ABSTRACT

Liver failure often has rapid progression, multiple complications, and dangerous conditions. Acute pancreatitis is a common comorbidity during the progression of liver failure, and since acute pancreatitis has extremely similar clinical symptoms and signs to liver failure complicated by spontaneous peritonitis, it is often neglected in clinical practice. This article elaborates on the mechanisms of liver failure complicated by acute pancreatitis from the five aspects of inflammatory response, duodenal papillary dysfunction, gut microbiota dysbiosis, oxidative stress, and microcirculatory disturbance and proposes corresponding preventive measures based on these mechanisms.

ABSTRACT

ObjectiveTo investigate the expression of phosphoinositide 3-kinase (PI3K), mammalian target of rapamycin (mTOR), 70-kDa ribosomal protein S6 kinase (p70S6K), and interferon-α (IFNα) in umbilical cord blood plasmacytoid dendritic cells (pDCs) in parturients in the immune tolerance stage of hepatitis B virus (HBV) infection versus normal parturients. MethodsA total of 20 parturients in the immune tolerance stage of HBV infection and 10 normal parturients who were hospitalized in Inpatient Department of Obstetrics in The First Affiliated Hospital of Hunan University of Chinese Medicine from October 2017 to January 2020 were enrolled as hepatitis B group and normal group, respectively. Umbilical cord blood pDCs were isolated and cultured, and CpG-A was added on day 7. The cells and the supernatant were collected after 24 hours; real-time PCR was used to measure the mRNA expression of PI3K, mTOR, and p70S6K, Western Blot was used to measure the protein expression of PI3K, mTOR, and p70S6K, and ELISA was used to measure the level of IFNα in the supernatant of pDCs. The two-independent-samples t-test was used for comparison of continuous data between the two groups. ResultsCompared with the normal group, the hepatitis B group had significantly lower mRNA and protein expression of PI3K, mTOR, and p70S6K in umbilical cord blood pDCs (mRNA expression: t=-81.04, -63.07, and -34.55, all P<0.001; protein expression: t=-8.13, -7.75, and -6.71, all P<0.001). The hepatitis B group had significantly lower expression of IFNα in the supernatant of umbilical cord blood pDCs than the normal group (t=-15.88, P<0.05). ConclusionParturients in the immune tolerance stage of HBV infection have reductions in the mRNA and protein expression of PI3K, mTOR, and p70S6K and the level of IFNα in umbilical cord blood pDCs, suggesting that pDC function is inhibited.

ABSTRACT

PURPOSE: Acetabular coverage deficiency displays individual difference among patients with developmental dysplasia of the hip (DDH). Therefore, the correct direction and degree of the acetabular fragment is patient-specific during Bernese periacetabular osteotomy (PAO). This paper introduces a feasible method using 3D computed tomography (CT) and computer image processing technology for customised surgical planning. METHODS: CT data of 96 hips in 60 DDH patients (male 15, female 45; average age/range 30 ± 8/14-49 years) and 53 normal hips (male 13, female 37; average age/range 52 ± 13/16-69 years) were reconstructed using commercially available software Mimics and Imageware. Geometric parameters of each hip were measured in relation to the anterior pelvic plane after correcting for pelvic tilt and rotation. Deficiency types and degrees of acetabular dysplasia in patients with DDH were determined by comparison with normal hips, and improvement in femoral-head coverage was analysed again after virtual PAO. A customised surgery programme for each DDH patient was designed and provided the reference for the actual operation. RESULTS: We produced a 3D pelvic model using image processing software, doing precise measurement and with close approximation to the actual PAO. Lateral centre-edge angle (LCEA), anterior centre-edge angle (ACEA), acetabular anteversion angle (AAVA), anterior acetabular sector angle (AASA) and posterior acetabular sector angle (PASA) of normal hips in the control group were 35.128 ± 6.337, 57.052 ± 6.853, 19.215 ± 5.504, 61.537 ± 7.291 and 99.434 ± 8.372°, respectively. Angles of hips with DDH before surgery were 11.46 ± 11.19, 35.79 ± 13.75, 22.77 ± 6.13, 43.58 ± 9.15 and 88.46 ± 8.24, which were corrected to 33.81 ± 2.36, 55.38 ± 2.09, 20.16 ± 2.18, 58.29 ± 7.60, and 4.71 ± 7.75°, respectively, after surgery. After virtual Bernese PAO, LCEA, ACEA, AAVA, AASA and PASA were corrected significantly (p < 0.01). There was no statistically significant differences between LCEA, ACEA and AAVA after virtual Bernese PAO and normal hips (p = 0.06, p = 0.23, p = 0.06°, respectively). AASA improved significantly (p = 0.002) post-operatively at the cost of reducing posterior coverage represented by PASA, which is significantly smaller than in normal and pre-operative hips of DDH patients (p < 0.01). CONCLUSIONS: The geometric feature of the pelvis for patients with DDH can be assessed comprehensively by using 3D-CT reconstruction and image processing technology. Based on this method, surgeons can design individualised treatment scheme and improve the effect of PAO.

