ABSTRACT

OBJECTIVE: to evaluate the patient safety climate in Primary Health Care from the perspective of nurses working in the services. METHOD: a quantitative and cross-sectional study conducted with 148 nurses from a municipality in the state of São Paulo. The Brazilian version of the Primary Care Safety Questionnaire Survey and personal, professional, and organizational performance variables (intention to stay at work, job satisfaction, care quality, and frequency of incidents) were used. Parametric and non-parametric comparison tests and Spearman's correlation coefficient were performed, considering a 5% significance level. RESULTS: the safety climate was positive, varying from 4.52 to 5.33 and differing across districts for workload (p=0.0214) and leadership (p=0.0129). The safety climate professional variables and dimensions differed in relation to the frequency of incidents. Teamwork and safety and learning system were strongly correlated with job satisfaction and moderately with perceived care quality. CONCLUSION: teamwork and safety and learning system stood out for their positive correlations with job satisfaction and care quality. A positive safety climate favors the involvement of Primary Care nurses to develop improvement plans aligned with the National Patient Safety Program. BACKGROUND: (1) The safety climate is perceived differently across health districts. (2) There is a correlation between the climate dimensions and professional satisfaction. (3) Workload and leadership exert an influence on the safety climate perception. (4) There is a relationship between the safety climate and reporting of care-related incidents. (5) The safety climate is perceived differently among nurses regarding their role.

Objetivo: evaluar el clima de seguridad del paciente en la Atención Primaria de la Salud, desde la perspectiva de los enfermeros de los servicios. Método: estudio cuantitativo y transversal, con 148 enfermeros de un municipio del Estado de São Paulo. Se utilizó la versión brasileña del Primary Care Safety Questionnaire Survey y variables personales, profesionales y de desempeño organizacional (intención de permanecer en el empleo, satisfacción laboral, calidad de la atención y frecuencia de incidentes). Se realizaron pruebas de comparación paramétricas y no paramétricas y coeficiente de correlación de Spearman, considerando un nivel de significación del 5%. Resultados: el clima de seguridad fue positivo, osciló entre 4,52 y 5,33; difirió entre distritos en carga de trabajo (p=0,0214) y liderazgo (p=0,0129). Las variables profesionales y dimensiones del clima de seguridad difirieron en la frecuencia de incidentes. El trabajo en equipo, el sistema de seguridad y el aprendizaje tuvieron una correlación alta con la satisfacción laboral y una moderada con la percepción de la calidad de la atención. Conclusión: el trabajo en equipo y el sistema de seguridad y aprendizaje se destacaron por su correlación positiva con la satisfacción laboral y la calidad de la atención. Un clima de seguridad positivo favorece la participación de los enfermeros de la atención primaria en el desarrollo de planes de mejora que sigan las indicaciones del Programa Nacional de Seguridad del Paciente.


Objetivo: avaliar o clima de segurança do paciente na Atenção Primária à Saúde, sob a perspectiva dos enfermeiros dos serviços. Método: estudo quantitativo e transversal, com 148 enfermeiros de um município do Estado de São Paulo. Foram utilizadas a versão brasileira do Primary Care Safety Questionnaire Survey e variáveis pessoais, profissionais e desempenho organizacional (intenção permanecer no trabalho, satisfação no trabalho, qualidade do cuidado e frequência de incidentes). Realizaram-se testes de comparação paramétricos, não paramétricos e coeficiente de correlação de Resultados: o clima de segurança foi positivo, com variação de 4,52 a 5,33; diferiu entre distritos para carga de trabalho (p=0,0214) e liderança (p=0,0129). As variáveis profissionais e dimensões do clima de segurança diferiram em relação à frequência de incidentes. Trabalho em equipe, sistema de segurança e aprendizagem foram fortemente correlacionados à satisfação no trabalho e moderadamente com percepção da qualidade do cuidado. Conclusão: trabalho em equipe e sistema de segurança e aprendizagem se destacaram pela correlação positiva com satisfação no trabalho e qualidade do cuidado. O clima de segurança positivo favorece o envolvimento dos enfermeiros da atenção primária para desenvolver planos de melhorias alinhados ao Programa Nacional de Segurança do Paciente.

