ABSTRACT

Morphofunctional changes in intestinal wall above the obstruction were studied experimentally in 112 rats with low intestinal obstruction. Treatment with antioxidants and lasertherapy was developed. After removal of obstruction this complex permits to eliminate its negative local and general consequences 1.5 times faster than without it. Laparoscopic decompressive colostomy was performed in 65 patients with sub- and decompensated colonic obstruction. The developed therapeutic scheme was included in combined treatment before surgery. There were no lethal outcomes, purulent-inflammatory complications were seen in 24 (39.6%) patients.

ABSTRACT

Long-term results of the developed variant of sphincter-saving operation in distal cancer of the rectum were analyzed. The patients were operated at the age of 50-70 years. Local spread of tumor didn't exceed T2-stage, if the tumor located 3 cm from the serrate line of the anal canal. Inclusion of complete sphincterotomy in perineal stage of operation for most radical surgery and also the complex of technical approaches, eliminating negative consequences of sphincterotomy, was the features of abdominal-anal resection. 69% from the all examined patients survived 5 years and more. Functional outcomes were evaluated. Some results of developed operation allow to recommend this variant for clinical use.

ABSTRACT

The experience with treatment of a most frequent complication after abdomino-anal resection with bringing down the intestine--marginal necrosis of a xenotransplant--is summarized. The expediency of individual approach to the choice of treatment is noted. Active therapeutical tactics is suggested.

ABSTRACT

A retrospective analysis of causes and outcomes of inflammatory complications in the presacral wound after abdomino-anal resection with bringing down for carcinoma of the rectum has shown active treatment of the wound to be necessary. The fractional lavage with the ozonization and active aspiration is highly estimated as a prophylactic and therapeutic method.

ABSTRACT

The expedience of using a portion of the large intestine as a colontransplant which has a two-trunk disloading stoma is substantiated. The operative method was successfully used in 6 cases.

ABSTRACT

The work analyzes the first twelve observations of a through ilioperineal drainage after simultaneous abdomino-perineal extirpation and abdomino-anal resection of the rectum. The authors present convincing data concerning high efficiency of the proposed method of drainage. It has a number of advantages over other methods: it markedly accelerates the healing of the postoperative wound, preventing postoperative complications (suppurations, retroperitoneal phlegmons etc).
