ABSTRACT

Introduction: Small bowel obstruction (SBO) is known as a common cause of acute abdominal complaints in the emergency department (ED). The modality of choice for the diagnosis of SBO has not yet been established. This systematic review and meta-analysis aimed to investigate the accuracy of ultrasonography for the diagnosis of SBO. Methods: Systematic search was performed on five electronic databases including Medline, Scopus, Web of Sciences, Embase, and Cochrane Library, and the retrieval period was from the inception of each database to November 2023. The quality of the included studies were investigated using the Quality Assessment of Diagnostic Accuracy Studies 2 (QUADAS-2). The pooled values of diagnostic characteristics for ultrasonography were estimated using meta-Disc and Stata statistical software. Results: Twenty-one studies with a total of 1977 patients were included in the meta-analysis. The pooled estimate for sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), diagnostic odds ratio (DOR), and area under the summary ROC curve of ultrasonography for diagnosing SBO were 0.93 (95% CI: 0.91-0.95), 0.8 (95% CI: 0.77-0.83), 5.69 (95% CI: 3.64-8.89), 0.1 (95% CI: 0.07-0.16), 83.51 (95% CI: 18.12-182.91) and 0.96, respectively. Conclusion: The findings of this meta-analysis showed that the utilization of ultrasonography holds promise as a diagnostic imaging for SBO with high accuracy. However, additional worldwide studies are essential to get more evidence on the value of ultrasonography for the diagnosis of SBO.

ABSTRACT

Magnetic particles are routinely used in many biochemical techniques. As such, the manipulation of these particles is of paramount importance for proper detection and assay preparation. This paper describes a magnetic manipulation and detection paradigm that allows sensing and handling highly sensitive magnetic bead-based assays. The simple manufacturing process presented in this manuscript employs a CNC machining technique and an iron microparticle-doped PDMS (Fe-PDMS) compound to create magnetic microstructures that enhance magnetic forces for magnetic bead confinement. Said confinement, generates increases in local concentrations at the detection site. Higher local concentrations increase the magnitude of the detection signal, leading to higher assay sensitivity and lower limit of detection (LOD). Furthermore, we demonstrate this characteristic signal enhancement in both fluorescence and electrochemical detection techniques. We expect this new technique to allow users to design fully integrated magnetic bead-based microfluidic devices with the goal of preventing sample losses and enhancing signal magnitudes in biological experiments and assays.

ABSTRACT

To compare the pattern of brain waves in video game addicts and normal individuals, a case-control study was carried out on both. Thirty participants were recruited from 14 to 20 years old males from two gaming centers. Twenty healthy participants were gathered from different schools in Tehran using the available sampling method. The QEEG data collection was performed in three states: closed-eye and open-eye states, and during a working memory task. As expected, the power ratios did not show a significant difference between the two groups. Regarding our interest in the complexity of signals, we used the Higuchi algorithm as the feature extractor to provide the input materials for the multilayer perceptron classifier. The results showed that the model had at least a 95% precision rate in classifying the addicts and healthy controls in all three types of tasks. Moreover, significant differences in the Higuchi Fractal Dimension of a few EEG channels have been observed. This study confirms the importance of brain wave complexity in QEEG data analysis and assesses the correlation between EEG-complexity and gaming disorder. Moreover, feature extraction by Higuchi algorithm can render support vector machine classification of the brain waves of addicts and healthy controls more accurate.

ABSTRACT

The function of tissue cells is strongly depends on the extracellular matrix (ECM) that can guide and support cell structure. This support plays a crucial role in the process of cell proliferation and differentiation. Herein, three different nanofibrous scaffolds that are highly attractive for tissue engineering were selected and then osteogenic related genes and protein expression patterns of human adipose-derived mesenchymal stem cells (AT-MSCs) were investigated when grown on substrates. Polycaprolactone, Poly (L-lactic acid) and Polyvinylidene-fluoride nanofibrous scaffolds were fabricated using Electrospinning method and then AT-MSCs viability and osteogenic differentiation were evaluated while cultured on them. The highest AT-MSCs survival rate when grown on the scaffolds was detected when grown on Polyvinylidene-fluoride. In addition, the highest ALP activity and mineralization were also observed in differentiated AT-MSCs has grown on Polyvinylidene-fluoride. The expression levels of Runx2, osteonectin and osteocalcin genes and osteocalcin protein in the AT-MSCs has grown on the Polyvinylidene-fluoride were also significantly higher than the rest of the scaffolds. Based on the results, it seems that since the studied substrate have a similar structural characteristics, their nature may have an important role in the stem cell's osteogenesis process, where the Polyvinylidene-fluoride piezoelectricity was a most distinguished characteristic.

ABSTRACT

Introduction: Frequent and regular exercises in the first six months of stroke may cause return of a significant portion of sensory and motor function of patients. This study aimed to examine the effects of passive range of motion exercise in the acute phase after stroke on motor function of the patients. Methods: A randomized controlled trial study was conducted. The patients with first ischemic stroke were randomly allocated to either experimental (n=33) or control (n=19) group. Passive range of motion exercises was performed in the experimental group during the first 48 hours of admission as 6 to 8 times of 30 minute exercise. Before intervention, and one and three months after intervention, motor function were measured by muscle strength grading scale (Oxford scale) and compared. SPSS version 13.0 for Windows was used for statistical analysis. Frequency distribution was used to describe the data. For comparisons, paired t-test, independent t-test was used, and repeated measures test was used. Results: In acute phase, the intervention in the experimental group led to significant improvement of motor function between the first and third month in both the upper and lower extremities. In control group, improvement was observed only in the muscle strength of upper extremity in the first and third month compared to pre-intervention measurement. The greatest improvement was observed in the interval from base to one month in the upper extremity, and base to the first month and the first to the third month in the lower extremity. Conclusion: It is recommended to use early passive range of motion exercise as part of care for people with stroke during the acute phase of the disease.

BACKGROUND: This study aimed to compare the effectiveness of two different protocols of wearing vacuum-formed retainers (VFRs) with the standard protocol of wearing Hawley retainer in maintaining the results of orthodontic treatment. MATERIAL AND METHODS: This single-blind randomized clinical trial consisted of 90 patients who finished orthodontic treatment at the Department of Orthodontics of Mashhad Dental School, and required removable retainers. The participants were randomly divided into 3 groups and received the following treatments. Group 1: Hawley retainers (4 months full-time and then night-only); group 2: VFR_4M (4 months full-time and then night-only); group 3: VFR_1W (1 week full-time and then night-only). The study models were prepared after debond and at 4 and 8 months later, and intercanine width, intermolar width, arch length and the Little's irregularity index were compared between groups. RESULTS: No significant differences were found in intercanine and intermolar widths between groups (P<05). Upper arch length was significantly lower in Hawley group than the two VFR groups (p<0.05), but lower arch length values were comparable. Upper irregularity index was significantly lower in two VFR groups compared to Hawley group (p<0.05), whereas in the lower jaw, only VFR_4M group showed significantly lower crowding than Hawley group (p<0.05). CONCLUSIONS: Both retention regimens of VFRs were more effective than Hawley retainer in maintaining arch length and tooth alignment in the upper arch. For better incisor alignment in the lower jaw, the patients should be advocated to wear VFR 4 months full-time and then at night instead of wearing Hawley retainer.
