinamide (vitamin B3), 0.4% hyaluronic acid, and 0.2% vitamin E (M89PF) to evaluate the clinical efficacy in preventing and repairing stressed skin. Skin barrier benefits of M89PF were shown in studies after the skin was exposed to sudden thermal changes, after skin irritation by tape stripping, and in sleep-deprived women. M89PF significantly accelerated skin renewal compared to untreated skin. Skin antioxidant defense activity of M89PF was shown after exposure to stress from UVA plus cigarette smoke aggression. Skin microbiome recovery after acute stress from a harsh cleanser was significantly better in M89PF-treated skin compared to bare skin. Clinical benefits of M89PF on correcting clinical signs of stressed skin were shown in both Caucasian and Asian women exposed to a stressful lifestyle and various external (pollution, tobacco smoking, solar radiation) and internal (poor sleep, stressful work, unbalanced diet, and alcohol consumption) exposome factors. M89PF also showed depigmenting properties on dark spots in Asian women. Further clinical studies are now warranted to evaluate the efficacy of M89PF as adjuvant care to prevent and repair skin barrier disruption and reinforce skin defenses in skin exposed to acute stresses.

El plasma autólogo rico en plaquetas (PARP). En el lugar de lesión acelera la regeneración de los tejidos locales mediantes un mecanismo que reproduce las etapas fisiológicas iniciales de la reparación tisular. Comparar la eficacia del PARP y la acetonida de triacinolona infiltrada en pacientes con diagnóstico de Entesitis calcánea plantar (ECP). 60 pacientes de ambos sexos, mayores de 30 años que acudieron a la consulta en junio 2008 a mayo 2009, Grupo A 30 pacientes infiltrados con 1,5 cc de esteroide diluido en 3 cc cifarcaína al 1 por ciento; Grupo B 30 pacientes infiltrado PARP 3,5 cc diluido en 1 cc de cifarcaína al 1 por ciento ambos grupos se indica cambio del tipo de calzado de uso diario y se controlaron cada 7 días. Grupo A I Control el 76,6 por ciento refirió no presentar dolor, el 23,3 por ciento presento dolor. En el III Control 83,3 por ciento de los pacientes refirió dolor, ameritó la segunda infiltración, el resto mostró una marcada mejoría. El Grupo B I Control 84 por ciento refirió dolor leve, el resto dolor moderado, a las 2 semanas, 88 por ciento desaparecio del dolor y el 12 por ciento dolor leve, no ameritaron una segunda infiltración. El PARP en 28 días provocó desaparición del dolor en 94 por ciento de los paciente comparado con 86 por ciento que uso esteroide. El análisis no paramétrico Ji cuadrado es 54,78 por ciento con grado de libertad 3 demostrando que las variables están relacionadas con resultados significativos.


The platelet-rich autologous plasma (PRAP) in the site of injury accelerates tissue regeneration by a local mechanism that reproduces the initial stages of physiological tissue repair. To compare the effcacy of infiltrating PRAP or triancinolona in diagnosed patiente with calcaneal plantar enthesitis (CPE). 60 patients of both sex, older than 30 years old who attendend the consultation from june 2008 to may 2009, Group A 30 patients with infiltrating 1,5 cc of diluted steroid 3 cc cifarcaina 1 percent, Group B 30 patients infiltrated PRAP 1 cc to 3,5 cc diluted cifarcaina 1 percent, both groups indicated change in the type of footwear for everyday use and were checked every 7 days, Group A I Control the present 76,6 percent referred no pain, 23,3 percent had pain. Control III in 83,3 percent of patients referred pain, they need the second infiltration, the rest showed a marked improvement. Group B I Control 84 percent referred mild pain, moderate pain the rest; at 2 weeks, 88 percent referred the disappearance of pain and 12 percent mild pain, did not need a second infiltration; The PRAP 28 days resulted in disappearance of pain in 94 percent of patients compared with 86,6 percent who use steroids. The non-parametric chi-square analysisis 54,78 with 3 degrees of freedom showing that the variables are associated with significant results.

ABSTRACT

The efficacy and safety of a new formulation of metronidazole 1 percent cream applied once daily was compared to vehicle cream in a double-blind, randomized, parallel group, ten-week clinical study. The results showed that metronidazole 1 percent cream was significantly better than vehicle in reducing the lesions of rosacea, improving erythema, and physician's global rosacea scores. The incidence of adverse events related to the skin was low.

