ABSTRACT

Los miomas uterinos son el tumor más frecuente en la edad reproductiva y suponen un impacto negativo en la calidad de vida de las pacientes. El tratamiento de esta entidad debe hacerse de forma individualizada. La embolización de la arteria uterina es un tratamiento mínimamente invasivo, seguro, eficaz y con una baja tasa de complicaciones cuando se usa como tratamiento de miomas sintomáticos. El objetivo de este artículo es realizar una revisión bibliográfica exhaustiva sobre el estado actual de la embolización de las arterias uterinas en el tratamiento de los miomas que incluya consideraciones previas sobre enfoques técnicos y selección de casos, resultados clínicos en comparación con otras opciones de tratamiento y resultados en términos de calidad de vida y fertilidad.(AU)


Uterine fibroids are the most common tumour in reproductive age and have a negative impact on the quality of life of patients. Cases must be treated on an individual basis. Uterine artery embolization is a minimally invasive, safe, effective treatment with a low rate of complications when used as a treatment for symptomatic fibroids. The objective of this article is to conduct a comprehensive literature review on the current status of uterine artery embolization in the treatment of fibroids, including preliminary considerations on technical approaches and case selection clinical outcomes compared with other treatment options, and outcomes in terms of quality of life and fertility.(AU)

ABSTRACT

El tabique vaginal transverso es una malformación genital femenina muy infrecuente con una etiología desconocida y un manejo clínico aún por discernir. Presentamos el caso de una mujer de 17 años que presentaba estrechez del introito vaginal asociada a una intensa dismenorrea, con el diagnóstico final de esta entidad tras una exploración bajo anestesia. El conocimiento de esta anomalía junto con sus hallazgos clínicos y posibles entidades extragenitales asociadas permiten realizar un apropiado diagnóstico de sospecha y una correcta clasificación, lo que conlleva un manejo terapéutico más óptimo.(AU)


Transverse vaginal septum is a rare female genital malformation of unknown aetiology, and for which there is still no clear clinical management. The case is presented of a 17-year-old woman who had a narrow vaginal introitus associated with intense dysmenorrhoea. She was diagnosed with this disorder after an examination under anaesthesia. The knowledge of this anomaly, as well as its symptomatology and associated extra-genital pathologies, can lead to an appropriate suspected diagnosis and correct classification, and to a more optimal therapeutic management.(AU)

ABSTRACT

El embarazo heterotópico es una condición clínica poco frecuente en la que se pueden presentar varias gestaciones que coexisten dentro y fuera del útero. Su incidencia se ha incrementado en los últimos años debido a las técnicas de reproducción asistida. Tener un alto grado de sospecha puede ayudar en su diagnóstico precoz y tratamiento adecuado. Presentamos un caso de embarazo heterotópico en una mujer de 35 años que desarrolló a la vez un embarazo extra e intrauterino. Fue diagnosticado por ecografía a las 10 semanas de amenorrea (AU)


Heterotopic pregnancy is a rare clinical condition in which several pregnancies can coexist inside and outside the uterus. In recent years, the incidence of heterotopic pregnancy has increased due to assisted reproductive techniques. A high index of suspicion may help in the early diagnosis and appropriate treatment of this entity. We report a case of spontaneous heterotopic gestation in a 35-year-old woman who developed both an extra- and an intrauterine pregnancy diagnosed by ultrasound 10 weeks after the onset of amenorrhea (AU)

ABSTRACT

OBJECTIVE: Our aim was to assess the efficiency of methotrexate therapy for unruptured ectopic pregnancy. STUDY DESIGN: From November 1990 to December 1996, 60 patients diagnosed with unruptured ectopic pregnancies were treated by the Departamento de Ginecología y Obstetricia at the Hospital Virgen Macarena of Seville. Of these, 14 patients received an intrasacular injection of 15 mg of methotrexate, 45 were given an intramuscular dose of 50 mg/m2 and only one received the drug by both routes. RESULTS: Forty-six patients (76.7%) responded successfully to treatment. Of these, 10 had been given the drug intrasacularly (10/15, 66.7%), while 36 had received the drug intramuscularly (36/46, 78.3%). Twenty patients (33.3%) had minor side effects, the most frequent being mild abdominal pain. Falling levels of leukocytes on the third day after initiation of treatment may serve as a fairly accurate criterion to predict the patient's response to the treatment. CONCLUSION: Our data support the use of methotrexate therapy as a reliable alternative to surgical intervention in the treatment of unruptured ectopic pregnancy. Intramuscular administration, without laparoscopy, appears to be the most convenient route as it is easily implemented and has comparatively few complications.