ABSTRACT

Resumo Com o objetivo de compreender o modo como mulheres transexuais têm sido atendidas em instituições do Sistema Único de Saúde, o estudo adotou o delineamento qualitativo. A análise de conteúdo temática de entrevistas e de grupo focal resultou em três categorias: utilização de serviços de saúde por mulheres trans; violência na Atenção à Saúde; falta de atendimento especializado. As participantes referiram não possuir vínculos com a Atenção Primária e buscar atendimento no ambulatório vinculado ao hospital de ensino e em pronto atendimento. Casos de violência institucional, negligência e preconceito permeiam suas experiências. Atendimento humanizado; credenciamento do hospital para o processo transexualizador; investimento no bem-estar do trabalhador; criação de protocolos de atendimento e canais de denúncia foram sugeridos. A equidade e a atenção integral requerem ampliação e qualificação dos serviços. Investimentos em Educação Permanente são essenciais. (AU)


Resumen Con el objetivo de comprender la forma en que las mujeres transexuales han sido atendidas en instituciones del Sistema Único de Salud, el estudio adoptó la delineación cualitativa. El análisis de contenido temático de entrevistas y de grupo focal resultó en tres categorías: Utilización de servicios de salud por mujeres trans; Violencia en la atención de la salud; Falta de atención especializada. Las participantes refirieron que no tenían vínculos con la atención primaria y que buscaban atención en el ambulatorio vinculado al hospital escuela y en urgencias. Casos de violencia institucional, negligencia y prejuicios atraviesan sus experiencias. Se sugirieron la atención humanizada, la acreditación del hospital para el proceso de transexualidad, la inversión en el bienestar del trabajador y la creación de protocolos de atención y canales de denuncia. La equidad y la atención integral requieren aplicación y calificación de los servicios. Son esenciales inversiones en educación permanente. (AU)


Abstract Aiming to understand how transgender women have been served in institutions of the Unified Health System, the study adopted the qualitative design. The thematic content analysis of interviews and focus group resulted in three categories: Use of health services by trans women; Violence in health care; Lack of specialized care. The participants reported having no links with primary care, seeking care in the outpatient clinic linked to the teaching hospital and in emergency care. Cases of institutional violence, negligence, and prejudice permeate their experiences. Humanized care; accreditation of the hospital for the transsexualization process; investment in the worker's well-being; creation of care protocols and complaint channels were suggested. Equity and integral attention require the expansion and qualification of services. Investments in permanent education are essential. (AU)

Resumo Introdução o estado nutricional e seus fatores associados assume uma importante função na saúde dos idosos. Objetivo Identificar os fatores associados ao estado nutricional dos idosos da atenção primária à saúde da cidade de Maringá, Paraná. Método Estudo transversal, do qual fizeram parte 654 idosos usuários das Unidades Básicas de Saúde. Como instrumentos foram utilizados um questionário sociodemográfico, o Mini Nutritional Assessment, o Questionário Internacional de Atividade Física. A análise de dados foi conduzida por meio dos testes de Kolmogorov-Smirnov, "U" de Mann-Whitney, qui-quadrado e regressão logística binária (p < 0,05). Resultados Os resultados evidenciaram maior proporção de idosos com percepção de saúde boa, que tomam menos medicamentos, não tiveram quedas e com nível muito ativo/ativo com o estado nutricional normal. Verificou-se que idosos com estado nutricional normal apresentaram maior circunferência de panturrilha (p = 0,01). Destaca-se que idosos com melhor percepção de saúde, maior nível de atividade física e circunferência de panturrilha, nível mais alto de escolaridade e renda mensal de 2,1 a 3 salários mínimos têm mais chances de apresentar estado nutricional normal. Conclusão Concluiu-se que o estado nutricional normal em idosos está associado ao maior nível de escolaridade, maior poder aquisitivo, melhor percepção de saúde e à prática regular de atividade física.


Abstract Background nutritional status and its associated factors play an important role in the health of the elderly. Objective To identify the factors associated with the nutritional status of the elderly in primary health care at the city of Maringá, Paraná. Method A cross-sectional study was carried out with 654 Basic Health Units' elderly patients. The research instruments included a sociodemographic questionnaire, the Mini Nutritional Assessment, and the International Physical Activity Questionnaire. Data analysis was performed using the Kolmogorov-Smirnov, Mann-Whitney U, Chi-square and Binary Logistic Regression tests (p <0.05). Results The results showed a higher proportion of elderly patients with good health perception, who took less medication, had no falls, and had a high/average activity level with normal nutritional status. It was verified that the elderly with normal nutritional status presented a higher calf circumference (p = 0.01). It should be emphasized that older people with better health perception, higher level of physical activity and calf circumference, higher level of education and monthly income of 2.1 to 3 minimum wages are more likely to present normal nutritional status. Conclusion It was concluded that a normal nutritional status in the elderly population is associated with a higher educational level, greater purchasing power, better health perception and regular practice of physical activity.

