ABSTRACT

Experience of organization of the surgical care in the military hospital to 438 wounded in extremities during armed conflict in Republic of Chechnya is generalized. Maximum reduction of stages of medical evacuation of the wounded in extremities, approaching of the qualified and urgent specialized surgical care directly to the region of battle actions, use of opportunities for it one-moment rendering corresponded to principles of the modern military-medical doctrine. Due to realization of the requirements of the doctrine life of many wounded ++ was saved, terms of treatment, medical and social rehabilitation are reduced. Besides lethality, treatment cost and numbers of transferring to the reserve from the Armed Forces were reduced.

ABSTRACT

An experience with treatment of 108 patients with extensive deep defects of extremity tissues was generalized. Such wounds made up 42.3% of the total number of patients with wounds of extremities. The introduction of microsurgical autotransplantation of tissues allowed to shorten the terms of medical and social rehabilitation, to reduce invalidism of the patients and to lower the cost of treatment.

ABSTRACT

Based upon results of topographo-anatomical research and personal experiences with transplantation of 91 tissue complexes on radial vascular bundle the authors made an attempt to systematize the available data concerning this source of tissues. The substantiation of using the tissue complexes consisting of several fascio-cutaneous (fascio-cutaneous and muscular) flaps at different levels of the vascular pedicle as well as superlong deltoid-radial flap on two vascular pedicles are first given.

ABSTRACT

An analysis of islet flaps of the forearm used in the performing of complex reconstructive operations on the hand in 37 patients is presented. A substantial shortcoming of transplantation of the radial and elbow flaps is a possible development of disturbance of blood circulation in the hand tissues which can require emergent plasty of the transected artery. In transplantation of the posterior flap of the forearm the blood circulation in the hand tissues was not disturbed.

ABSTRACT

New thermosensitive mutants of the yeast Saccharomyces cerevisiae which block the secretion of periplasmic enzymes at restriction temperature have been obtained. These mutants accumulate active low molecular weight and mature invertase species in the cell; the buoyant density of the cells in a Percoll gradient is higher than that in the wild strain cells. The mutant cells transferred to permissive temperature (25 degrees C) in the absence of protein synthesis can secrete some amount of accumulated invertase. It was found that the secretory defects of conditional mutants do not affect the activity of cytoplasmic enzymes (e.g., alcohol dehydrogenase) or the level of total protein synthesis and glycosylation and do not induce non-specific disturbances in energy metabolism and plasma membrane functions at restriction temperature. Some strains of new secretory mutants revealed uncoupled defective secretion of periplasmic enzymes and intrinsic membrane proteins (proline permease). The possibility of branching of the secretory pathway for periplasmic enzymes and cytoplasmic membrane proteins is discussed.

ABSTRACT

A detailed characteristic of the ways of using fascio-cutaneous, osteo-cutaneous and muscular-cutaneous flaps disposed in the basin of the radial vascular bundle is given on the basis of an experience with 18 operations. Non-free plasty by an island flap on a peripheral vascular pedicle was used in 13 patients. Free plasty with microvascular anastomoses was used in 4 patients. In 16 of the 17 patients a complete take of the flap took place.

ABSTRACT

It was shown that medium proteins possess a double regulatory effect on the synthesis of proteinase secreted by Aspergillus awamori 78-2. These proteins are nitrous nutrition that provides for the utilization of the proteolysis products in the course of their gradual formation, which excludes their inhibiting action on the enzyme synthesis. On the other hand, medium proteins possess the ability to induce secreted proteinase synthesis. It is assumed that the concept "enzyme synthesis inducer" can be applied to any one of its substrates, which stimulates protein synthesis at any level and via any mechanism.

ABSTRACT

It was shown that under conditions of regulation of secreted enzyme synthesis by its concentration in microorganisms, the repression of synthesis of the enzyme protein polypeptide chain and the concomitant partial repression of labelled amino acid transport into the cells occur. The regulatory effect can be exerted in the absence of transcription as well. These effects are not realized at the post-transcription or post-translation levels.