ABSTRACT

After peripheral nerve injury, axons often project sprouts from the node of Ranvier proximal to the damage site. It is well known that one parent axon can sprout and maintain several regenerating axons. If enough endoneurial tubes in the distal stump are present for the regenerating axons to grow along, then the number of mature myelinated nerve fibers in the distal stump will be greater than the number in the proximal stump. "Multiple regeneration" is used to describe this phenomenon in the peripheral nerve. According to previous studies, a prominent nerve containing many axons can be repaired by the multiple regenerating axons sprouting from another nerve that contains fewer axons. Most peripheral nerves contain a mixture of myelinated motor and sensory axons as well as unmyelinated sensory and autonomic axons. In this study, a multiple regeneration animal model was developed by bridging the proximal common peroneal nerve with the distal common peroneal nerve and the tibial nerve. Differences in the multiple regeneration ratio of motor and sensory nerves were evaluated using histomorphometry one month after ablating the dorsal root ganglion (DRGs) and ventral roots, respectively. The results suggest that the motor nerves have a significantly larger multiple regeneration ratio than the sensory nerves at two different time points.

ABSTRACT

Objective To investigate the fibrosis-associated genes, especially those transforming growth factor-β(TGF-β) associated, expression in the tissue of extensive fibrotic kidney resulting from severe by-dronephrosis. Methods Compared 12 extensive fibrotic kidneys resulting from severe hydronephrosis with 6 normal renal tissues using PCR array. All samples were collected from our hospital from Aug. 2005 to Dec. 2006. The data were treated with △△Ct method. When 2△△Ct≤<0.5 or≥2, the difference was considered sig-nificant. Results A total of 49 differential genes expressed in the fibrotic renal tissues were screened out, of which, 25 were up-regulated and 24 were down-regulated. The differential genes included members and receptors of TGF-β super family and bone morphogenetic protein(BMP) family, target molecule of TGF-β or BMP signal pathway and several regulators. Conclusions Through investigating with PCR array, we certifi-cated the positive effect of Nodal, GSC and IGFI to renal fibrosis. And we suppose that TGF-β play a sec-ondary role in renal fibrosis, while BMP family and Leftyl work in a functional genome in the regulating of renal fibrosis.

ABSTRACT

OBJECTIVE:To study the spermicidal and antibacterial effects of tannic acid from Chinese gall.METHODS:Human sperm samples of 20 subjects were taken for spermicidal tests in vitro in accordance with the standard method recommended by WTO.The minimum inhibitory concentration and minimum bactericidal concentration(MIC and MBC)of tannic acid from Chinese gall on bacterium coli and staphylococcus aureus were detected.RESULTS:The lowest effective spermicidal concentration of tannic acid at 20s was 20mg? mL-1.The MIC and MBC of tannic acid against bacterium coli(3 strains)were 0.195~ 0.390mg? mL-1 and 0.390~ 0.780 mg? mL-1 respectively,against staphylococcus aureus(2 strains)were 0.049~ 0.098mg? mL-1 and 0.195~ 0.390mg? mL-1 respectively.CONCLUSION:Tannic acid from Chinese gall showed potent coagulating effect on spermatines and remarkable antibacterial effect,and which is expected to be an effective and safe vaginal spermicidal and antibacterial agent,further study on which remains to be carried out.

ABSTRACT

AIM:To observe the myogenic effect of bone marrow-derived mesenchymal stem cell(MSC)transplantation after chemical-induction and stress-induction on denervated skeletal muscle atrophy in vivo. METHODS:The experiment was performed at the Animal Experimental Center and Central Laboratory in Xinhua Hospital affiliated to Shanghai Jiao Tong University from April to October 2007.SD rats were provided by B&K Universal Group Limited.The experimental procedure was consistent with animal ethical standard.The MSCs from male SD rat femurs and tibia were isolated by density gradient centrifugation.The third passage of MSCs after chemical-induction and stress-induction was labeled with DAPI before transplantation.Thirty-six 8-week-old SD rats were randomly divided into control group and experimental group(n=18).The animal model of denervated gastrocnemius muscles were made by cutting the left sciatic nerve and creating nerve defect about 1 cm. MSCs were percutaneously transplanted to medial and lateral gastrocnemius of the experimental group rats,while low carbohydrates DMEM culture solution without MSCs and fetal bovine serum were transplanted to gastrocnemius of the control group.Bilateral gastrocnemius muscles of each rat from the two groups were weighed 4,8 and 12 weeks postoperatively after examining the motor unit potential and fibrillation potential.The gastrocnemius muscles underwent HE staining and image analysis to measure cross-section area of muscle fiber.The amount of protein was detected by BCA method. RESULTS:Muscular atrophy was observed 2 weeks after denervation.The motor unit waveshape of gastrocnemius became single; time limit became long;voltage became low,and the fibrillation potential norientation wave was increased.Differences were observed between two groups 4 weeks and 8 weeks,but no differences were observed after 12 weeks.Cells with fluorescence were observed in transplantation sites of the experimental group,but not in the control group.The wet weight remnant rate,cross-sectional area of fiber remnant rate and muscle amount protein content remnant rate in rates transplanted with MSCs were significantly lower than those in the control group 4 and 8 weeks after surgery(P