INTRODUCTION: Contamination of cell phones can contribute to the dissemination of pathogens in the community and/or hospital environment. OBJECTIVE: To characterize Staphylococcus aureus strains isolated from cell phones of university students. METHODS: Samples were collected from 100 cell phones. Detection of genes associated with virulence factors such as biofilm formation (icaA and icaD), enterotoxins production (SEA, SEB, SEC, and SED), and resistance to methicillin (mecA and mecC) was performed in S. aureus isolates by PCR. Typing mecA gene performed by multiplex PCR. Susceptibility to antimicrobials and biofilm formation rate also evaluated by using disk diffusion test and crystal violet staining. RESULTS: S. aureus was present in 40% of the total samples and about 70% of them belonged to Nursing students. Of the isolates, 85% presented resistance to penicillin and 50% were classified as moderate biofilm producers. In addition, 92.5% of isolates contained the gene icaA and 60% of the gene icaD. Approximately 25% of the isolates presented the mecA gene. Typing of the mecA gene showed the presence of staphylococcal chromosome cassette SCCmec I and c III respectively in 20% and 10% of the isolates. 70% of the samples could not be typed by the technique. Regarding the enterotoxins, the most prevalent gene was SEA (30%) followed by the SEC gene (2.5%). The presence of SED and SEB genes not observed in any of the isolates. CONCLUSION: The cleaning and periodic disinfection of cell phones can contribute to the reduction of the risk of nosocomial infection.


INTRODUÇÃO: A contaminação de celulares pode contribuir para a disseminação de patógenos na comunidade e/ou ambiente hospitalar. OBJETIVO: Caracterizar cepas de Staphylococcus aureus de telefones celulares de estudantes universitários. MÉTODOS: Foram coletadas amostras de 100 telefones celulares. Detecção de genes associados a fatores de virulência quanto a: formação de biofilme (icaA e icaD), produção de enterotoxinas (SEA, SEB, SEC e SED) e resistência à meticilina (mecA e mecC) foi realizada em isolados de S. aureus por PCR. A Tipagem do gene mecA foi realizada por PCR multiplex. A susceptibilidade a antimicrobianos e a taxa de formação de biofilme pelo teste de difusão em disco e coloração com cristal violeta. RESULTADOS: S. aureus esteve presente em 40% do total de amostras, destas, 70% pertenciam a estudantes do curso de enfermagem. Dos isolados, 85% apresentaram resistência à penicilina e 50% foram classificados com moderada formação de biofilme. Além disso, 92,5% dos isolados continham o gene icaA e 60% o gene icaD. Aproximadamente 25% dos isolados apresentaram o gene mecA. A tipagem do gene mecA mostrou a presença do cassete cromossômico estafilocócico SSCmec I e III em respectivamente 20% e 10% dos isolados. 70% das amostras não puderam ser identificadas pela técnica. Das enterotoxinas, o gene mais prevalente foi o SEA (30%), seguido pelo gene SEC (2.5%). A presença dos genes SED e SEB não foi observada nos isolados. CONCLUSÃO: A limpeza e desinfecção periódica dos telefones celulares podem contribuir para a redução do risco de infecção nosocomiais.

ABSTRACT

Objetivo: descrever a vivência de estudantes da primeira série dos cursos de Enfermagem e Medicina na área de cuidado coletivo em uma Unidade da Estratégia de Saúde da Família. Métodos: trata-se de um relato de experiência sobre o processo ensino-aprendizagem desenvolvido na área de vigilância à saúde para o cuidado das necessidades coletivas de saúde. Foi utilizada a Estimativa Rápida Participativa, observação do território, análise dos relatórios de cadastros do sistema e-SUS e entrevistas com informantes-chave como estratégias para elaboração do Diagnóstico Situacional de Saúde. Resultados: a ação dos estudantes na prática estimulou o interesse na busca de fundamentação teórica e possibilitou a aprendizagem significativa em relação à organização do Sistema Único de Saúde, às necessidades de saúde da população e ao planejamento em saúde, favorecendo a compreensão sobre o processo saúde-doença e as diferentes formas de intervenção. Conclusão: A inserção precoce dos estudantes no cenário de prática constituiu-se em uma importante estratégia para as integrações prática-teoria e ensino-serviço e, além disso, favoreceu o protagonismo dos estudantes na aplicação dos princípios do SUS, como a participação social, o trabalho multidisciplinar e a integralidade do cuidado.

