ABSTRACT

Objective To discuss the clinical effect of facial contour remodeling using transplantation of the autologous granular fat grafting combined with botulinum toxin A injection.Methods Negative-pressure liposuction was carried out in the inner thigh or abdomen using liposuction needle connected with an injector,to let stand for layered,discharge lower layer water,rinse,extract the fat particles into a 2 ml syringe,inject into facial depression area with multipoint,multiple-tunnel,and multilayer manner,over injection of about 20%-30% every time.89 patients received the injection;some accepted two injections;13 cases were injected in temple (14.6%),and 16 in forehead (17.98%).30 of them received subcutaneous injection of botulinum toxin A,each side dose was less than 20 U.Results Patients had been followed up for 3-36 months,and obvious improvements were observed in facial contour in the frontal and temporal region as well as the skin texture;the survival rate of fat granule reached to 60%-80%.No hematoma,nodes and infection been observed.Patients combined with botulinum toxin A injection had more ideal facial contour and more satisfaction.Conclusions Autologous fat granules transplantation in the face has stable clinical effect,and combining with botulinum toxin A injection can improve facial contour.It is a safe,ideal treatment in facial rejuvenation and facial contour remodeling and it therefore can be widely recommended in clinical treatment.

ABSTRACT

Hidradenitis suppurativa is a cutaneous, chronic, recurrent inflammatory disease. Here, we report the case of a 66-year-old man who had hidradenitis suppurativa in the buttocks. He suffered from diabetes mellitus. In the past, he had perianal abscesses. Because of improper treatment of furuncle infections in the buttocks, skin ulcers formed, which worsened and resulted in multiple fistulas. The skin lesion surface was large and the infection was severe. After wound debridement treatment, topical negative pressure and nutritional support were given. After one and a half months, the wound healed with split-thickness skin grafting. In a 2-year follow-up, there was no evidence of hidradenitis suppurativa recurrence.

ABSTRACT

The treatment of chronic skin ulcers with exposed bone in the lower extremity is difficult. The development of topical negative pressure (TNP) technology and microsurgical techniques have achieved good clinical results in the repair of such ulcers. Between January 2007 and March 2012, TNP combined with flap transfer was applied to repair lower-limb skin and soft-tissue defects associated with exposed bone in 22 cases at our hospital, and good results were achieved that are reported herein.

ABSTRACT

Objective To provide structure and function applied anatomical of nasal sub-units for plastic and reconstruction. Methods Twelve cases fresh nose from adult head specimen,to dissect the skin,muscle fascia system, cartilage and bone,to observe and digiital measure nerve, blood vessel and ligament between cartilage. Results The thinnest skin in the point of bone and cartilage junction,the thickest parts in the nasion and the supratip breakpoint.Nose contours include of the skin,cartilage,bones and vascular muscle fascia system;Nasal subunit can be divided into nasion area,nasal dorsum area, nasal tip area,nasal ala area and nasal columella area;Nasal valve was a important anatomic part of bremh.The nasal lateral osteotomy of maxilla can change 2 mm height and width of the nasal bone;Lower lateral cartilage and upper nasal cartilage connection can be separated 6~8 mm. Curvature changes in the crura intermedium of alar cartilage Can raise nasal tip 2 mm. Extent of septal cartilage was 15 mm×20 mm,thickness was 1 mm.The mainly blood supply come from facial artery and ophthalmic artery. Vein accompanying with the same name.Lymphatic flow in the area of superficial muscle fascia. Concision Nasal bone lateral osteotomy can raise up bridge of the nose.To lengthen and highten nasal tip based on the complete strip technique of lower lateral cartilage and upper lateral,and to change angel of medial crus and middle crus of lower lateral cartilage by suture rotation.Nasal skin is rich in artery and vein,so much as four networks.Because the syndrome of nasal contraction deformation occurred more and more recently we must to avoid the vascular network injury when operation, and the anatomical level should be give more noted.
