ABSTRACT

A simple implantation technique for multiple bypass of the ascending aorta with cervico-encephalic and infradiaphragmatic targets is reported. A conduit made by termino-terminal suture of two bifurcated grafts is anastomosed latero-laterally to the ascending aorta. The procedure is easy to perform and provides a suitable trajectory to the grafts.

ABSTRACT

The authors present a series of 98 aorta-coronary bypass procedures without the help of Extracorporeal Circulation. Indications and technique are detailed. There was no hospital mortality and short-term results are satisfactory. This technique, feasible in about 10% of all cases, should be considered especially in old patients, in case of chronic renal insufficiency and in redo surgery.

ABSTRACT

In 88 patients with pectus deformities radiologic chest indices and routine pulmonary function tests were measured before and 1 to 20 years after corrective surgery. A combination of anteroposterior indices at the upper and lower level of the chest were investigated to quantitate and to discriminate the different pectus deformities. The study comprised four groups: pectus excavatum, pectus carinatum, pectus deformatum and pectus excavatum with scoliosis. These indices were also assessed in 250 healthy males and females. Generally, several indices showed significant and discriminative changes in the different patient groups and improved again after surgery. Preoperative lung function was decreased in pectus excavatum only. In all groups lung function worsened after surgery. A stepwise discriminant analysis performed on the large group with pectus excavatum indicated that postoperative lung function was decreased if the preoperative value of FEV1 or VC was more than about 75 percent predicted and vice versa, but that it was not related to other factors such as radiologic indices, age at operation or time since operation.

ABSTRACT

In 24 subjects with pectus excavatum we evaluated whether the previously detected unfavourable effects of corrective surgery on the ventilatory capacity were attributable to pulmonary or to chest wall factors. We found that 12.2 +/- 3.7 yrs postoperatively (i.e. at the age of 23.3 +/- 5.4 yrs) the vital capacity was decreased from 89 +/- 10% predicted (pred) preoperatively to 64 +/- 6% pred (p less than 0.001) and forced expiratory volume in one second from 88 +/- 17 to 66 +/- 11% pred (p less than 0.001). At total lung capacity (TLC; 69 +/- 5% pred) we found an obvious reduction in transpulmonary pressure (59 +/- 23% pred) and in transdiaphragmatic pressure (30 +/- 17 cmH2O) postoperatively. This indicated an extrapulmonary cause of the restrictive defect, attributable to abnormal chest wall mechanics secondary to the extensive surgery on the sternum and parasternal zones.

ABSTRACT

1968-1972 in five districts of DR Bulgaria 666 persons with Taeniarhynchus saginatus infection were registered, 64% of these tapeworm carriers lived in the country, 36% were townspeople. The incidence of infection in women was twice as high as in man. There were differences in incidence in jobs, 21.6% in farm workers, 20.2% in industrial workers, 13.8% in workers in the tobacco industry, 14.2% in housewives, 12.1% in pulls, 9.2% in office workers and 8.1% in pensioners carried tapeworms. Only 1.4% of animal breeders were infected with T. saginatus. The incidence of infestation increased in relation to the age. - In the Kardschali district, the tobacco farmers with a disease incidence of 34.8% represented a large part of the total incidence in the country. The private keeping of animals is still widespread in the country. The disease incidence increases between the age groups 20-40 years. The reason for the above is people eating increased uncontrolled animals. Individual food preparation habits and ignorance of the source of meat also account for spread of infection of people. As a trial three prophylactic methods were tested to establish a reduction of this disease.
