ABSTRACT

An infrequently encountered case of well-differentiated CLOS containing dedifferentiated tumor foci in a 28-year-old Japanese man is reported. The patient died from widespread metastases with a microscopic appearance identical to the anaplastic dedifferentiated lesions of the original tumor. This is, to our knowledge, the first report of a case of CLOS with dedifferentiation at the time of initial surgery. The literature concerning CLOS was reviewed and the diagnostic features of CLOS discussed.

ABSTRACT

A case of a 36-year-old woman with progressive systemic sclerosis (PSS) was reported. The patient had two additional immune-mediated diseases: Hashimoto's thyroiditis and membranous nephropathy. It was implicated that overlapping of systemic lupus erythematosus (SLE) occurred in the course of PSS. Notably, immune complex deposition along the follicular basement lamina of the thyroid was suggested by immunofluorescence and electron microscopy. Although the glomerular lesion was in general accord with membranous nephropathy, it was complicated by occasional crescent formations and necrotic arterial changes, probably resulting from malignant hypertension that appeared during the terminal stage. It was implied that the same circulating antigen-antibody complexes might have been involved in the pathogenesis of Hashimoto's thyroiditis and membranous nephropathy in this case.

ABSTRACT

A case of a 60-yr-old man with biliary cystadenocarcinoma is reported. By needle biopsy of a cystic lesion performed under ultrasonographic guidance, overt malignant tissue could be obtained successfully. Histologic examination on autopsy material revealed that the tumor arose from preexisting benign liver cysts. The tumor showed a feature of well-differentiated, mucin-producing papillary adenocarcinoma in the primary site while, in the metastatic foci, it produced solid cell clusters with a tendency toward the differentiation into squamous cells.

ABSTRACT

Endocardial lesions in acute serum sickness in rabbits were studied with the aid of scanning electron microscopy (SEM). Prominent inflammation developed most often in the ventricular (pocket) side of the mitral valve and was confined to a hollowed area formed between valvular ridges. The early lesion consisted of swelling of endothelial cells with monocytic attachments. Subsequently, frequent endothelial perforations occurred, through which a number of monocytes emigrated into the subendothelial space. Focal or diffuse endothelial desquamation then followed. SEM revealed that the denuded basement membrane also had many perforations enabling monocytic emigration into the deeper valve layer. The accumulated monocytes and proliferating mesenchymal cells produced a granulomatous focus in the valvular tissue in which some monocytes were observed to convert to Anitschkow cells. Infiltration by polymorphs was minimal. Deposition of fibrin occurred in the more severe valvulitic lesions, but no verrucous vegetation formation was observed. The inflammatory process regressed rapidly, leaving focal areas of fibrosis. A possible pathogenesis is discussed in connection with the pathophysiologic conditions of the mitral valve pocket.

ABSTRACT

An autopsy report of a patient with a hugh biliary cystadenocarcinoma arising in the right lobe of the liver is presented. The tumor had been though to be benign, but peritoneal carcinomatosis developed 18 months after the operation. Autopsy revealed marked peritoneal dissemination of mucinous adenocarcinoma, apparently a result of peritoneal seeding of hepatic cystadenocarcinoma, which was presumed to be triggered by cyst aspiration performed during the operation. Experience indicates that surgical procedures on cystic liver disease should be carried out carefully, considering the possible existence of malignancy.
