ABSTRACT

Two novel genotypes of species human mastadenovirus D designated 109 and 110 were isolated from three epidemiologically unrelated cases of acute respiratory disease detected in January 2018 by surveillance efforts at the California/Mexico border. Both genotypes represent examples of intertypic recombination. Genotype D109 is most closely related to genotype D56 (97.68% genomic similarity) and features a type D22-like penton base, a type D19-like hexon gene, and a type D9-like fiber [P22/H19/F9]. On the other hand, genotype D110 is most closely related to type D22 (96.94% genomic similarity) and features a type D67-like penton base, a novel hexon gene, and a type D9-like fiber [P67/H110/F9]. Importantly, the fibers of both novel genotypes are highly similar to those of genotypes D56 and D59, which have also been isolated from a few cases of respiratory infections. The present report shows data contributing to the understanding of the molecular determinants of the expanded tissue tropism of certain members of species HAdV-D.

ABSTRACT

BACKGROUND: Dogs are vulnerable to pathogens transmitted by brown dog ticks. An epidemic of Rocky Mountain spotted fever (RMSF) is underway in Mexicali, a Mexican city bordering California affecting people and dogs; several human cases have been reported in California residents who travelled to Mexico. To evaluate risks of RMSF, we conducted seroprevalence surveys in Imperial County in 2016 and 2017 using dogs as sentinels. METHODS: Blood was collected from 752 dogs and was tested for antibodies against R. rickettsii, E. canis and A. phagocytophilum (as a proxy for A. platys). Samples were considered seropositive to spotted fever group rickettsia (SFGR) if the R. rickettsia titre was ≥1:64 and seropositive to E. canis and A. phagocytophilum if the titre was ≥1:32. Owners provided information on dog age, exposure risks, health status and tick prevention. We assessed associations between SFGR seropositivity and driving distance to the nearest US-Mexico border crossing station, whether proximity to a border crossing increased likelihood of taking dogs across the border, and whether distance to the border was associated with seropositivity. Logistic regression was performed to assess relationships between the titre classes and other predictor variables. RESULTS: 12.2% of dogs were seropositive against SFGR. Dogs close to the border were significantly more likely to be taken across the border and to be seropositive. Risk factors that increased seropositivity included owners seeing ticks on the dog (OR = 1.9), being an adult dog, travel to Mexico (OR = 3.0) and living in a rural area (OR = 4.0). There was statistically significant co-exposure to SFGR and Anaplasma spp. CONCLUSION: Surveillance for brown dog tick-vectored pathogens can help identify dogs and people at risk for RMSF. Tick prevention, particularly in dogs, and surveillance of tick-borne pathogens can help prevent the spread of rickettsioses and other diseases in this dynamic border region.

ABSTRACT

BACKGROUND: Diabetes is a leading cause of morbidity and mortality in Mexico and understudied among indigenous populations. This study aimed to determine the prevalence and identify correlates of Type 2 diabetes mellitus (Type 2 DM) and metabolic syndrome (MetS) in a rural, indigenous community in Northwestern Mexico. METHODS: A cross-sectional study was conducted in the community of San Quintin, Baja California, Mexico, among a sample of households. A total of 275 participants (≥18 years old) underwent a questionnaire, physical examination, and serologic test. Prevalence and adjusted odds ratio (AOR), using logistic regression modeling, were estimated with 95% confidence intervals (95% CI). RESULTS: The prevalence of Type 2 DM and MetS was 21.8 and 53.1%, respectively. Mean ± standard deviation (SD) age and body mass index of study participants was 35.8 ± 13.0 years and 28.7 ± 5.6 kg/m2, respectively. Participants were 75% female and 60.7% self-identified as indigenous. Thirty-seven percent of adults had high blood pressure. After controlling for age, higher educational attainment had a protective effect on Type 2 DM (AOR = 0.39; 95% CI 0.20, 0.77). Additionally, the presence of MetS was associated with being female (AOR = 2.27; 95% CI 1.23, 4.14) and having lower educational attainment (AOR = 0.62; 95% CI 0.37, 0.94). CONCLUSIONS: The prevalence of Type 2 DM and MetS was high in this rural and indigenous population, and education was shown to play a critical role. These findings support the need for community-inclusive health-promoting interventions in rural communities.

