ABSTRACT

A case of direct communication between right pulmonary artery and left atrium is reported. The diagnosis was made before surgical correction. A surgical ligation of the fistula resolved the cyanosis of the patient. Selective angiocardiogram of the right pulmonary artery 4 months after surgery revealed no residual shunt. This very rare malformation should be considered in the clinical setting of unexplained cyanosis. This is the number 50 case reported in the literature.

ABSTRACT

From September 1996 to March 1997 we implanted stents Palmaz (P-308 Johnson & Johnson) in six patients with aortic coarctation. Age ranged from 13 to 30 years (mean = 20), 3 female and 3 male. We implanted the stent without predilation. Balloon diameter was 12 mm in two cases and 15 mm in four cases for complete expansion we used 14 to 20 mm balloon diameter in five instances and in one case we used a dual balloon 15 + 15 mm. The gradient pre-stent ranged from 30 to 65 mmHg (mean = 44) and decreased to 0 mmHg in five cases and in one patient the residual gradient was 4 mmHg. Systolic aortic pressure pre-stent was 135 mmHg and decreased to 117 mmHg. There were no significant complications. In conclusion, we have demonstrated the feasibility of balloon-expandable stent implantation as a method of alleviation the obstruction in coarctation of the aorta in this group of patients. It is an excellent alternative to surgical treatment and it is better than balloon angioplasty that results in marked improvement in the angiographic appearance of the thoracic aorta. It eliminates the gradient, has minimum morbidity and no mortality, no aneurysm formation. Late restenosis and possible aneurysm formation remains an unlikely complication in view of the ample luminal diameter and the high velocity of flow.

ABSTRACT

Five cases with cor triatriatum were diagnosed in less than a two years period. There were 4 females and one male. The mean age was 3.6 years. Three patients were operated on successfully. One patient died shortly after cardiac catheterization in acute pulmonary edema, another case is waiting for surgery with mild obstruction. We do not have a complete explanation for this high incidence of cor triatriatum. The utility of Echo Doppler color in diagnosis is highlighted. A literature review is made.

ABSTRACT

A 6-month-old male infant with coexistent type B interrupted aortic arch, distal aortopulmonary window, and anomalous origin of the right pulmonary artery arising from the ascending aorta was diagnosed by two-dimensional echocardiography, color Doppler, and cardiac catheterization. Review of the available literature reveals this patient to be the fourteenth reported case of this unusual association of cardiovascular defects.

ABSTRACT

We report two infants with Kawasaki disease and coronary aneurysms diagnosed by echocardiography. First case, a one year old male with abnormalities of left coronary artery, developed a myocardial infarction and died three weeks later. Second case a two months old male with aneurysm in the right coronary artery who doing well three months after the diagnosis was made. Echocardiography is the primary tool for evaluation and follow up of coronary abnormalities.

The history of a 12 year old male, who developed a bidirectional tachycardia and sudden death without evidence of neurological or cardiac disease is described. The role of bidirectional tachycardia related to sudden death is discussed, as well as its possible origin and mechanisms, based on the case described.

ABSTRACT

La finalidad de este estudio fue determinar la sensibilidad y la especificidad de la ecocardiografia bidimensional subcostal con vertice de la imagem inferior (EBSVI) para el diagnostico de la comunicacion interventricular (CIV) y su localizacion. Se estudiaron 72 pacientes menores de seis anos antes del cateterismo cardiaco. En 32 de 34 enfermos (94 por ciento), la CIV se diagnostico antes del cateterismo cardiaco y hubo dos resultados negativos falsos, ambos musculares.La prediccion fue correcta en los 38 casos en los que se establecio tabique interventricular integro. La localizacion del defecto se diagnostico correctamente en 90 por ciento de los casos. De este estudio se concluye que la EBSVI constitutye el recurso no invasor mas sensible y especifico para el diagnostico y la localizacion de la CIV

ABSTRACT

Se informa por primara vez en Mexico un caso de aneurisma subanular valvular del ventriculo izquierdo en un nino de 12 anos de edad. El diagnostico se sospecho clinicamente por la deformacion de la silueta cardiaca en la radiografia de torax; este diagnostico se apoyo con los hallazgos una camara aneurismatica a un este inmediatamente por abajo del anillo valvular mitral El pacientes se trato quirurgicamente con exito, y constituye el primer caso asi tratado que aparece en la literatura a nuestro alcance.La recuperacion postoperatoria fue muy satisfactoria

ABSTRACT

Se reportan las observaciones clinicas en veinte pacientes portadores de comunicacion interauricular con lesion unica, congenita, todos menores de dos anos. Las manifestaciones fueron de una cardiopatia grave como sintomatologia y evolucion que hicieron considerar diagnosticos diferentes y consecuentemente, indujo a errores en el diagnostico. La edad al inicio vario de 10 dias a 24 meses, con promedio de 17 meses, infecciones respiratorias, insuficiencia cardiaca y cianosis dominaron el cuadro clinico. El diagnostico se corroboro por cateterismo cardiaco a todos.Se intervino quirurgicamente con exito a tres pacientes por presentar insuficiencia cardiaca de dificil control, gran cardiomegalia e hipertension pulmonar grave. Se concluye que la comunicacion interauricular en el lactante puede caracterizar-se por un cuadro de gravedad de dificil diagnostico, el cual tiene que verificarse por quirurgico

ABSTRACT

Se estudiaron 15 lactantes con gastroenteritis aguda que les produjo deshidratacion sicion corregida de los grandes vasos (TCGV) en diez pacientes.En todos el diaga concentraciones de 75 mEg/1 (solucion glucosada y electrolitica 1:1), administrandose en ellas un promedio de 11.4 mEq/ kg/24 h. Se midio la excrecion de sodio en orina en varios periodos de 8 horas y se comparo con los resultados obtenidos de un grupo testigo de lactantes sanos cuya excrecion normal de sodio fue de 0.54 mEq/ kg/8 horas; se observo un incremento progresivo en la eliminacion del ion/kg/8h.ficos para ventriculo morfologicamente dedoble en el cuarto periodo y fue mucho mayor en el quinto (p < 0.05), con aparicion de edema en la tercera parte de ellos Segun los resultados obtenidos, se recomienda disminuir el aporte do sodio cuando la deshidratacion se ha corregido, empleado soluciones endovenosas en proporcion de 2:1 o de 3:1 (glucosada y electrolitica), lo que consideramos suficiente como tratamiento de sosten en los lactantes con diarrea

ABSTRACT

Se establecio por medio de ecocardiografia bidimensional el diagnosistico de transposicion corregia de los grandes vasos (TCGV) en diez paciente. En todos el diagnostico se confirmo por cateterismo cardiaco y angiografia. Se empleo el analisis segmentario en cada uno de los estudios ecocardiograficos, con los siguientes pasos: 1) Establecer el situs auricular mediante identificacion de la posicion de la vena cava (eje corto subcostal) y la posicion del apex (cuatro camaras, subcostal).2) Reconocimiento la discordancia auricular aplicando los criterios ecocardiograficos para entriculo morfologicamente derecho e izquierdo, y sus relaciones espaciales (cuatro camaras, apical y subcostal y ejes paraesternales corto y largo. 3) Identificar las grandes arterias y su discordancia ventricular las anomalias acompanantes. Este estudio demuestra que la ecocardiografia bidemensional mediante analisis segmentario constituye el recurso no invasor mas sensible para el diagnostico de la TCGV
