ABSTRACT

Introducción: Los quistes epidermoides (QE) son lesiones de lento crecimiento y naturaleza benigna. Se presenta un caso de un desarrollo de un linfoma cerebral (LC) con relación a un tumor epidermoide, segundo caso descrito en la literatura. Presentación del caso: Mujer de 40 años con QE intracraneal de larga evolución que desarrolla una lesión intraaxial rápidamente progresiva de acuerdo con dicho tumor. Tras la resección el diagnóstico es de linfoma primario difuso de células B. Discusión: La transformación de QE a carcinomas de células escamosas ha sido reportado en contadas ocasiones, siendo el desarrollo de otras lesiones malignas conforme este proceso, absolutamente excepcional. Se han invocado mecanismos inflamatorios como la causa de dicha transformación. En la génesis de los linfomas están involucrados estos mecanismos y en el caso que nos ocupa, podría haber jugado un papel en el desarrollo del tumor. Conclusiones: Aún siendo lesiones benignas, los QE tienen cierto potencial de malignización secundario a mecanismos de inflamación crónica (AU)


Introduction: Epidermoid cysts (EC) are benign and slow growing lesions. A primary brain lymphoma development related to a EC is presented, second case described in literature. Case presentation: A woman 40 years old, harbouring a EC for more than 20 years, develops a fast growing brain lesion next to the EC. Surgery was performed and diagnosis was primary diffuse B cells lymphoma. Discussion: Malignant transformation of EC has been described, usually to squamous cells carcinoma, and much less frequently, to another tumours. Inflammatory mechanisms have been advocated to explain this evolution. Chronic inflammation and lymphoma genesis are related, and this could be the mechanism behind this rare evolution of an EC. Conclusions: Even being benign lesions, EC can develop malignant tumours due to the chronic inflammation secondary to them (AU)

ABSTRACT

OBJECTIVE: To analyse the characteristics of the perceived quality in hospitals of the Andalusia healthcare system and compare this with that in Andalusian Neurosurgery departments. MATERIAL AND METHODS: Randomised surveys, adjusted for working age, were performed in Andalusia using a telephone questionnaire based on the SERVQUAL model with the appropriate modification, with the subsequent selection of a subgroup associated with neurosurgery. Perceived quality was classified as; technical, functional and infrastructure quality. RESULTS: The overall satisfaction was 76.3%. Frequency analysis found that variables related to the technical quality (good doctors, successful operations, trained staff, etc.) obtained more favourable outcomes. Those related to time (wait, consulting, organizing schedules) obtained worse outcomes. The care of families variables obtained poor results. There was no difference between the overall Andalusian healthcare system and neurosurgery departments. In the mean analysis, women and older people gave more favourable responses, especially for variables related to infrastructure quality. In the "cluster" analysis, there were more favourable responses by elderly people, with no differences in gender (P<.009). CONCLUSIONS: There is no difference in perceived quality between the Andalusian healthcare system overall and neurosurgery departments. The perceived quality of the Andalusian healthcare system is higher in the elderly people. The analysis of perceived quality is useful for promoting projects to improve clinical management.

ABSTRACT

Objetivo: Analizar las características de la calidad percibida de los usuarios del sistema público hospitalario de salud de Andalucía y compararla con las de los usuarios de los servicios de neurocirugía andaluces. Material y métodos: Encuestas aleatorizadas ajustadas por población activa en la comunidad andaluza mediante cuestionario telefónico basado en modelo SERVQUAL con modificación propia, y selección posterior de subgrupo relacionado con los servicios de neurocirugía. Se dividió la calidad percibida en calidad técnica, funcional y de infraestructuras. Resultados: La satisfacción general fue del 76,3%. Del análisis de frecuencias se observó que las variables relacionadas con la calidad técnica (buenos médicos, éxito en operaciones, personal preparado, etc.) obtuvieron resultados más favorables. Aquellas relacionadas con el tiempo (espera, en consulta, organización de horarios) obtuvieron resultados más desfavorables. Las variables de atención a familiares obtuvieron resultados desfavorables. No existieron diferencias entre el global de la sanidad andaluza y los servicios de neurocirugía. En el análisis de medias las mujeres y las personas de mayor edad presentaron respuestas más favorables, sobre todo para variables relacionadas con la calidad de infraestructuras. En el análisis de cluster se observó tendencia a respuestas más favorables conforme la edad era mayor sin predominio de género (p < 0,009). Conclusiones: No hay diferencias en la calidad percibida entre los servicios de neurocirugía y el resto de la sanidad. La calidad percibida en la sanidad andaluza es mayor conforme la edad del usuario es mayor. El análisis de calidad percibida es útil para plantear proyectos de mejora de la gestión clínica (AU)

ABSTRACT

There are several complications and disadvantages related to sitting position. Among the most frequent are air embolism, pneumocefalus, quadriplegia presumably due to flexion myelopathy, the risk of haematomas at the operative site and the fatigue of the surgeon. Despite the advent of technical innovations the choice of patient position is still done depending on the surgeon preference or experience. We report an unusual complication of sciatic neuropathy due to the pyriformis syndrome after an operation performed in a 29 year old patient who underwent removal of an acoustic neurinoma in sitting position.

ABSTRACT

Son varias las complicaciones y los inconvenientes derivados de la posición sentado, entre las más frecuentes se encuentran el embolismo aéreo, el neumoencéfalo, la temible tetraplejia en los paciente afectos de mielopatía cervical, el sangrado postquirúrgico o la incomodidad del Neurocirujano. La posición de los pacientes y el acceso a la fosa posterior sigue siendo un tema de actualidad, en el que abundan los defensores de uno u otro abordaje. La aparición de la nueva generación de microscopios, mesas quirúrgicas y navegadores, han influido de alguna manera, pero la elección de la posición sigue siendo aleatoria, dependiendo de las costumbres y de la experiencia de cada cirujano. El objetivo del presente trabajo, es presentar una complicación poco descrita relacionada con la posición sentado. Se trata de un paciente de 29 años intervenido de neurinoma del acústico, mediante abordaje suboccipital paramedial, presentando como complicación, parálisis del nervio ciático secundaria a la compresión muscular piriforme, conocido como el "síndrome piriforme" ocurrida en la posición sentado. ABSTRACT

We report the case of a 54-year-old woman with cephalea of five months duration caused by a chronic subdural hematoma that appeared after epidural anesthesia and accidental dural puncture for bilateral saphenectomy. Computed tomography of the brain revealed an intracranial hematoma. After surgical evacuation, the patient's headaches resolved and she recovered fully. The appearance of cephalea after dural puncture is a relatively frequent complication of spinal anesthesia, but its persistence over time, as well as changes in its initial characteristics, should lead to a suspicion of life-threatening subdural hematoma.
