ABSTRACT

A comprehensive survey, including clinical, X-ray, thermographic, immunological, and radioimmunological studies, was made in 56 patients with lung cancer detected on prophylactic fluorographic surveys and erroneously referred to the Lvov Regional Clinical and Diagnostic Phthisiopulmonological Center for suspected active pulmonary tuberculosis. Routine clinical and laboratory studies provided preliminary information in only 26.8% of cases and 73.2% needed additional examination by using currently available laboratory and instrumental studies. Of the most informative value for diagnosing malignancy is radioimmunocompetitive assay of serum levels of cancer embryonic antigen.

ABSTRACT

Computerized devices were used for evaluation of the state of pulmonary ventilation and central hemodynamics in 32 patients with pulmonary tuberculosis depending on the spread of the specific process. Patients with limited changes in the lungs, showed disorders of bronchial patency and compensatory increase of organic ejection of the right ventricle. In the presence of spread changes in the lungs the above mentioned changes were supplemented by restrictive ventilation disorders with a reduction of the contractile myocardial capacity. These functional changes should be considered in the treatment of patients with pulmonary tuberculosis.

ABSTRACT

As a result of a study of 262 patients with chronic destructive tuberculosis of the lungs the authors revealed that these patients showed personality disorders that impede adequate treatment. Psychotherapy carried out in 5.2% of patients aimed at correction of personality disorders enabled to increase treatment efficacy of chronic destructive pulmonary tuberculosis.

ABSTRACT

The criteria of differential administration of thymalin in the combined management of patients with destructive pulmonary tuberculosis were defined on the basis of immunologic studies. A ++clinico-roentgenologic and immunologic evaluation of in-patient treatment of 60 cases demonstrated that a differential administration of thymalin in combination with chemotherapeutic drugs greatly shortened the stay at a hospital and increased the efficacy of treatment. Besides, more evident normalizing influence of the proposed option of therapy on the recovery of the immune system links is noted.

ABSTRACT

The antiproteolytic activity of the blood serum was examined in the course of hospital treatment of 128 adolescents with prevailingly torpid forms of pulmonary tuberculosis and in 19 adolescents with tubercular infection (control). A direct correlation has been found between the degree of clinico-roentgenological manifestations of the disease and level of serum alpha 1-inhibitor of proteinases as well as between the efficacy of treatment and normalization of the antiproteolytic activity of the blood of the patients. It proposed to use the determination of alpha 1-inhibitor of proteinases as an additional criterion of evaluation of the clinical course and outcomes of the disease in adolescents.

ABSTRACT

The effect of various doses of rifampicin on intensity of rosette formation was studied, and the immunological criteria for selective use of the drug in combined specific chemotherapy of extended destructive tuberculosis of the lungs were determined. It was shown that differential use of rifampicin with an account of its administration multiplicity and route promoted a decrease in the hospitalization terms and increased rate of discontinuation of tubercle bacilli isolation and cavern healing. The clinicoroentgenological findings were confirmed by the immunological tests characterizing various elements of the immunity system in patients.
