OBJECTIVE: To analyze the cause of infertility in a patient with polycystic kidney disease. SETTING: General hospital. PATIENT: A 32-year-old man known with adult polycystic kidney disease, complaining of infertility and testicular pain. INTERVENTIONS: Testicular biopsy; imaging. RESULTS: Examination revealed bilateral severe congestion of the epididymis. The semen contained no spermatozoa and practically no fructose. Testicular biopsy, however, showed normal spermatogenesis. Transrectal ultrasonography and computer tomography revealed severe congestion and extensive cystic changes in the seminal vesicles. CONCLUSION: The dilated cysts in the seminal vesicles had led to obstructive azoospermia, probably through compression of the ductal system.

Radiologic intervention procedures are being introduced more and more in larger centers. The role of the radiologist in small hospitals has to be established. The results of 3 years of experience with radiologic intervention procedures in a 280-bed small center hospital are reported. Drainage procedures and percutaneous transluminal angioplasty have been employed successfully but, as yet, the experiences with embolization have been disappointing.

Opacification of collateral pathways other than the central channels is very rare in lumbar epidural venography. Two cases of opacification of the inferior mesenteric vein following extravasation of contrast medium at the tip of the lateral sacral vein catheter are reported. One case is presented in which filling of normal parametrial venous plexuses and the left ovarian vein occurred as a consequence of incompetent or absent valves in the internal iliac vein. The literature containing comparable collateral flow patterns in disease is reviewed. The significance of the phlebographic features in our cases is discussed.

By roentgenographic methods alone no certain differentiation can be made between primary and secondary gastrointestinal lymphomas, nor can the histological characteristics and the cell type be derived from the roentgenological pattern. Only in proximal small intestinal lymphomas with malabsorption does a diffuse malignant histiocytosis seem to exist. The roentgenographic presentation of a gastrointestinal lymphomatous lesion is dependent on the direction of growth of the tumor cells in the bowel wall, and is therefore accidental.