ABSTRACT

OBJECTIVES: to analyze the practices of nursing auxiliaries and technicians in the context of Primary Health Care in a city in the interior of the state of São Paulo. METHODS: transversal, quantitative study, realized with 104 nursing auxiliaries and 46 nursing technicians acting in the Primary Attention. The data, collected using structured questionnaires, are presented by descriptive statistics, and analyzed based on the essential elements of First Contact and Longitudinality. RESULTS: the professionals were, majority, women, married, with children, low political and collective participation. We identified thirteen activities related to the First Contact: and ten on Longitudinality. CONCLUSIONS: the insertion and capillarization of auxiliary and nursing technicians' practices show these professionals as the main ones responsible for the first contact in this context, highlighting their relevance in the longitudinality of care and the work composition of health teams, contributing to the consolidation of the Unified Health System (SUS).

OBJETIVO: el objetivo del presente estudio fue analizar las relaciones entre la actividad física vigorosa y la composición corporal en personas mayores que asistían a centros de atención primaria. MATERIAL Y MÉTODOS: este estudio se caracteriza por ser un estudio epidemiológico descriptivo y transversal de 654 adultos mayores (288 hombres y 366 mujeres), la mayoría de ellos de 60 a 69 años (59 %), que asistían a centros de atención primaria en la ciudad de Maringá, Brasil. Los participantes fueron evaluados con el Cuestionario Internacional de Actividad Física (IPAQ), mediciones antropométricas y un cuestionario sociodemográfico para caracterizar la muestra. RESULTADOS: los ancianos que practican actividad física en los centros de atención primaria con intensidad vigorosa presentan mediciones más bajas de circunferencia abdominal, cintura, cadera, cuádriceps y pantorrilla en comparación con las personas mayores que practican actividad física de baja a moderada intensidad. CONCLUSIONES: la práctica de actividad física vigorosa está relacionada con bajos perímetros corporales (abdominal, cadera, cintura, pantorrilla) en las personas adultas mayores atendidas en centros de atención primaria

ABSTRACT

Objetivo: descrever os aspectos epidemiológicos da morbimortalidade pelo Vírus da Imunodeficiência Humana no nordeste brasileiro entre 2013 e 2017. Métodos: estudo descritivo e transversal realizado com dados do Sistema de Informações Hospitalares. Selecionou-se as variáveis: unidades federativas, sexo, faixa etária, e cor/raça. Resultados: registrou-se 34.647 internações no nordeste brasileiro. Destes, 4.031 evoluíram para o óbito, correspondendo a mortalidade de 11,63%. Evidenciou-se maior morbidade (30,77%) e óbito (19,40%) em Pernambuco e maior mortalidade em Alagoas (19,40%). Os homens prevaleceram nas internações (65,98%), óbitos (69,51%) e mortalidade (12,26%). Os adultos entre 35 e 39 anos tiveram maior morbidade (17,66%) e óbitos (17,39%), já os idosos ≥80 anos, tiveram maior mortalidade (22,64%). A raça/cor parda prevaleceu nas internações (51,36%) e óbitos (49,71%) e os indígenas na mortalidade (50%). Conclusão: a epidemiologia das infecções reflete na necessidade de intervenções em saúde principalmente no estado de Alagoas por apresentar maior mortalidade pela patologia


Objective: to describe the epidemiological aspects of morbimortality due Human Immunodeficiency Virus in the Brazilian northeast between 2013 and 2017. Methods: descriptive and cross-sectional study with data collected through the hospital information systems. It were selected the variables: federative units, sex, age groups and color/race. Results: it was registered 34.647 hospitalizations in Brazilian northeast. Of these, 4.031 died, corresponding to the mortality of 11,63%. It were evidenced higher morbidity (30,77%) and death (19,40%) in Pernambuco and higher mortality in Alagoas (19,40%). The men have prevailed in the hospitalizations (65,98%), deaths (69,51%) and mortality (12,26%). The adults between 35 and 39 years old had greater morbidity (17,66%) and deaths (17,39%) and the elderly ≥80 years old had greater mortality (22,64%). The brown color/race have prevailed in the hospitalizations (51,36%) and deaths (49,71%) and the indigenous in the mortality (50%). Conclusion: the epidemiology of infections reflects in the need of health care interventions, mainly in the state of Alagoas for presenting greater mortality due pathology

Los hombres prevalecieron en la morbilidad (65,98%), muertes (69,51%) y mortalidad (12,26%). Los adultos entre 35 y 39 años tuvieran mayor morbilidad (17,66%) y muertes (17,39%), y las personas mayores ≥80 años tuvieran mayor mortalidad (22,64%). La color/raza parda prevaleció en las hospitalizaciones (51,36%) y muertes (49,71%) y los indígenas en la mortalidad (50%). Conclusión: la epidemiología de las infecciones refleja en la necesidad de intervenciones en la salud principalmente en el estado de Alagoas por presentar mayor mortalidad pela patología

