Macroenzymes are serum enzymes that have a greater molecular mass than the corresponding enzyme normally found in serum (Klonoff. West J Med 1980; 133: 392-407). Serum AST (aspartate aminotransferase) has rarely been reported to complex with immunoglobulins, resulting in an elevation in serum AST activity. Physicians should be aware of this condition so that patients are not subjected to invasive and costly procedures unnecessarily (Litin et al. Mayo Clin Proc 1987; 62: 681-687). We report on such a patient with a raised AST due to macroenzyme formation.

We describe a case of rhabdomyolysis with renal dysfunction in a 41-year-old previously healthy man with hypothermia. We believe this to be the first case of rhabdomyolysis and hypothermia so far described in the UK.

The mucosal potential difference in the body of the stomach was measured in 18 patients with gastric ulcers, five with gastritis, and three with pernicious anaemia. The results were compared with those of 12 normal volunteers. Significantly lower than normal levels were found in the groups of patients with gastric ulcers and gastritis. The patients with pernicious anaemia had even lower potential differences. In an additional 25 subjects, measurements were made in the duodenal bulb and at five sites in the stomach under direct vision at endoscopy. Biopsies were taken from each site for histology. Subjects with gastritis at the high lesser curve had a significantly lower potential difference at that site than those without gastritis. In the latter group, the potential difference at the high lesser curve was as low as in the antrum, and invariably lower than at the high greater curve.

A mentally subnormal patient presenting with oedema was found at gastroscopy to have a large trichobezoar and multiple gastric polyps. The serum concentrations of albumin and IgG were low in the absence of proteinuria, and the gastrointestinal clearance of radiochromium after the intravenous administration of radiochromic chloride was increased. These findings are compatible with increased gastrointestinal loss of plasma proteins. In addition, the patient had steatorrhoea. All of these abnormal findings were markedly improved after operative removal of the bezoar.
