ABSTRACT

Training residents is a task that is both exciting and demanding for which tutors need appropriate training and tools. One of the tools that have been developed is the structured interview, which is based on providing residents with feedback about the learning process according to pre-established goals. Structured feedback focused on the learner is an adult learning technique characterized by the student's self-reflection, preparation for the interviews, and relationship of trust between the tutor and the resident. Periodic structured interviews using a script are very useful. But there is something more. An interview poses multiple challenges for the tutor and for the resident, because both parts must continuously adapt. It is important to create a climate of empathy and ensure that the interview motivates, stimulates, and inspires the resident.

ABSTRACT

As women in western countries delay childbearing, it has been hypothesized that the incidence of breast cancer diagnosed during pregnancy will increase. Breast carcinoma during pregnancy(BCP) put the health of the mother in conflict with that of the fetus. The aim is to give optimal treatment to the mother to maximise the chances of survival, whilst minimising the risk of harm of the fetus. Few breast surgeons or oncologist develop expertise in this area owing the rarity of the association. We report the epidemiology, pathology, clinical picture, therapeutic management and fetal outcome of pregnant women with breast cancer treated in our institution.

ABSTRACT

Breast Magnetic Resonance (BMR) imaging is a useful tool in the evaluation of breast cancer before surgical treatment. BMR imaging plays an important role in the evaluation of the extension of the malignant lesions, and the study of multifocality and multicentricity. BMR may have a role in the detection of synchronous contralateral breast cancer that is occult to conventional imaging methods (mammography and ultrasonography). In this study we review 13 series of different authors in which they have used BMR in the evaluation of the contralateral breast in patients with newly diagnosed breast cancer. Two thousand five hundred and eleven patients were evaluated with BMR and 123 contralateral cancers, that were occult to conventional methods, were detected with this technique (4,9 %). Therefore, BMR imaging of the breast is useful as a complementary tool because of its high sensitivity in local staging of a breast cancer and its ability in the detection of synchronous contralateral breast cancer in patients with newly diagnosed breast cancer.

ABSTRACT

La resonancia magnética de mama (RMM) se emplea cada vez más en laevaluación preoperatoria del cáncer de mama, ya que ofrece informaciónsobre el tamaño de la lesión maligna, así como sobre multifocalidad omulticentricidad. Además puede ser de utilidad para detectar cánceresde mama contralaterales que pasan desapercibidos con las técnicas deestudio habituales (mamografía y ecografía). En este artículo revisamos13 series de diferentes autores en las que se ha empleado la RMM en laevaluación de la mama contralateral tras un diagnóstico previo de cáncerde mama. Sobre un total de 2511 pacientes se detectaron medianteRMM 123 cánceres contralaterales, no sospechados por otras técnicas(4,9 %). Por lo tanto, la RMM es útil como técnica complementaria, yaque además de aportar datos sobre el tamaño y extensión del cáncerde mama, puede detectar cánceres contralaterales no sospechadosmediante otras técnicas (AU)

La frecuencia global oscila entre el 0.2 al 3.8% del total delos tumores malignos de la mama. ABSTRACT

Our objective was to evaluate our experience with the Advanced Breast Biopsy Instrumentation system (ABBI) in non-palpable breast lesions in a prospective study from July 1998 to November 2000. The ABBI system was included in a protocol for BIRADS 4 non-palpable, small (<15 mm) breast lesions. Digital radiographs of both specimen and biopsy cavity were obtained to validate the procedure. A total of 255 ABBI biopsies were performed in 254 patients. In 251 cases the lesions were successfully removed (98.4%). Mammographic lesions consisted of 176 cases of microcalcifications (69%), 51 cases of architectural distortions (20%) and 28 cases of nodules (11%). Seventy-two carcinomas were diagnosed (28.2%). Affected margins were found in 41 cases (56.9%). Residual tumour was seen in 31 patients (43%). Seventeen borderline results and 33 benign architectural distortions obviated further procedures. The complication rate in 10 cases was as follows: 3 wound infections; 4 haematomas; and 3 vasovagal reactions. The main utility of the ABBI system is to allow a reliable diagnosis in complex lesions, such as small clusters of microcalcifications and especially architectural distortions. Surgery can be avoided for borderline cases if the lesion is completely removed and free margins are obtained in the pathology study. Therapeutic use is controversial and can be applied only in selected cases.

ABSTRACT

We report a case of mucinous breast carcinoma whose mammographic features consisted of a suspicious cluster of microcalcifications as the unique sign of malignancy. The lesion was non-palpable and measured approximately 2 cm at the greatest diameter. The microcalcifications showed several morphologies: round, pleomorphic, and fine shapes were identified. The total number of microcalcifications was > 30 and the number per square centimeter varied from 10 to 20. The histological calcifications showed good correlation with the mammographic ones and were localized predominantly at the periphery of the tumor inside ducts with ductal carcinoma in situ or in the acellular mucin. Two types were observed: psammomatous and gross-irregular calcifications. To our knowledge, only one case with similar findings has been reported previously.