ABSTRACT

ABSTRACT Objectives: to analyze the practices of nursing auxiliaries and technicians in the context of Primary Health Care in a city in the interior of the state of São Paulo. Methods: transversal, quantitative study, realized with 104 nursing auxiliaries and 46 nursing technicians acting in the Primary Attention. The data, collected using structured questionnaires, are presented by descriptive statistics, and analyzed based on the essential elements of First Contact and Longitudinality. Results: the professionals were, majority, women, married, with children, low political and collective participation. We identified thirteen activities related to the First Contact: and ten on Longitudinality. Conclusions: the insertion and capillarization of auxiliary and nursing technicians' practices show these professionals as the main ones responsible for the first contact in this context, highlighting their relevance in the longitudinality of care and the work composition of health teams, contributing to the consolidation of the Unified Health System (SUS).


RESUMEN Objetivos: analizar las prácticas de las auxiliares y técnicas de enfermería en el contexto de Atención Primaria de Salud en un municipio de São Paulo. Métodos: estudio transversal, cuantitativo, realizado con 104 auxiliares de enfermería y 46 técnicas de enfermería actuantes en Atención Primaria. ABSTRACT

OBJECTIVE: To analyze the scientific evidences in the literature about the evaluation of Lean Healthcare after its implementation. METHOD: An integrative review conducted in the CINAHL, Scopus, WOS, and Embase databases, and in the PubMed portal, resulting in 18 articles published in English, Spanish and Portuguese, from 2008 to 2019. RESULTS: The findings were categorized into results for the institution, professional, and patient. The predominant category was institutional, with cost analysis and increased productivity, followed by the professional, with job satisfaction and leadership, and finally the patient, with satisfaction, attitudes, and behaviors. CONCLUSION: This study reinforces the need to establish, for management, a systematic method of monitoring the results achieved in the Lean Healthcare implementation phase. Since the value in this method is defined by the patient, further research in this aspect may lead to new evidence.

ABSTRACT

INTRODUCTION: Intermittent 1-34 parathyroid hormone (iPTH) administration, a bone-forming treatment, is widely used as a therapy for severe osteoporosis. It can only be used for a maximum of 24 mo and must be followed by an antiresorptive drug to retain the new formed tissue. Mechanical load, in the form of low-intensity and high-frequency vibration, has received considerable attention due to its ability to prevent bone loss. AIM: To investigate the ability of whole body mechanical vibration (MV) to potentiate the anabolic effects of iPTH and to inhibit bone resorption following discontinuation of iPTH treatment in estrogen-deficient rats. METHODOLOGY: Fifty-four 6-month-old female Wistar rats were ovariectomized (OVX) or sham-operated. After 5 mo, they were divided into 7 groups: Sham - non-OVX; Control - OVX, vehicle for 60 d; MV - OVX, submitted to MV for 60 d; PTH60d - OVX, injected with iPTH for 60 d; PTH+MV - OVX, injected with iPTH combined with MV for 60 d; PTH30d - OVX, injected with iPTH for 30 d, and untreated for 30 d; PTH30d/MV30d - OVX, injected with iPTH for 30 d, followed by MV for 30 d. Bone mineral density (BMD) and body composition (lean mass and fat) were evaluated at OVX (T0), the beginning (T1), and at the end (T2) of treatments by dual X-ray absorptiometry (DXA). Femurs were processed for histomorphometry (bone volume - BV/TV and cortical thickness - Ct.Th) and tibias for biomechanical test. RESULTS: Body composition and BMD were similar among the groups at T0. In T2, MV presented higher fat than other groups (except PTH60d) and PTH30d/MV30d showed greater lean mass than Control. At T1, Sham presented the highest BMD, but between T1 vs T2 there was an increase in all iPTH-treated groups. At T2, BMD was higher in PTH60d and PTH+MV than in the Control and MV groups. The highest BV/TV was observed in the PTH+MV group, followed by PTH60d. Cortical thickness was increased in PTH60d and PTH+MV compared to Sham. Vibration applied post-iPTH (PTH30d/MV30d) improved the force at failure in tibias when compared to Sham and Control groups. CONCLUSION: MV potentiated iPTH anabolic effects in cancellous bone; however, MV was unable to maintain bone mass after stopping iPTH in ovariectomized rats.

