Numerous observations in children under two years of age tend to demonstrate that the functional amblyopia of esotropia is not congenital in origin, but develops progressively between the ages of 3 months and 2 years. The initial motor disorder is an abduction deficiency. When this is symmetrical there is no amblyopia, but there is a greater risk for the deviated eye to become amblyopic when the deficiency is, or becomes, asymmetrical. Preventive treatment of the amblyopia involves the wearing of symmetrical nasal sectors, with associated partial occlusion in established cases, duration of which depends upon the child's age. Marked amblyopia with nasal fixation requires the use of a nasal sector placed in front of the amblyopic eye (associated with occlusion of the normal eye).

Certain specific points concerning the treatment of divergent strabismus are discussed, particular emphasis being placed on the importance of the sensorial element, which must be restored to normal levels in order to obtain complete recovery. Orthoptic reeducation is a highly effective necessary stage, and should be attempted in all its forms. Excellent or good results have been obtained in 85 p. cent of cases, 1 half of these by the use of orthoptic exercises alone.

The operative indications are more or less difficult according to the clinical type ; some of them belong to a very high specialization. The operative technique is not very difficult if the operator is accustomed with the surgery of the posterior segment. On the contrary, the operator who is only accustomed with the surgery of the anterior segment or the classical techniques for strabismus, will find real difficulties.