ABSTRACT

We conducted a double-blind, placebo-controlled, prospective, randomized study to assess the effects of tretinoin pretreatment on healing after trichloroacetic acid (TCA) chemical peel. Sixteen male patients (mean age, 67 years) with actinically damaged skin were treated daily with 0.1% tretinoin and placebo creams to the left and right halves of the face and the left and right forearms and hands, respectively, for 14 days prior to the 35% TCA peel. We subjectively noted that during the peel, "frosting" was more pronounced and uniform and occurred earlier in tretinoin-pretreated skin in 94% of the patients. Healed skin was measured planimetrically, and the healed area was determined with point stereology. Regardless of pretreatment, the face healed twice as fast as the forearm or hand. In all regions, the mean area healed was significantly greater in skin that had been pretreated with tretinoin. The differences between tretinoin and placebo, respectively, in healed skin were maximal after 5 days for the face (68% vs 52%), after 11 days for the forearms (72% vs 24%), and after 9 days for the hands (61% vs 29%). After 7 days, 75% of the tretinoin-pretreated hemifaces were completely healed, as opposed to 31% of the placebo-pretreated hemifaces. By visual inspection, we could not appreciate a cosmetic difference between tretinoin- and placebo-pretreated skin 2 weeks and 3 months after the TCA peel. We conclude that 0.1% tretinoin pretreatment for 2 weeks prior to the TCA peel will significantly speed healing, which may result in greater patient satisfaction. Patients presently being treated with tretinoin who later undergo a TCA peel might be expected to have similar results.

ABSTRACT

The pruritic papular eruption of the acquired immunodeficiency syndrome is characterized by generalized, pruritic, skin-colored papules and nodules. Chronic lesions are excoriated and hyperpigmented. The eruption and pruritus typically wax and wane and are resistant to oral antihistamine and topical steroid therapy. The characteristic histologic features are (1) superficial and mid dermal perivascular and perifollicular mononuclear cell infiltrate with numerous eosinophils and (2) follicular damage of varying degrees. When compared with control subjects, these patients did not demonstrate any significant difference in laboratory or demographic data.

ABSTRACT

A 35-year-old obese black American woman presented with nausea, vomiting, diarrhea, fever, cough, and chest pain of 2 weeks duration. She was pancytopenic and acidotic, with respiratory failure and hypotension. A diagnosis of septic shock was made, and the patient died 48 hours after admission. Blood cultures were positive for organisms that were reported to be Klebsiella rhinoscleromatis. At autopsy she had massive hepatic necrosis with numerous Mikulicz's cells. The lungs, spleen, and bone marrow were also involved. To our knowledge, this is the first report of a case of systemic infection with K rhinoscleromatis.

We report the case a 69-year-old woman with dermatofibrosarcoma protuberans, present, apparently, since birth. To our knowledge, this case represents the longest reported duration of a dermatofibrosarcoma protuberans.

ABSTRACT

Primary mucinous carcinoma of the skin is a rare entity. It usually presents as a painless nodule, cyst, or ulcer in the head and neck region (especially the eyelid). These tumors have a low metastatic potential but tend to recur locally. Histologically, groups of light- and dark-staining epithelial cells form cords, nests, and ducts separated by clear areas representing sialomucin. Due to tumor continuity and a low rate of lymphatic or hematologic spread, Mohs micrographic surgery appears to be an effective treatment. Herein, two cases of primary mucinous carcinoma are presented.

ABSTRACT

In 1980 a syndrome was first described in two adult males, consisting of macrocephaly, pigmented macules on the glans and shaft of the penis, and hamartomatous intestinal polyps. Since then, 10 additional cases have been identified. Herein, we present two new cases and review the cutaneous manifestations as well as additional features in patients with the Ruvalcaba-Myhre-Smith syndrome.

ABSTRACT

Bloom's syndrome is a rare autosomal recessive disorder that characteristically shows a telangiectatic, sun-sensitive facial rash in conjunction with stunted growth. These patients are prone to respiratory and gastrointestinal infections along with immunologic abnormalities. Chromosomal aberrations include an elevated frequency of sister chromatid exchange in cultured lymphocytes, a phenomenon pathognomonic for this disorder. These patients exhibit a great tendency for neoplasia, particularly acute leukemia and lymphoma. Three cases are reported, including a follow-up of a patient diagnosed 16 years ago.

Syringoid eccrine carcinomas (SECs) are rare adnexal neoplasms that may display aggressive local recurrence and occasional metastases. This report describes three cases of SEC treated by Mohs micrographic surgery.
