ABSTRACT

Abdominal volvulus represents a critical condition characterized by the abnormal twisting of the GI tract, potentially leading to obstruction and vascular compromise. Prompt recognition and appropriate management are essential to prevent complications and improve patient outcomes. This comprehensive review examines the anatomy, pathophysiology, clinical presentation, and diagnostic evaluation of, and management strategies for abdominal volvulus. Non-operative techniques, including detorsion and decompression, as well as surgical interventions, such as laparoscopic and open approaches, are discussed. Additionally, the importance of multidisciplinary collaboration and postoperative care is emphasized. Despite significant advancements, unresolved issues remain, necessitating further research to refine diagnostic and therapeutic approaches. Future directions, including exploring emerging technologies, offer promise for enhancing the management of this challenging condition. Overall, this review provides clinicians with valuable insights into the optimal management of abdominal volvulus, aiming to improve patient outcomes and enhance clinical practice.

ABSTRACT

Inguinal hernias present a significant healthcare burden globally, necessitating effective surgical management. This comprehensive review evaluates two primary surgical techniques for managing bilateral inguinal hernias: bilateral open inguinal hernia and Rives-Stoppa repair. This review aims to provide insights into optimal surgical approaches through a comparative analysis of these techniques, including examining advantages, disadvantages, outcomes, and factors influencing technique selection. Bilateral open inguinal hernia repair offers simplicity and familiarity, while Rives-Stoppa repair may provide advantages such as reduced recurrence rates and postoperative complications. The findings underscore the importance of considering patient-specific factors, surgeon expertise, and hospital resources when selecting the optimal approach. Further research is warranted to conduct long-term comparative studies and explore innovations in surgical techniques and materials, ultimately enhancing patient outcomes and advancing inguinal hernia repair practices.

ABSTRACT

Robotic innovations in breast surgery have ushered in a new era of precision, safety, and patient-centred care. This comprehensive review explores the multifaceted realm of robotic breast surgery, from preoperative planning to postoperative outcomes, learning curves for surgeons, and the implications for healthcare policies. We examine the ethical considerations, cost-effectiveness, and future directions, including integrating artificial intelligence and telesurgery. Key findings reveal that robotic systems provide improved surgical precision, reduced complications, and enhanced patient satisfaction. Ethical concerns encompass informed consent, resource allocation, and equitable access. The future of breast surgery lies in continued research and development, ensuring that robotics becomes a standard of care accessible to all patients. This technology is reshaping breast surgery and offering new possibilities for minimally invasive, patient-centred care, ultimately redefining the standards of care in this critical field of medicine.

ABSTRACT

The treatment of postoperative pain is a crucial component of postoperative patient care. To reduce discomfort, encourage early mobilisation, reduce complications, and improve overall recovery, effective pain management is crucial. To maximise pain relief and reduce the need for opioids and their accompanying adverse effects, a multimodal analgesic strategy, combining several drugs and procedures, is frequently used. Opioids continue to be a crucial part of postoperative pain treatment for moderate to severe pain; however, due to potential side effects and dependency concerns, their usage should be carefully controlled. Non-opioid analgesics, such as non-steroidal anti-inflammatory medications (NSAIDs) and paracetamol, are widely used as a component of a multimodal regimen and play an important role in pain management. Postoperative pain management ultimately aims to deliver efficient analgesia, enhance patient comfort, and foster a speedy recovery while minimising the dangers and adverse effects connected with pain medicines. To create a thorough and unique pain management strategy for each patient, close coordination between the surgical team, anesthesiologists, and pain management specialists is essential.

ABSTRACT

This comprehensive case report meticulously outlines the clinical manifestation, diagnostic trajectory, surgical intervention, pathology, chemotherapy, and patient follow-up in a challenging instance of a giant exophytic gastrointestinal stromal tumor (GIST) emerging from the antral part of the stomach in a 56-year-old male patient. Over the course of a year, the patient presented with symptoms including abdominal distension, a progressively enlarging lump, constipation, and abdominal fullness. Our diagnostic strategy, anchored by ultrasonography and contrast-enhanced computed tomography (CECT), yielded crucial insights into the tumor's precise dimensions and anatomical location. The subsequent surgical planning and execution were pivotal, entailing a meticulous dissection of the cystic mass from adjacent structures to ensure negative margins. Histopathological scrutiny of frozen sections from the lymph nodes and excised antral margins conclusively confirmed the absence of malignancy, facilitating primary closure. This case report underscores the decisive role of accurate diagnostic imaging in shaping surgical strategies and emphasizes the need for a multidisciplinary approach to managing GISTs. Beyond the surgical realm, the case highlights the significance of tyrosine kinase inhibitors (TKIs), particularly imatinib, in the treatment paradigm. Additionally, the report sheds light on ongoing research endeavors to refine treatment modalities in GISTs.

ABSTRACT

Hydrocele represents a commonly encountered pathological condition characterized by the development of scrotal swelling. In contrast, spermatic cord hydrocele is a relatively rare condition that arises from the abnormal closure of the processus vaginalis. This condition entails the accumulation of fluid within the spermatic cord, and although hydrocele itself is a frequently observed ailment affecting roughly 100 out of 100,000 men, encysted hydrocele of the spermatic cord is a seldom-seen variant. Furthermore, it is noteworthy that spermatic cord hydrocele tends to be more prevalent in the pediatric population compared to adults. In our clinical practice, a patient presented with swelling localized to the right hemi-scrotum and was initially diagnosed as suffering from a chronic right-sided hydrocele of the tunica vaginalis. However, during the surgical intervention, the true nature of the condition became evident, as it was determined to be an encysted hydrocele of the cord. This case report chronicles the diagnostic journey surrounding hydrocele, highlighting the distinction in surgical management between encysted hydrocele and vaginal hydrocele of the testis.

ABSTRACT

Tropical chronic pancreatitis represents a variant of chronic calcific non-alcoholic pancreatitis, typically found in adolescents and young adults, predominantly in developing nations. This condition usually presents as a classic triad of recurrent and severe abdominal pain, diabetes, and steatorrhea. As the disease progresses and diabetes develops, it is called fibrocalculous pancreatic diabetes. A defining characteristic of this ailment is the presence of pancreatic calculi, leading to duct dilation. Key features of this condition include an early onset in youth, intraductal calculi, an aggressive disease course, and a heightened susceptibility to pancreatic cancer. Diagnostic tools such as ultrasound, CT, magnetic resonance cholangiopancreatography, and endoscopic retrograde cholangiopancreatography aid in identifying the disease. Timely diagnosis and treatment significantly reduce mortality and morbidity. Our patient, a young female, presented solely with recurrent episodes of abdominal pain resembling pancreatitis, along with a normal biological profile and an absence of readily apparent symptoms. She received a diagnosis of tropical chronic pancreatitis and underwent the Partington-Rochelle procedure for surgical decompression of the main pancreatic duct.