ABSTRACT

We examined drug-scene familiarity and exposure to gang violence among residents of a migrant farming community in rural Baja California, Mexico. In October 2010, 164 members of a single colonia (community) underwent an interviewer-administered survey to assess 'exposure to gang violence' and 'drug-scene familiarity', as well as other health indicators. Logistic regression was used to identify correlates of exposure to gang violence. Overall, 20% of participants were male, the median age was 27 years, 24% spoke an indigenous language, 42% reported exposure to gang violence and 39% reported drug-scene familiarity. Factors independently associated with exposure to gang violence included being younger (adjusted odds ratio [AOR] =0.80 per 5-year increase, 95% confidence interval [CI]=0.67-0.96), living in the community longer (AOR=1.47 per 5-year increase, 95% CI=1.11-1.72), higher educational attainment (AOR=1.70 per 5-year increase, 95% CI=1.07-1.12) and drug-scene familiarity (AOR=5.10, 95% CI=2.39-10.89). Exposure to gang violence was very common in this community and was associated with drug-scene familiarity, suggesting a close relationship between drugs and gang violence in this rural community. In a region characterised by mass migration from poorer parts of Mexico, where drugs and gangs have not been previously reported, emerging social harms may affect these communities unless interventions are implemented.

OBJECTIVE: Estimate the 2006 HIV prevalence among adults aged 15-49 from the general population and at-risk subgroups in Tijuana, Mexico. METHODS: Demographic data was obtained from the 2005 Mexican census and HIV prevalence data was obtained from reports in the literature. We developed a population-based HIV prevalence model for the overall population and stratified it by gender. Sensitivity analysis consisted of estimating standard errors in the weighted-average point prevalence and calculating partial derivatives of each parameter. RESULTS: HIV prevalence among adults was 0.54% (N = 4347) (range 0.22-0.86% [N = 1750-6944]). This suggests that 0.85% (range 0.39-1.31%) of men and 0.22% (0.04-0.40%) of women could have been HIV-infected in 2006. Men who have sex with men (MSM), followed by female sex workers who are injection drug users (FSW-IDU), FSW-non IDU, female IDU, and male IDU were the most at risk groups of infected individuals. CONCLUSIONS: The number of HIV-infected adults among at-risk subgroups in Tijuana is significant, highlighting the need to design tailored prevention interventions that focus on the specific needs of certain groups. According to our model, as many as 1 in 116 adults could potentially be HIV-infected.

Objetivo: Estimar la prevalencia para 2006 de HIV/sida entre población general de 15 a 49 años de edad y en los grupos de riesgo, en la ciudad de Tijuana, México. Métodos: Se obtuvieron datos demográficos del censo mexicano de 2005 y la prevalencia del VIH, de la literatura. Se construyó un modelo de prevalencia del VIH para la población general y de acuerdo con el género. El análisis de sensibilidad consistió en estimar los errores estándar del promedio-ponderado de la prevalencia del VIH y tomar derivados parciales respecto a cada parámetro. Resultados: La prevalencia del VIH resultó ser de 0.54% (N = 4,347) (rango 0.22-0.86% [N = 1750-6944]). Esto sugiere que 0.85% (rango 0.39-1.31%) de los hombres y 0.22% (rango 0.04-0.40%) de las mujeres podrían ser VIH-positivos. Los hombres que tienen sexo con hombres, las mujeres trabajadoras sexuales usuarias de drogas inyectables (UDI), las mujeres trabajadoras sexuales-no UDI, mujeres- UDI y los hombres-UDI, contribuyeron a las proporciones más elevadas de personas infectadas por el VIH. Conclusiones: El número de adultos VIH-positivos entre subgrupos de riesgo en la población de Tijuana es considerable, lo que denota la necesidad de enfocar las intervenciones de prevención en sus necesidades específicas. El presente modelo estima que hasta uno de cada 116 adultos podría ser VIH-positivo.
