ABSTRACT
Rapid deceleration while wearing a lap-shoulder strap seatbelt may result in a traction injury to the brachial plexus on the side of the shoulder strap. Occult vascular injury should be considered in patients with this injury pattern. The deficit will recover after a neuropraxic type injury.

Many arthritic conditions can affect the small joints of the hand and wrist. An understanding of the disease process helps in managing the problem. Conservative care generally consists of rest, splinting, use of anti-inflammatory drugs, intra-articular injection with corticosteroids, and rehabilitation therapy. Surgical procedures for the arthritic hand are reserved for persistent cases of pain or instability that do not respond to conservative treatment.

A randomized, prospective study comparing Ender nailing with external fixation for open fractures of sixty-three tibiae (sixty patients) was undertaken. Ender nailing proved to be at least as effective as external fixation with respect to seven parameters: time to union, tibial alignment, total number of operations, ranges of motion of the knee and ankle, pain, presence of infection, and complications. Ender nailing is a safe alternative to external fixation for grade-I and grade-II open fractures. It should not be used for comminuted fractures, which might shorten about the nails.

A six-year-old boy with divergent elbow dislocation is reported, emphasizing the rarity of this condition. A literature review revealed only three roentgenographically documented divergent elbow dislocations in the pediatric age group. Reduction is usually accomplished with ease; however, a full range of elbow motion and forearm rotation must be obtained to ensure complete reduction of the radiohumeral joint.
