ABSTRACT

PURPOSE: Since saliva protects the oesophageal and oral mucosa against hydrogen ions, the aim of the study was to establish the relationship between the secretion of gastric acid and salivary bicarbonate. MATERIAL AND METHODS: The study involved 43 Helicobacter pylori positive duodenal ulcer patients receiving: 1. omeprazole alone (O), 2. omeprazole and amoxicillin (OA) or 3. omeprazole, amoxicillin and tinidazole (OAT). In each study group the examination was performed twice, before and at the end of a two-week treatment, both under basal conditions and during a gastric secretory test with pentagastrin. Concentrations of gastric hydrogen ions and salivary bicarbonate were evaluated by the titration method. RESULTS: In all therapeutic groups analysed separately, the secretion of gastric acid as well as salivary bicarbonate decreased at the end of the treatment, however only in OA and OAT groups the differences in bicarbonate reached statistical significance. As the changes in the concentration and output of both salivary bicarbonate and gastric acid had the same direction, the three therapeutic groups (O, OA, OAT) were subjected to combined analysis. It showed that under basal conditions and during stimulation with a gastric catheter or catheter and pentagastrin, bicarbonate concentration and output were higher before than at the end of the treatment. However, no direct correlation between gastric acid secretion and salivary bicarbonate was found in groups subjected to either separate or combined analysis. CONCLUSIONS: The results of our study provide evidence for the partial involvement of hydrogen ions of gastric origin in the regulation of salivary bicarbonate secretion in duodenal ulcer patients.

ABSTRACT

Adenosine deaminase activity was studied in endoscopically taken slices from gastric mucosa in patient after partial gastric resection performed due to complicated duodenal ulcer, and currently with peptic ulcer in the stump. The samples of gastric mucosa were taken before and after 6 weeks of treatment with ranitidine, 150 mg twice daily, at a distance within 2 cm and greater than 2 cm from the ulcer crater. Adenosine deaminase activity was measured in mucosa homogenates by determination of ammonia liberated from substrate. It was found that therapy with ranitidine was accompanied by an increase in enzyme activity in the mucosa surrounding unhealed stump ulcers, while no changes were noted in mucosa around healed stump ulcers. A possible role of mucosal adenosine deaminase activity in stump ulcer healing is postulated.

ABSTRACT

Adenosine deaminase activity was studied in gastric corpus mucosa close to an ulcer crater. It was found that 6 weeks of therapy with ranitidine was accompanied by a decrease in enzyme activity in the mucosa around healed ulcers and an increase around those which failed to heal. The different activities of adenosine deaminase in the vicinity of healed and unhealed ulcers may indicate its possible role in peptic ulcer healing.

ABSTRACT

The examinations relate to 206 patients suffering from a bioptically confirmed sarcoidosis, observed during 1959-1974. The observation period amounted to 2-15 years. The indications of the corticosteroid-therapy ran as follows: 1. in cases of a lymph node sarcoidosis without respiratory disorders, after a 2 years' observation without a regression. 2. in cases with pulmonary alterations already after 3-6 months without a regression. 3. in all cases of an extended pulmonary sarcoidosis. In the first group the sarcoidosis developed spontaneously in 65.2%, after a treatment in 15.7%. In 19.1% there was an unfavourable course, even in treated cases (2.2%). In the second group there was a spontaneous regression in 35.5%, after a treatment in 57%. In 10% the sarcoidosis changed over into the chronic form. The treatment failed in about 3%. In the third group we did not observe any spontaneous regression, after the corticosteroid-therapy a regression appeared in 35.7%. The condition remained unchanged in 64.3%. Totally there was a spontaneous regressions in 44.2% of the patients. Good results after the corticosteroid-therapy were attained in 36.4%, and the rest of about 20% developed to irreversible forms of the disease.

ABSTRACT

In order to achieve good results with the corticosteroid-therapy in not spontaneously regredient sarcoidoses a longterm treatment (12-24 months) by considerable corticosteroid-doses proves to be necessary; thereby the problem of side-effects by steroids will arise, especially in young women frequently developing a Cushing-syndrome. By our endeavour to prevent this unpleasant phaenomenon we carried through an alternating attempt with two variant kinds of intermittent treatment. The first method relates to 4 successive days in the week (as it is proposed by P. Krakowka) and the second procedure relates to each 2nd day. The second treatment has proved to be the better one. Side-effects appeared in 31. 9 per cent in case of a daily treatment by steroids, in 27.3 per cent after the application of the method of 4 days a week, and in no one case after an application each second day. The results of the treatment by corticosteroids are equal in all the three groups.

ABSTRACT

The numbers of patients registered in chest clinics and of those treated in hospitals or sanatoria enable to calculate the index of sarcoidosis: 7.1/100000 inhabitants in 1973 and 7.7/100000 in 1974. The comparison with the data of the years 1963, 1967, 1968 and 1969 shows an increase of sarcoidosis. Highest frequency has been observed among the age group 30--40 years. Women are slightly predominating (56 p.c.).

ABSTRACT

The data on the epidemiologic situation of sarcoidosis from 24 countries of Europe have been reviewed. The new facts seem to demonstrate that the differences between the frequency of this disease in the north and south are not real. The actual situation is dependent on the general knowledge of this disease and on the extent and intensity of the active detection of its asymptomatic stage. A new prospective cooperative study of the yearly incidence of all forms of sarcoidosis in the total population of at least some European countries would be desirable.