Objetivo: Descrever os casos de internações e custos hospitalares por aborto espontâneo no estado da Bahia, Brasil no período de 2011 a 2016. Métodos: Estudo epidemiológico, descritivo e transversal baseado no banco de dados secundários do Departamento de Informática do Sistema Único de Saúde. As variáveis coletadas foram: faixa etária, raça/cor e custos hospitalares. Resultados: Foram registrados 53.761 casos de internações. A maior prevalência ocorreu na macrorregião leste (n=21.925), com faixa etária entre 20 e 24 anos (n=11.944) e cor/raça parda (n=30.319). Os custos hospitalares foram de R$ 6.497.904,99 reais, sendo que o ano de 2012 e a macrorregião Leste geraram maior ônus ao estado. Conclusão: Evidenciou-se grande prevalência de internações na Bahia. A análise desse quadro retoma a necessidade de conceder atenção especial à problemática, principalmente nos grupos prioritários da macrorregião leste do estado


Objective: To describe the cases of hospitalizations and hospital costs for spontaneous abortion in the state of Bahia, Brazil, in the period from 2011 to 2016. Methods: Epidemiological, cross-sectional and descriptive study based on secondary data from the Information Technology Department of the Unified Health System. The variables collected were: age groups, race and hospital costs. Results: It has registered 53.761 cases of hospitalizations. The greatest prevalence occurred in the east macro region (n=21.925), aged between 20 and 24 years old (n=11.943) and color/race brown (n=30.317). The hospital costs were 6.497.904,99 reais, and the year 2012 and the east macro region generated the greatest cost to the state. Conclusion: The study highlighted great prevalence of hospitalizations in Bahia. The analysis of that situation shows the need to give special attention to the problematic, mainly on the priority groups from east macro region of the state

Considerando as alterações funcionais e diante das mudanças na composição corporal as quais o idoso está exposto, faz-se necessário realizar estudos que busquem verificar a influência de variáveis como sexo e idade na composição corporal e no estado nutricional nessa população. Este estudo comparou a composição corporal e o estado nutricional entre idosos ativos fisicamente e sedentários de acordo com o sexo e a idade. Participaram 651 idosos usuários de Unidades Básicas de Saúde de Maringá-PR, que responderam a Mini Avaliação Nutricional e o Questionário Internacional de Atividade Física. A análise dos dados foi conduzida por meio dos testes de Kolmogorov-Smirnov, Kruskal-Wallis e U" de Mann-Whitney (p<0,05). Os resultados evidenciaram que os homens irregularmente ativos apresentaram menor peso, circunferência abdominal e índice de massa corporal do que os ativos e sedentários. Os homens sedentários apresentaram maior circunferência de quadril do que os demais grupos, enquanto que os ativos apresentaram maior escore na mini avaliação nutricional. As mulheres ativas apresentaram menor relação cintura-quadril do que as irregularmente ativas e sedentárias. Ainda, os homens irregularmente ativos relataram pior estado nutricional do que os ativos e sedentários. Concluiu-se que o nível de atividade física pode ser considerado um fator interveniente na composição corporal de idosos, principalmente no sexo masculino, além de interferir no estado nutricional de idosos acima de 70 anos.


Considering the functional changes and in view of the changes in body composition to which the elderly are exposed, it is necessary to conduct studies that seek to verify the influence of variables such as sex and age on body composition and nutritional status in this population. This study compared body composition and nutritional status among physically active and sedentary elderly people according to sex and age. 651 elderly users of Basic Health Units in Maringá-PR participated, who answered the Mini Nutritional Assessment and the International Physical Activity Questionnaire. Data analysis was performed using the Kolmogorov-Smirnov, Kruskal-Wallis and Mann-Whitney U tests (p<0.05). The results showed that irregularly active men had a lower weight, waist circumference and body mass index than active and sedentary men. Sedentary men had a greater hip circumference than the other groups, while the active ones had a higher score in the mini nutritional assessment. Active women had a lower waist-to-hip ratio than irregularly active and sedentary women. In addition, irregularly active men reported a poorer nutritional status than active and sedentary men. It was concluded that the level of physical activity can be considered an intervening factor in the body composition of the elderly, especially among males, in addition to interfering in the nutritional status of the elderly over 70 years old.

INTRODUCTION: The field of nutritional psychiatry is a fast-growing one. Although initially, it focused on the effects of vitamins and micronutrients in mental health, in the last decade, its focus also extended to the dietary patterns. The possibility of a dietary cost-effective intervention in the most common mental disorder, depression, cannot be overlooked due to its potential large-scale impact. METHOD: A classic review of the literature was conducted, and studies published between 2010 and 2018 focusing on the impact of dietary patterns in depression and depressive symptoms were included. RESULTS: We found 10 studies that matched our criteria. Most studies showed an inverse association between healthy dietary patterns, rich in fruits, vegetables, lean meats, nuts and whole grains, and with low intake of processed and sugary foods, and depression and depressive symptoms throughout an array of age groups, although some authors reported statistical significance only in women. While most studies were of cross-sectional design, making it difficult to infer causality, a randomized controlled trial presented similar results. DISCUSSION: The association between dietary patterns and depression is now well-established, although the exact etiological pathways are still unknown. Dietary intervention, with the implementation of healthier dietary patterns, closer to the traditional ones, can play an important role in the prevention and adjunctive therapy of depression and depressive symptoms. CONCLUSION: More large-scale randomized clinical trials need to be conducted, in order to confirm the association between high-quality dietary patterns and lower risk of depression and depressive symptoms.

ABSTRACT

INTRODUCTION: Attention deficit hyperactivity disorder (ADHD) is the most common psychiatric diagnosis in childhood and adolescence, with an estimated worldwide-pooled prevalence of 5,29%. The type of treatment depends on several factors. Psychopharmacological treatment entails undesirable side effects, with unclear long-term benefits, which has led the scientific community to investigate other therapeutic approaches, such as dietary interventions. METHOD: The authors conducted a classical review on the current treatment recommended in individuals with ADHD diagnosis, their dietary patterns, as well as dietary factors possibly implicated in the etiology and treatment of this disorder. An extensive bibliographic research was carried out in the databases PubMed, The Cochrane Library and the National Guideline Clearinghouse. DISCUSSION: The most common dietary interventions in the case of ADHD are food supplementation diets (e.g. PUFAs, vitamins) and elimination diets. Supplementation with omega-3 PUFAs lacks further studies that can validate them as an effective therapeutic approach in this disorder. Also, regarding vitamin supplementation, studies are not consistent as to their role in the etiology of ADHD. Elimination diets are unclear as to the benefits provided in individuals with ADHD. Children with ADHD are less likely to engage in healthy lifestyle behaviors than non-ADHD youth. CONCLUSION: There is no clear evidence that supports dietary interventions for the treatment of ADHD. The effects of unhealthy diet patterns in ADHD individuals are not yet fully understood and, like the general population, children with ADHD may benefit from a healthy lifestyle.

BACKGROUND: Sexual dysfunction and infertility are conditions with high prevalence in the general population. Nutritional factors have been reported to have an impact on sexual and reproductive health. OBJECTIVE: The aim of this review is to summarize the data on nutritional factors that have influence on male and female sexual and reproductive function, including nutritional status, specific foods (e.g. dairy food), nutrients and other food components and dietary supplements. METHOD: A literature search was performed using Cochrane Library, Medline and Science Direct databases without time limitations. RESULTS: Obesity has a negative influence on male fertility, and weight loss improves male fertility. Food insufficiency is associated with increased sexual risk behaviours, more significant in women. Regarding macronutrients and group foods, trans-fatty acids, high glycemic index food, high carbohydrate diet and high animal protein intake prejudices fertility; omega-3 and omega-6 fatty acids, low glycemic index food and low carbohydrate diet, vegetable proteins and antioxidants improve fertility. Isoflavones have a negative impact on men fertility and improve sexual health of menopausal women. Whole milk improves women fertility, but men benefit from skim milk. Concerning dietary supplements, there is weak evidence sustaining efficacy, and the most promising supplements are yohimbine, vitamin B, L-arginine and vitamin D. CONCLUSION: The compiled results indicate that despite the multifactorial etiology of sexual/ reproductive dysfunction, nutritional factors may affect the sexual and reproductive health in both men and women. However, it is necessary to further study to clarify this association and simultaneously improve the approach and treatment of patients with sexual and/or reproductive problems.

Objetivo: averiguar os registros sobre a realização de tratamento hemodialítico e seus impactos financeiros no Nordeste do Brasil, entre 2014 e 2017. Método: trata-se de estudo quantitativo, descritivo e ecológico, com dados do Sistema de Informações Ambulatoriais. Selecionou-se as opções referentes à hemodiálise, resultando em oito tipos de procedimentos. Analisaram-se os dados por meio de estatística descritiva simples que se apresentaram em tabelas. Resultados: realizaram-se 11.837.165 procedimentos hemodialíticos. Evidenciou-se maior prevalência de hemodiálise na Bahia (24,6%) e em caráter eletivo (97,9%). Destaca-se, ainda, o impacto financeiro superior a R$ 2,2 bilhões, maior prevalência de ônus na Bahia (24,6%) e o procedimento realizado na modalidade de no máximo três vezes por semana responsabilizou-se por 93,6% do total dos custos. Conclusão: conclui-se que, houve comportamento crescente na realização de hemodiálises, implicando no incremento dos custos públicos e tornando-se um fardo econômico para o Sistema Único de Saúde. Ressalta-se que, esse estudo fornece subsídios para incentivar a adesão, o fortalecimento e valorização da Atenção Básica, especialmente em relação ao aumento da qualidade assistencial aos usuários com fatores de risco.(AU)


Objective: to analyze the records on the accomplishment of hemodialysis treatment and its financial impacts in Northeast Brazil, between 2014 and 2017. Method: this is a quantitative, descriptive and ecological study, with data from the Outpatient Information System. The options related to hemodialysis were selected, resulting in eight types of procedures. Data were analyzed by means of descriptive and simple statistics and presented as tables. Results: there were 11,837,165 hemodialysis procedures. There was higher prevalence of hemodialysis in Bahia (24.6%) and in elective character (97.9%). Moreover, the financial impact exceeding R$ 2.2 billion, higher prevalence of burden in Bahia (24.6%) and the procedure carried out up to three times per week are responsible for 93.6% of the total costs. Conclusion: there was a growing behavior in the number of hemodialysis procedures, implying increased public costs and becoming an economic burden for the Unified Health System. This study provides subsidies to encourage the adherence, strengthening and enhancement of Basic Care, especially in relation to the increased quality of care to users with risk factors.(AU)

ABSTRACT

Objetivo: descrever as internações, óbitos e custos hospitalares pelas intercorrências dialíticas em pacientes renais crônicos no Nordeste. Método: trata-se de um estudo quantitativo, descritivo e ecológico, voltado para a análise de dados do Sistema de Informações Hospitalares, organizados em frequências absolutas e relativas, a partir de tabelas construídas no software Excel. Resultados: notificaram-se 14.052 internações e 987 óbitos no Nordeste. Gerou-se, como consequência, um custo superior a R$ 19,6 milhões aos cofres públicos, com um valor médio de internação de R$ 1.543,09 e uma média de permanência de 9,1 dias. Destacaram-se os Estados de Alagoas, com a prevalência das internações (38,2%), e Bahia, em relação aos óbitos (40%), custos hospitalares (61,4%), média de permanência (14,4 dias) e valor médio de internação (R$ 2.794,42). Conclusão: aponta-se que as internações e óbitos pelas intercorrências dialíticas constituem um importante problema na Nefrologia, causando prejuízos diretos aos recursos financeiros públicos, especialmente, nos Estados da Bahia e Alagoas, por evidenciarem a maioria dos casos.(AU)

Method: this is a quantitative, descriptive and ecological study, focused on the analysis of data from the Hospital Information System, organized in absolute and relative frequencies, using tables built using Excel software. Results: 14,052 hospitalizations and 987 deaths were reported in the Northeast. As a result, the public coffers cost over R $ 19.6 million, with an average hospitalization value of R $ 1,543.09 and an average length of stay of 9.1 days. The states of Alagoas stood out, with the prevalence of hospitalizations (38.2%), and Bahia, in relation to deaths (40%), hospital costs (61.4%), average length of stay (14.4 days) and average value of hospitalization (R $ 2,794.42). Conclusion: it is pointed out that hospitalizations and deaths due to dialysis complications are an important problem in Nephrology, causing direct damage to public financial resources, especially in the states of Bahia and Alagoas, as they show the majority of cases.(AU)

Método: se trata de un estudio cuantitativo, descriptivo y ecológico, destinado a analizar los datos del Sistema de Informaciones del Hospital, organizado en frecuencias absolutas y relativas, a partir de tablas construidas en el software Excel. Resultados: se reportaron 14.052 hospitalizaciones y 987 muertes en el noreste. Se generó como resultado de eso, un costo en exceso de R $ 19,6 millones para el gobierno, una hospitalización con costo promedio de R$1,543.09 y una estadía promedio de 9.1 días. Entre los estados, Alagoas tuvo una mayor prevalencia de hospitalizaciones (38,2%) y Bahía en las muertes (40%), costos hospitalarios (61,4%), duración media de la estancia (14,4 días) y media hospitalización (R $ 2.794,42). Conclusión: se observa que las hospitalizaciones y muertes debidas a las intercurrencias dialíticas se constituyen un problema importante en la nefrología, lo que implica directamente en los cofres públicos, especialmente en los Estados de Bahía y Alagoas, como evidencia de una mayor prevalencia de casos.
